Psoriasis: Which therapy for which patient, updated, part II. Focus on special populations and chronic infections.

Choosing systemic therapy for psoriasis remains challenging in populations underrepresented in clinical trials, including pregnant and pediatric patients, patients with chronic infections, and those with difficult-to-treat phenotypes. Since the 2019 version of this article, therapeutic options have expanded substantially, including new pediatric approvals, oral targeted agents, and biologics with increasingly phenotype-specific evidence. This second article in the continuing medical education series reviews systemic treatment selection for moderate-to-severe psoriasis in pregnancy, pediatric p

GM2A links keratinocyte metabolism to inflammation in psoriasis: Integrated transcriptomic and functional analyses.

Psoriasis is a chronic inflammatory dermatosis driven by abnormal keratinocyte proliferation and extensive immune cell infiltration. Emerging evidence links its pathogenesis to lipid metabolic dysregulation, yet the specific molecules bridging lipid remodeling to immune activation remain largely unknown. In this study, we integrated transcriptomic datasets through weighted gene co-expression network analysis (WGCNA), differential expression analysis, and ensemble machine learning (random forest [RF] and support vector machine recursive feature elimination [SVM-RFE]), identifying ganglioside GM

Zhuhuang Granules alleviate psoriatic inflammation via restoring gut microbiota balance and enhancing intestinal barrier integrity.

Psoriasis is a chronic inflammatory skin disorder characterized by excessive keratinocyte proliferation, immune dysregulation, and systemic inflammation. Emerging evidences highlight the gut-skin axis as a key regulatory mechanism linking gut microbiota dysbiosis to psoriasis pathogenesis. This study investigates the therapeutic effects of Zhuhuang Granules (ZH) on psoriasis, focusing on cutaneous inflammation, intestinal barrier integrity, and gut microbiota homeostasis. Using an imiquimod (IMQ)-induced psoriasis-like dermatitis mouse model, ZH treatment significantly reduced Psoriasis Area a

Hematological inflammatory markers as predictors of atherosclerotic progression in patients with psoriasis: A retrospective case-control study from the SH-YIMED database.

Chronic inflammation drives cardiovascular comorbidity in psoriasis. Clinical-only risk models have limited utility, whereas the predictive value of blood inflammatory markers for psoriatic atherosclerosis is unconfirmed. This hospital-based case-control study enrolled 636 patients with psoriasis who underwent vascular ultrasonography, and 134 matched pairs were obtained through 1:1 propensity score matching. We analyzed primary and exploratory hematological inflammatory markers, examined their links to atherosclerosis using logistic regression and supplementary stratified and nonlinear analys

Potential role of druggable gene SLC15A2 in keratinocyte hyperproliferation in psoriasis.

Psoriasis is a chronic inflammatory skin disorder marked by abnormal keratinocyte proliferation and persistent immune activation. Identifying genetically supported druggable genes may offer new avenues for therapeutic development. We performed Mendelian randomization and colocalization analyses to identify druggable genes associated with psoriasis. These findings were then validated using an imiquimod-induced psoriasis-like mouse model, single-cell transcriptomics, and in vitro functional assays. SLC15A2 was identified as a gene potentially associated with psoriasis and was upregulated in psor

Single-cell analysis reveals CXCL16+ dendritic cells and clonally expanded GZMK+ CD4 T cells driving joint inflammation in psoriatic arthritis.

OBJECTIVE: Psoriatic arthritis (PsA) is a chronic immune-mediated arthritis affecting approximately 30% of individuals with psoriasis. The mechanisms driving PsA joint inflammation remain incompletely defined. METHODS: To address this gap, we performed multimodal single-cell sequencing of immune cells from matched peripheral blood and synovial fluid (SF) from 6 PsA patients, with comparisons to a rheumatoid arthritis (RA) patient and 6 healthy controls (HC). RESULTS: SF CD4 and CD8 T cells displayed elevated expression of TOX, PDCD1, CTLA4, GZMK, CXCR3, and CXCR6, reflecting a tissue-specifi

Calcipotriol-betamethasone foam modulates serum and tissue IL-17A, IL-31, IL-31RA, and substance P in mild-to-moderate psoriatic itch: A clinical and immunopathological study.

Although itching is a common psoriasis symptom, its pathogenesis remains partially elucidated. We evaluated clinical pruritus features and associated serum and tissue cytokine profiles in 25 patients with mild-to-moderate psoriasis who were treated with calcipotriol-betamethasone foam over 4 weeks. Clinical scores comprised the PASI, Dermatology Life Quality Index, pruritus Visual Analogue Scale, and Itch Severity Scale. Serum and tissue levels of IL-17A, IL-31, IL-31RA, and substance P were evaluated before and after therapy. Most patients had severe pruritus and marked QOL impairment. PASI w

Nerandomilast treatment improves a case of progressive pulmonary fibrosis combined with severe plaque psoriasis: A case report from a clinical trial.

Psoriasis (PsO) and interstitial lung disease (ILD), especially progressive pulmonary fibrosis (PPF), are often associated with abnormal immune-activation and chronic inflammation. The management of patients with PsO existing with PPF remains challenging. There is an urgent need for treatment approaches that effectively manage the progression of both diseases and minimize treatment-related risks. We report the case of a 70-year-old male patient with PsO and ILD that progressed to PPF. The patient was initially managed for PsO with methotrexate, acitretin capsules, compound glycyrrhizin tablets

Bioactive M2 exosome hybrid nanocomposites for immune therapy of psoriasis.

Psoriasis is a chronic autoimmune skin disease characterized by persistent oxidative stress and sustained inflammation, affecting approximately 1-8% of the global population, with no curative therapy has been well-established to date. To address this, we developed a bioactive hybrid nanocomposite (Exo-Ce) by chemically conjugating M2 macrophage-derived exosomes with cerium oxide nanoparticles (CeNP), which was applied for synergistic and immunomodulatory therapy of psoriasis. This nanohybrid system effectively integrates the natural immunomodulatory properties of exosomes with the reliable ant

Microneedle-guided ultraviolet phototherapy with local calcipotriol delivery for enhanced psoriasis treatment.

Narrow-band ultraviolet B (NB-UVB) phototherapy is clinically regarded as a first-line treatment for moderate-to-severe plaque psoriasis owing to its high tolerance, broad applicability and easy affordability. However, the therapeutic efficacy of short-wave UV light is limited by poor penetration through the thickened stratum corneum of psoriatic skin, whereas excessive NB-UVB irradiation may provoke cutaneous photodamage. Moreover, NB-UVB administration necessitates trained personnel and frequent hospital visits, resulting in poor patient compliance. Here, we developed microneedles with high

A Case of Alopecia Secondary to Dupilumab in a Patient With Atopic Dermatitis.

Dupilumab inhibits the signaling pathway of interleukins 4 and 13, and can be effective in the management of treatment-refractory atopic dermatitis, with alopecia areata being uncommonly reported as a side effect and only limited to several cases. This is the case of a patient who experienced dupilumab-induced alopecia in treatment for atopic dermatitis and whose histological exam represented findings distinct from classical alopecia areata, telogen effluvium, or underlying psoriatic alopecia, with a potential explanation being inhibition of the Th2 pathway, enhancing a Th1-mediating response

Targeting Macrophage STING O-GlcNAcylation via Small-Molecule Allosteric Modulation for Psoriasis Therapy.

STING undergoes distinct post-translational modifications in autoimmune disorders, particularly in psoriasis. However, selectively targeting these disease-associated STING modifications remains a major therapeutic challenge. Here, we show that macrophage STING O-GlcNAcylation is a critical driver of psoriatic inflammation and identify the natural small-molecule paeoniflorin (PF) as a selective inhibitor of STING O-GlcNAcylation. Mechanistically, PF engages the cyclic dinucleotide-binding domain (CBD) of STING, inducing an allosteric conformational shift that traps STING in an inactive open sta

Impaired Myostatin Function Alters Body Composition Without Detectable Attenuation of Imiquimod-Induced Psoriasiform Dermatitis.

Psoriasis is a chronic inflammatory skin disease closely associated with obesity and metabolic dysfunction. While excess adiposity is considered a key risk factor, the independent contribution of body composition to psoriatic inflammation remains unclear. Myostatin negatively regulates skeletal muscle growth, and its inhibition leads to muscle hypertrophy and reduced adiposity. In this study, we investigated whether altered body composition due to impaired myostatin function affects imiquimod (IMQ)-induced psoriasiform dermatitis. Wild-type (WT) and dominant-negative myostatin transgenic mice
