Nerandomilast treatment improves a case of progressive pulmonary fibrosis combined with severe plaque psoriasis: A case report from a clinical trial.

Psoriasis (PsO) and interstitial lung disease (ILD), especially progressive pulmonary fibrosis (PPF), are often associated with abnormal immune-activation and chronic inflammation. The management of patients with PsO existing with PPF remains challenging. There is an urgent need for treatment approaches that effectively manage the progression of both diseases and minimize treatment-related risks. We report the case of a 70-year-old male patient with PsO and ILD that progressed to PPF. The patient was initially managed for PsO with methotrexate, acitretin capsules, compound glycyrrhizin tablets

Therapeutic potential of zinc supplementation in rheumatoid arthritis: clinical evidence, molecular mechanisms, and formulation-specific effects.

Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disorder primarily affecting the synovial joints, characterized by persistent synovitis, progressive joint destruction, and systemic complications that contribute to significant morbidity. Despite advances in disease-modifying anti-rheumatic drugs and biologic therapies, many patients fail to achieve sustained disease control or experience treatment-related adverse effects, highlighting the need for safe adjunctive strategies. Zinc, an essential trace element with critical roles in immune regulation, antioxidant defense, and inflam

Ianalumab for the Treatment of Patients with Sjögren's Disease.

BACKGROUND: Sjögren's disease is a complex, systemic autoimmune disease with no approved targeted therapies. Ianalumab is an IgG1 monoclonal antibody with a dual mechanism of action, depleting B cells through antibody-dependent cellular cytotoxicity and inhibiting B-cell activation and survival through blockade of B-cell activating factor receptor. METHODS: We conducted two 52-week, double-blind, randomized, placebo-controlled trials involving patients with active Sjögren's disease. Patients in the NEPTUNUS-1 trial were assigned in a 1:1 ratio to receive subcutaneous ianalumab (300 mg monthly

Preclinical evaluation of a CD137-targeted PET tracer for detection of inflammatory arthritis.

Inflammatory arthritis (IA) is characterized by early T cell activation in synovial tissue, yet noninvasive methods to detect this process are limited. This study evaluated Al[18F]F-NOTA-BCP137, a CD137-targeted bicyclic peptide PET tracer, for imaging CD137+ activated immune cells in two mouse models of IA. CD137 expression was characterized in human RA synovium and arthritic mouse joints by transcriptomics, qPCR, and immunohistochemistry. Tracer specificity was assessed in vitro using activated PBMCs, and immune safety was evaluated in human CD137 transgenic mice. PET imaging was performed i

Bioactive M2 exosome hybrid nanocomposites for immune therapy of psoriasis.

Psoriasis is a chronic autoimmune skin disease characterized by persistent oxidative stress and sustained inflammation, affecting approximately 1-8% of the global population, with no curative therapy has been well-established to date. To address this, we developed a bioactive hybrid nanocomposite (Exo-Ce) by chemically conjugating M2 macrophage-derived exosomes with cerium oxide nanoparticles (CeNP), which was applied for synergistic and immunomodulatory therapy of psoriasis. This nanohybrid system effectively integrates the natural immunomodulatory properties of exosomes with the reliable ant

Risk assessment and prevention of disease progression: new concepts in bronchiectasis management.

Bronchiectasis management is traditionally guided by exacerbation frequency. Previous guidelines have recommended treatment escalation only in patients experiencing three or more exacerbations per year. In contrast, the 2025 European Respiratory Society (ERS) guideline introduces a paradigm shift by advocating individualised risk assessment to identify patients at high risk of future complications, instead of relying solely on prior exacerbation. High-risk features include severe daily symptoms, underlying aetiologies or comorbidities, such as chronic obstructive pulmonary disease, rheumatoid

Global, regional and national burden of pertussis in 1990-2023: a Global Burden of Disease study.

Despite long-standing immunization programs, pertussis remains a major cause of vaccine-preventable mortality, particularly among infants. Here we present a primary assessment of global pertussis incidence, mortality and disability-adjusted life years across 204 countries and territories from 1990 to 2023. We show that despite short-term pandemic-period fluctuations, the dominant global pattern from 1990 to 2023 was long-term decline: age-standardized mortality decreased by 61.2% and age-standardized incidence by 41.6%. Relative to the anomalously low pandemic-period estimates, incidence and m

Deep dynamical models of single-cell multiomic velocities predict loss-of-function and rescue perturbations in B cells.

Here we present DynaVelo, a generative neural ordinary differential equation model that learns the joint dynamics of gene expression and transcription factor (TF) motif activity from single-cell multiome data. DynaVelo learns a functional form of the vector field, such that the model can be perturbed in silico and not only used to draw trajectories. We show that DynaVelo recovers the bifurcating in vivo dynamics of wild-type murine germinal center (GC) B cells and reveals how these dynamics change under heterozygous loss of epigenetic regulators Arid1a and Ctcf. In silico perturbation yields d

Plant-derived extracellular vesicles deliver spermidine to reprogram macrophage metabolism and ameliorate rheumatoid arthritis.

Rheumatoid arthritis (RA) is driven by macrophage-mediated synovial inflammation closely linked to metabolic dysfunction, highlighting immunometabolism as a therapeutic target. Here, we engineered spermidine-loaded red cabbage-derived extracellular vesicles (RCEVs@SPD) as a plant-based nanoplatform to reprogram macrophage bioenergetics. In LPS-stimulated macrophages, RCEVs@SPD suppressed pro-inflammatory polarization and enhanced mitochondrial function, as evidenced by increased ATP production and mitochondrial membrane potential. Assay-based measurements further showed reduced extracellular a

Health-related quality of life in childhood arthritis: the impact of generic versus disease-specific instruments.

BACKGROUND: Health-related quality of life (HRQoL) data provided by children with arthritis and their parents is critically important to enable co-design of care and shared decision making. However, there is significant heterogeneity in how HRQoL is captured, as both generic (EQ-5D-Y-5L) and disease-specific instruments (JAMAR) are available. This study aimed to examine the concordance of the JAMAR and EQ-5D-Y-5L questionnaires in childhood arthritis. METHODS: The international UCAN CAN-DU study prospectively enrolled consecutive children with arthritis across Canada and the Netherlands. This

Ex vivo JAK and TNF inhibitor-modulated immune signatures guide first-line therapy selection in rheumatoid arthritis.

BACKGROUND: Rheumatoid arthritis (RA) is a heterogeneous autoimmune disease for which selecting the most appropriate first targeted therapy remains challenging. We integrated ex vivo immune-cell functional assays, targeted proteomics, clinical outcomes, and a three-dimensional (3D) synovial model to identify treatment-associated molecular signatures and evaluate their relationship with subsequent response to Janus kinase inhibitors (JAKi) or tumour necrosis factor inhibitors (TNFi). METHODS: Peripheral blood mononuclear cells and neutrophils from 48 b/tsDMARDs-naïve RA patients were cultured

The effect of symptomatic extra-articular manifestations of rheumatoid arthritis on disease activity and patient-reported outcomes: a cross-sectional and observational study.

INTRODUCTION: Rheumatoid arthritis (RA) is a chronic inflammatory disease that primarily affects the joints but also has extra-articular manifestations. Extra-articular manifestations (EAMs) in RA encompass a wide range of clinical findings that may significantly affect patients' quality of life. This study aimed to compare daily life, social function, and work productivity between RA patients with and without EAMs. In this study, we aimed to determine the differences in daily life, social life, and work between patients with and without EAM. METHODS: This cross-sectional and observational st

Cadmium exposure triggers a functional switch of APP to drive joint degeneration.

BACKGROUND: Cadmium (Cd) is a pervasive environmental heavy metal linked to various chronic pathologies; however, its specific role and underlying molecular mechanisms in osteoarthritis (OA) pathogenesis remain poorly characterized. METHODS: We performed a cross-sectional analysis using the NHANES database (2001-2018) to evaluate the association between blood Cd levels and OA risk. Hub targets were identified by integrating the CTD and GEO databases through network toxicology and machine learning algorithms. Single-cell RNA sequencing (scRNA-seq) was employed to resolve the cellular localizat

Spermidine, a chondrocyte apoptotic metabolite, delays osteoarthritis by regulating macrophage M2c polarization.

BACKGROUND: Chondrocyte apoptosis and synovial macrophage polarization represent two key pathological hallmarks of osteoarthritis (OA), but the interplay between them remains largely unknown. This study aimed to investigate the roles and mechanisms of the chondrocyte apoptotic secretome in mediating macrophage polarization during the pathological process of OA. METHODS: In vitro cultures of chondrocytes and macrophages, along with an in vivo surgically induced OA mouse model, were used to evaluate the effect of the chondrocyte apoptotic secretome. Nuclear Receptor Subfamily 1 Group D Member 1 Promotes Wound Healing in Diabetic Mice through Transcriptional Repression of Stimulator of Interferon Genes.

Diabetic foot ulcer is one of the most debilitating complications of diabetes mellitus, primarily driven by persistent inflammation and impaired angiogenesis. This study investigates the role of the circadian clock protein, nuclear receptor subfamily 1 group D member 1 (NR1D1), in diabetic wound healing and elucidates its underlying mechanism. Clinical and experimental analyses revealed that NR1D1 levels were significantly reduced in both the serum and cutaneous wounds of diabetic patients and mice, with down-regulated NR1D1 correlating with delayed wound closure. Functionally, NR1D1 knockout
