Extraintestinal manifestations (EIMs) are a significant disease burden in inflammatory bowel disease (IBD). However, large-scale epidemiological data from the Chinese population remain scarce. This nationwide, multicenter, retrospective cross-sectional study included 2,252 adult IBD inpatients from 10 clinical centers. Data on demographics, clinical features, and EIMs were collected and analyzed using descriptive statistics and logistic regression. Of the 2252 patients, 384 (17.1%) reported at least one EIM. Among these, data on temporal sequence were available for 310 patients, of whom 70 (22

Background and Objectives: Shoulder disorders-including rotator cuff disease, glenohumeral osteoarthritis (GH OA), and adhesive capsulitis-are among the most prevalent musculoskeletal conditions and are frequently refractory to conservative management. Platelet-rich plasma (PRP) and bone marrow aspirate concentrate (BMAC) have attracted increasing clinical interest as regenerative alternatives to corticosteroid injection, but a comparative appraisal of these two principal biologics across the full spectrum of shoulder pathology is lacking. Materials and Methods: This narrative review is based

Primary Sjögren's disease (pSjD) is a chronic systemic autoimmune disease in which immune-mediated pathology is not confined to exocrine glands but also affects multiple organ systems. Renal involvement constitutes a clinically meaningful manifestation of pSjD and may exert a considerable impact on disease progression, prognosis, and treatment selection. However, the pathogenic basis of renal injury in pSjD is still incompletely understood, and existing therapeutic approaches remain largely empirical. Available studies suggest that renal involvement in pSjD arises from the interaction of multi

BACKGROUND: Chronic pain affects approximately 20% of the global population. In primary care settings, chronic pain may account for up to 37.5% of weekly consultations. Most chronic pain is non-specific, complicating diagnosis and treatment. Kinesiophobia and movement avoidance may accompany chronic pain, necessitating a biopsychosocial approach to treatment. Pain neuroscience education and corrective exercise have been proposed as treatment strategies. This review aims to critically examine the relationships among kinesiophobia, movement avoidance, and chronic pain; assess the effectiveness o

Guttate psoriasis is often precipitated by strepto-coccal infections and is generally regarded as a mild subtype of psoriasis; however, its potential associations with systemic comorbidities are insufficiently explored. In this study, we aimed to evaluate the association of guttate psoriasis with systemic comorbidities such as metabolic syndrome and psoriatic arthritis and identify factors affecting recurrence. In this retrospective single-centercentre study, we reviewed the data of patients with the first eruption of guttate psoriasis between 1998 and 2020 and at least 4 years of follow-up da

This case describes a rare early-onset presentation in a 15-month-old girl, who first developed guttate psoriatic lesions followed by knee arthritis confirmed by ultrasound. The patient had a strong family history of autoimmune diseases. Immunological testing, including antinuclear antibodies, was negative, but with positive non-specific inflammatory markers, such as C-reactive protein and erythrocyte sedimentation rate, offering a clue about an inflammatory process causing this presentation. The diagnosis of juvenile psoriatic arthritis was made according to the International League of Associ

PURPOSE: The fucosyltransferase 2 gene (FUT2) is responsible for the regulation of mucosal blood type antigens which can act as receptors for bacterial adhesion and regulate intestinal flora. A homozygous nonsense mutation in this gene leads to the absence of these antigens, termed FUT2 non-secretor status, and is associated with the development of Crohn's disease (CD). The mechanism of this association is likely via changes in homeostatic symbiosis, thus promoting development of inflammation. However, the impact of FUT2 genotype on CD phenotype and outcomes is not known.
METHODS: Subjects we

The ketogenic diet (KD) is an established treatment for intractable epilepsy in children. KD has also become increasingly popular for weight loss, and it has shown therapeutic potential for treating neurodegenerative diseases. However, KD causes bone loss in epileptic children, and its effects on the adult skeleton are unknown. In the current study, we investigated the effect of KD on bone at different ages using growing (4-week-old) and adult (14-week-old) male C57BL/6 J mice. We also examined if KD-induced bone loss could be recovered via exercise. We hypothesized that KD would cause bone lo

OBJECTIVES: Systemic autoinflammatory diseases (SAIDs) arise from genetic defects in innate immunity, leading to dysregulated activation of inflammatory pathways, including interleukin (IL)-1, IL-6, TNF, and JAK/STAT. Clinical manifestations range from recurrent fever to severe complications such as encephalitis and AA amyloidosis. Management aims to control inflammation using immunosuppressive agents and targeted monotherapies (biologics or JAK inhibitors). Advanced combination therapy (ACT), defined as the use of biologics and/or JAK inhibitors in combination, has emerged as a strategy for r

INTRODUCTION: Hidradenitis suppurativa (HS)-related autoinflammatory syndromes, simply termed as syndromic HS (sHS), represent a group of rare immune-mediated inflammatory disorders in which HS coexists with systemic or cutaneous autoinflammatory features like PASH (pyoderma gangrenosum-PG-, acne and HS), PAPASH (PASH, pyogenic arthritis), PASS (PG, acne, HS, and ankylosing spondylitis), and SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, and osteitis). In recent years, genetic studies identified several novel pathogenic variants underlying sHS; however, most investigations rely exc

Systemic psoriasis treatment options have rapidly expanded in Japan, including the approval of tyrosine kinase 2 and interleukin-36 receptor inhibitors in 2022. However, contemporary real-world treatment patterns across plaque, erythrodermic, and generalized pustular psoriasis remain incompletely characterized. Using a Japanese hospital-based insurance claims database (Medical Data Vision Co. Ltd.), we conducted a retrospective cohort study for patients with these psoriasis subtypes who initiated systemic drug therapy between 2020-2023 and had a ≥ 12-month observation period. Overall, 3965 pat

BACKGROUND: Osteoarthritis (OA) is a leading cause of pain and disability in older adults. Sleep disturbance and depressive symptoms are common and have been linked to multiple chronic conditions, yet how their co-developing longitudinal patterns relate to OA onset remains unclear. We aimed to identify joint trajectories of sleep disturbance and depressive symptoms and examine their associations with incident OA.
METHODS: Data were drawn from the English Longitudinal Study of Ageing (ELSA), a nationally representative prospective cohort of community-dwelling adults aged ≥ 50 years. Sleep dist

Vascular Ehlers-Danlos syndrome (vEDS) is a rare connective tissue disorder characterised by vascular fragility and risk of arterial complications. Further overlap with autoimmune diseases and multisystem comorbidities creates unique medical management challenges. We present an early 60s female with vEDS suffering from extensive comorbidities including systemic lupus erythematosus, rheumatoid arthritis, fibromuscular dysplasia and more. Over 3 years as our patient, she required multiple surgical interventions (eg, hysterectomy, salpingo-oophorectomy) and multidisciplinary management for chronic

Pemphigoid diseases (PDs) are autoimmune disorders marked by autoantibodies (Aabs) against skin and mucous membrane proteins, causing mucocutaneous blistering in predominantly elderly patients. Since current therapeutics are often insufficient, PDs impose a significant morbidity and mortality burden. CD19×CD3 bispecific T cell engagers (TCEs)-originally developed for B cell malignancies-have shown promise in treatment-refractory autoimmune diseases such as systemic sclerosis and rheumatoid arthritis. To explore their potential in PDs, we tested a murine CD19×CD3 TCE in a mouse model of epiderm

Background and Objectives: Patients with rheumatoid arthritis (RA) are found to have a higher risk of dental diseases. Although herbal medicines (HMs) have long been used to treat various conditions, few studies focus on its impact on dental diseases. In this longitudinal cohort study, we assessed the correlation between HM use and risk of dental diseases in RA groups. Materials and Methods: A total of 2359 persons with RA aged 20-80 who were free of dental diseases between 2001 and 2010 were retrospectively enrolled from nationwide register-based data. They were then classified into HMs and n

Primary cutaneous histoplasmosis (PCH) is a rare fungal infection, often misdiagnosed due to its polymorphic skin manifestations. We describe a woman in her 60s with rheumatoid arthritis receiving infliximab and long-term prednisone who presented with progressive, non-healing lesions of the left forearm. Initially treated as cellulitis without improvement, further investigation led to a diagnosis of PCH due to Histoplasma capsulatum confirmed by fungal culture. She was treated with systemic antifungal therapy with clinical resolution. This case underscores the importance of considering endemic

OBJECTIVE: Hip osteoarthritis (OA) is characterised by increased remodelling of the subchondral bone, for which the underlying cellular and microvascular mechanisms are unclear. This study investigated alterations in the osteocyte lacunocanalicular network and vascular canal architecture in the femoral head in human hip OA, using a multimodal imaging and histological approach.
DESIGN: Femoral head samples from OA patients (n = 8; 4 female, 4 male; age 69 ± 14) undergoing hip replacement, and healthy controls (n = 8; 5 female, 3 male; age 64 ± 12), were imaged by MRI to assess cartilage volume
