Metastatic epithelioid sarcoma on the arm: presentation of a case report.

Epithelioid sarcoma (ES) is a highly malignant soft tissue tumor characterized by its tendency to distant metastasis, regional lymph node involvement, and recurrence. ES frequently manifests as a deep dermal mass in the distal extremities of young adults. The variety of pathological morphology makes it clinically misdiagnosed as a granuloma or rheumatoid nodule frequently. In this case, we report a new case of a 56-year-old female who was diagnosed with metastatic ES on her left upper arm. The diagnosis was confirmed by pathological biopsy and immunohistochemistry analysis, and metastatic afte

A Case of Rheumatoid Nodulosis with Extensive Bone Erosion by a Rheumatoid Nodule in the Foot.

Rheumatoid nodulosis is a rare variant of rheumatoid arthritis, characterized by similar but milder joint and systemic symptoms compared to typical rheumatoid arthritis. We report a case of rheumatoid nodulosis in a 61-year-old woman with a 6-month history of swelling and pain in her right foot, with subsequent swelling on the back of her right hand, in her left wrist, and over the right tibial tubercle. Radiography showed well-defined radiolucent lesions in several bones of the right foot and the left hand, without narrowing of joint spaces. The 18F-fluorodeoxyglucose positron emission tomogr

Cutaneous Nodules and Inflammatory Arthritis: Two Illustrative Cases of Rare Mimics of Rheumatoid Arthritis.

Background: Rheumatoid arthritis is a relatively common rheumatic disease that can present with inflammatory arthritis and subcutaneous nodules. Multicentric reticulohistiocytosis and fibroblastic rheumatism are rarer entities that also present with these features. Methods: Two cases, one of each of fibroblastic rheumatism and multicentric reticulohistiocytosis, are described highlighting characteristic clinical, radiographic, and histologic findings. A narrative review of the literature on these rarer conditions, compared with rheumatoid arthritis, is provided with a focus on articular and cu

Revisit of Histopathology in Rheumatoid Lung Nodules.

Diagnosing pulmonary rheumatoid nodules (RN) is challenging because they are rare and share clinical, radiologic, and histopathologic features with other necrotizing granulomatous diseases such as infectious granulomas (IG) and granulomatosis with polyangiitis (GPA). Herein, we revisit the histopathologic features of RN and the findings in the adjacent lung tissue, in comparison with IG and GPA, to identify distinguishing features. Twenty-eight cases with surgically resected RN evaluated at our institution (1991 to 2024), 33 IG (10 mycobacterial infection, 10 histoplasmosis, 13 coccidiomycosis

Trapped Lung: A Rare Complication of Rheumatoid Arthritis.

Trapped lung is a rare complication of rheumatoid arthritis (RA), often resulting from chronic pleural inflammation and fibrosis. It presents as a non-expandable lung with persistent pleural separation despite drainage. We report a 67-year-old woman with seropositive RA and multiple co-morbidities, who presented with chronic left-sided hydro-pneumothorax to the respiratory clinic. Imaging showed pleural thickening, lung nodules, and pleural tethering in addition to the pneumothorax. Due to poor functional reserve, multiple lung nodules and stable symptoms, she was managed conservatively after

Recurrently Infected Rheumatoid Nodule Causing Posterior Interosseous Nerve Palsy: A Report of a Rare Case.

Rheumatoid nodules are typically benign and asymptomatic, requiring no specific treatment. However, we encountered a diagnostically challenging case involving a painful elbow mass that rapidly enlarged. The lesion eventually caused posterior interosseous nerve (PIN) palsy. Surgical excision revealed histological features of a rheumatoid nodule with marked neutrophilic infiltration, raising suspicion of superimposed infection. Despite initial negative cultures, the lesion recurred aggressively, and infection was later confirmed by pathogen isolation. After adjusting immunosuppressive therapy an

Rheumatoid nodule masquerading as a bone metastasis: a diagnostic case report.

INTRODUCTION AND IMPORTANCE: Rheumatoid arthritis (RA) presents with extra-articular manifestations in 15-25% of cases. Intra-osseous rheumatoid nodules occur in <1% of patients and mimic metastatic disease on imaging. When extra-articular features precede joint symptoms, diagnostic delays are common. CASE PRESENTATION: A 78-year-old male smoker presented with chronic cough and dyspnea. CT revealed interstitial lung disease and a pulmonary nodule. PET-CT demonstrated multiple FDG-avid osteolytic lesions in ribs, scapula, and spine (SUV 4.2-8.7), suggesting metastatic malignancy. Initial biops

Pedicled Omentoplasty for Treatment of Rheumatoid Nodule Pneumothorax: A Case Report.

Background  No consensus exists regarding surgical intervention for rheumatoid nodule-related pneumothorax. Clinical policy decisions rely on individual clinicians' experience and are usually intractable. Case Description  A 50-year-old man with a difficult-to-treat rheumatoid arthritis-related pneumothorax was successfully treated with pedicle omentoplasty without recurrence at approximately 2 years posttreatment. To the best of our knowledge, this is the first report of a patient where pneumothorax did not recur due to firm adhesions despite fluctuating postoperative rheumatoid nodules, as c

Rheumatoid Arthritis With Multiple Lung Nodules: A Case Report.

Rheumatoid arthritis (RA) is an inflammatory multisystemic disease characterized by erosive arthritis with many extra-articular manifestations. Pleuropulmonary manifestations are frequently seen in patients with RA. Risk factors include male gender, severe erosive arthritis, high titers of rheumatoid factor, subcutaneous nodules, smoking, genetic predisposition, and the presence of other extra-articular manifestations of RA. We report a patient known to have RA presenting with multiple lung nodules. A 35-year-old female patient, known to have seropositive RA, was diagnosed 10 years ago. She wa

Surgical Removal of Rheumatoid Nodule That Presented as a Thyroglossal Duct Cyst: A Case Report.

Rheumatoid nodules are extraarticular complications in rheumatoid arthritis (RA). This report details how a 63-year-old female patient with a history of lung and kidney cancer presented to the otolaryngology clinic with left submandibular region neck pain and a lesion that looked like a thyroglossal duct cyst on imaging. The patient also had accompanying joint pain. After the patient underwent a full workup including positron emission tomography and computed tomography scans, she underwent surgery to remove the suspected thyroglossal duct cyst via a Sistrunk procedure. Pathology indicated that

The prevalence and risk factors of rheumatoid arthritis-associated interstitial lung disease: a systematic review and meta-analysis.

BACKGROUND: Interstitial lung disease (ILD) is the most widespread and fatal pulmonary complication of rheumatoid arthritis (RA). Existing knowledge on the prevalence and risk factors of rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is inconclusive. Therefore, we designed this review to address this gap. MATERIALS AND METHODS: To find relevant observational studies discussing the prevalence and/or risk factors of RA-ILD, EMBASE, Web of Science, PubMed, and the Cochrane Library were explored. The pooled odds ratios (ORs) / hazard ratios (HRs) with 95% confidence intervals

Caplan Syndrome Mimicking Progressive Massive Fibrosis on CT: A Case Report.

This report presents a unique case of Caplan syndrome that mimicked accelerated progressive massive fibrosis. The patient, a former coal miner, had been diagnosed with coal worker's pneumoconiosis 15 years prior and had been treated for rheumatoid arthritis for over 20 years. Accelerated progressive massive fibrosis and the development of multiple nodules with cavitation in the basal lungs were subsequently observed on serial CT scans. Here, the CT manifestations of Caplan syndrome are highlighted in a case in which Caplan syndrome mimicked accelerated progressive massive fibrosis.

Beyond rheumatoid nodules in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease predominantly affecting synovial joints. Extra-articular manifestations, including skin involvement, can also occur. The most frequent cutaneous manifestation in RA patients is rheumatoid nodules, occurring in 20-30% of seropositive individuals. These nodules are typically firm, painless, and located on pressure points such as the hands and elbows. However, in a minority of cases, other skin conditions can complicate RA, notably palisaded neutrophilic granulomatous dermatitis (PNGD). PNGD presents as erythematous papules or pla

Rheumatoid Nodule on the Hallux.

A subcutaneous rheumatoid nodule is a superficial soft tissue lesion that generally affects areas of frequent mechanical contact, such as the extensor aspect of the extremities, occiput, and sacrum. Rheumatoid nodules of the foot are rare, occurring in only 1% of cases. Herein, we present a case of a rheumatoid nodule on the plantar side of the left hallux in a 65-year-old woman with rheumatoid arthritis. The tumor was a 15 × 10 mm, dome-shaped, nontender, elastic hard mass in the plantar aspect of the left hallux. The discovery of rheumatoid nodules provides crucial information for the treatm
