Microneedle-guided ultraviolet phototherapy with local calcipotriol delivery for enhanced psoriasis treatment.

Narrow-band ultraviolet B (NB-UVB) phototherapy is clinically regarded as a first-line treatment for moderate-to-severe plaque psoriasis owing to its high tolerance, broad applicability and easy affordability. However, the therapeutic efficacy of short-wave UV light is limited by poor penetration through the thickened stratum corneum of psoriatic skin, whereas excessive NB-UVB irradiation may provoke cutaneous photodamage. Moreover, NB-UVB administration necessitates trained personnel and frequent hospital visits, resulting in poor patient compliance. Here, we developed microneedles with high

Targeting Cannabinoid 1 Receptor in Macrophages Alleviates Psoriatic Skin Inflammation Through the AUF1-NLRP3 Axis.

Psoriasis is a chronic inflammatory skin disease associated with systemic metabolic dysregulation; however, the molecular pathways linking metabolic signaling to skin inflammation remain poorly understood. Here, we identify cannabinoid 1 receptor (CB1R) in macrophages as a key immunometabolic regulator of psoriatic inflammation and a potential therapeutic target. CB1R expression was increased in macrophages infiltrating psoriatic lesional skin in both humans and mice, and both pharmacological inhibition and genetic deletion of CB1R markedly attenuated psoriasis-like pathology in mouse models b

A Multi-Axis Plantar Load Measurement Platform for the Assessment of Foot Disorders: Comparison of Patients With Psoriatic Arthritis to a Healthy Control Cohort.

BACKGROUND: Accurate assessment of plantar loading, including shear forces, is crucial for understanding foot pathologies such as dactylitis in psoriatic arthritis (PsA). Existing devices predominantly measure plantar pressures, neglecting important shear stresses which may initiate dactylitis. This study presents a novel multi-axis plantar load measurement platform to address this gap. METHODS: The study utilised a newly developed plantar load measurement system, comprising a Hall-effect-based magnetic sensing array, custom data logging electronics, and tailored data processing software. Temporomandibular disorders in women with psoriatic arthritis by history of psoriasis: a cross-sectional study.

Temporomandibular joint involvement in psoriatic arthritis may result in impaired jaw function and reduced oral health-related quality of life. Patients with PsA have generally been analyzed as a single group regardless of psoriasis history, despite a reported association between psoriasis and TMJ involvement. To assess TMD symptoms, patient-reported outcome measures, and self-reported beneficial effects of systemic therapies and TMD-related treatment modalities in women with psoriatic arthritis, stratified by psoriasis history. A digital questionnaire was sent to members aged ≥ 18 years of th

Eczema-Psoriasis Overlap: An eDelphi Consensus on Clinical Presentation and Therapeutic Management Among Italian Experts.

INTRODUCTION: Atopic dermatitis and psoriasis (PsO) are common chronic inflammatory skin diseases traditionally considered distinct, driven by Th2 and Th1/Th17 pathways, respectively. However, evidence reveals that some patients may present with both eczematous and psoriatic lesions, either simultaneously or sequentially. This study's objective was to establish an Italian consensus on the clinical definition and management of eczema-psoriasis overlap. METHODS: A steering committee of eight dermatologists identified three key areas: problem definition, clinical presentation and therapeutic man

A Dual-Action Microneedle Patch Inhibiting Keratinocyte Ferroptosis and Scavenging Cell-Free DNA for Psoriasis Treatment.

Psoriasis is a chronic inflammatory skin disorder characterized by dysregulated epithelial-immune interactions. Emerging evidence implicates keratinocyte ferroptosis as a critical pathological mechanism, whereby oxidative epidermal damage and elevated levels of cell-free DNA (cfDNA) amplify immune dysregulation and sustain chronic psoriatic inflammation. Here, we developed a dual-action microneedle system (CAD), with needle tips composed of guanidine-functionalized chitosan and deferoxamine (DFO), designed to eliminate pro-inflammatory cfDNA while locally releasing DFO for iron chelation. In a

Effects of Switching From Deucravacitinib to Tildrakizumab in Psoriasis: A 28-Week Real-World Study.

BACKGROUND: Deucravacitinib, a tyrosine kinase 2 inhibitor, has shown efficacy and tolerability in moderate-to-severe psoriasis. However, evidence on switching from deucravacitinib to an interleukin-23 inhibitor is limited. OBJECTIVE: We examined outcomes after switching from deucravacitinib to the anti-interleukin-23p19 antibody tildrakizumab. METHODS: We conducted a single-center retrospective study of 15 adults (aged ≥18 years) with psoriasis who showed an insufficient response to ≥16 weeks of deucravacitinib 6 mg. Insufficient response was defined as < 75% improvement of psoriasis area a

The epidemiology of inflammatory rheumatic diseases in Germany in a European context: a systematic review to support workforce planning.

OBJECTIVES: The number of adults living with inflammatory rheumatic diseases (iRMDs) in a country is key for calculating the demand for rheumatologists. Our aim was to estimate the incidence and prevalence of iRMD in Germany by compiling current epidemiologic German and other European data. METHODS: A systematic review included original studies on prevalence and incidence of iRMDs in Germany and other European countries, covering study data from 2013 to the present. Considering heterogeneous methodology, we qualitatively estimated the prevalence and incidence for 11 iRMDs and extrapolated the

Mitochondria-Nanozyme Hybrid Therapeutic for Modulation of the Inflammatory Cascade in Psoriasis.

The chronic recurrence of psoriasis is driven by a self-amplifying inflammatory cascade in which oxidative stress and mitochondrial metabolic dysfunction reinforce each other, and single-target conventional therapies often fail to break this cycle. Here, we present a micro-nano hybrid system, Mito-Ce, constructed by covalently immobilizing ceria nanozymes (CeNPs) onto the surface of isolated functional mitochondria through thiol-maleimide chemistry. This design endows the hybrid with dual functionalities, including mitochondrial bioactivity and durable ROS-scavenging capabilities of CeNPs. In

H2A.J is an epigenetic driver of IL-17-associated inflammation in TH17 cells and keratinocytes.

Psoriasis is a common autoinflammatory disease driven by excessive IL-17 expression. While the pathomechanism of the disease has been revealed, epigenetic factors contributing to psoriasis are less well understood. Here, we investigated the role of the poorly studied histone variant H2A.J in psoriasis. We found that H2A.J gets overexpressed in the skin of psoriasis patients, with even non-lesional skin showing increased expression compared to the skin of healthy donors. To gain mechanistic insights, we studied mice with a global H2A.J knockout (KO) and found them to be partially protected agai

Clinic-Based Proportion of Psoriatic Arthritis and HLA-B27 Distribution in Korean Psoriasis Patients: A Retrospective Study.

BACKGROUND: Psoriatic arthritis (PsA) develops in up to one-third of patients with psoriasis over the disease course, and the genetic factors are thought to modulate susceptibility. Although HLA-B27 has long been implicated in PsA, its clinical utility for identifying PsA in East Asian populations remains uncertain. OBJECTIVE: We aimed to estimate the clinic-based prevalence of PsA among Korean psoriatic patients, and to evaluate the diagnostic value of HLA-B27, along with other clinical markers, for PsA. METHODS: We conducted a retrospective, cross-sectional study of 209 patients with psori

Temporary Cessation of DMARD Therapy and COVID-19 Supplemental Dose Vaccine Immunogenicity in Patients With Inflammatory Arthritis: The COVER Randomized Clinical Trial.

IMPORTANCE: The COVID-19 pandemic disproportionately affected patients with autoimmune and rheumatic diseases. Understanding whether certain medications affect the response to a SARS-CoV-2 supplemental dose vaccination is critical for counseling and treating patients receiving targeted immunomodulatory medications. The effect of holding these medications around the time of COVID-19 vaccination on immunogenicity and rheumatic disease outcomes has not been well studied. OBJECTIVE: To assess whether temporarily holding biologic or targeted synthetic disease-modifying antirheumatic drug therapy i

Capsulectomy at Explant: Rethinking Spinal Cord Stimulator Infected Generator Pocket Management in the Immunocompromised-A Case Series.

BACKGROUND: Surgical site infections (SSI) associated with spinal cord stimulation (SCS) implants pose risks of morbidity, mortality, and increased healthcare costs. Immunocompromised patients are especially vulnerable due to impaired immune responses, delaying diagnosis and increasing complications. Current guidelines recommend SCS explantation for deep or inadequately treated superficial SSI, though the role of capsulectomy remains underexplored despite its use in other fields like plastic surgery and cardiology. CASE PRESENTATION: We present two cases of immunocompromised patients with SSI

Predictors at diagnosis for initiation of biologic or targeted synthetic disease-modifying anti-rheumatic drugs in patients with spondyloarthritis and psoriatic arthritis.

BACKGROUND: There is limited data on factors predicting the initiation of biologic and targeted synthetic disease modifying anti-rheumatic drugs (bDMARDs/tsDMARDs) in Spondyloarthritis (SpA) and Psoriatic arthritis (PsA). The relative importance of objective inflammation and other patient factors in this context is unclear. METHODS: A retrospective cohort study included consecutive patients diagnosed with SpA and PsA in 2012-2016 at a single rheumatology center. Data at diagnosis were obtained from medical records and from linkage to national and regional registers. Baseline predictors of b/t
