Nitroblue tetrazolium test scores were obtained in 43 patients with psoriatic arthritis, 32 patients with rheumatoid arthritis, and 16 patients with osteoarthrosis. The mean score in all groups was similar and was higher than previously reported in patients awaiting heart surgery or with noninfective dermatoses. The test is not thought to be helpful in differentiating psoriatic arthritis from rheumatoid arthritis, and its usefulness in the diagnosis of septic arthritis superimposed on other forms of arthropathy is limited.

Using sections from 10 early and five fully developed psoriatic lesions, we counted the mast cells in the papillary body. The results were evaluated statistically and compared with the findings of other researchers. The number of mast cells in the papillary body beneath the basal membrane shows a significant difference between early psoriatic lesions, fully developed psoriasis, and normal skin. Even in the very early lesion, the number of mast cells per mm3 is increased compared with the normal skin, though it is less than in fully developed lesions.

Mitotic nests have been noted both in tissue culture (Cone, 1968) and in human skin (Rowe and Dixon, 1975). The present paper is concerned with the disposition of mitoses in human tumor tissue. We describe the case of a patient with psoriasis, peripheral and axial arthropathy, and upper lobe fibrosis. This association of findings has not, to the best of our knowledge, been described in the past. Pulmonary fibrosis is not found to be more common in people with psoriasis than would be expected in a control Veterans Administration population and, while this association may be coincidental, pulmonary fibrosis in psoriatic spondylitis should be searched for in other patients.

In an open investigation on 50 patients with various dermatological lesions 17 (81%) out of 21 psoriatics and 28 out of 29 other patients inproved considerably or were clinically cured after three weeks' treatment with an ointment containing 0-05% betamethasone dipropionate and 3-0% chinoform. Among the patients chosen for the study 34 had previously tried topical corticosteroids without success. Of these 31 improved markedly during the steroid trial.

A double-blind comparison of the effectiveness of 0.25% desoxymethasone and 0.05% betamethasone 17,21-dipropionate creams was carried out in 40 patients with symmetrical, chronic psoriatic lesions. The lesions were pre-treated for 1 week with an inactive cream base and then the trial preparations were applied, without occlusion, to one or other side at random twice daily for 21 days. Overall response to treatment and the effect of the two topical steroids on scaling, induration, erythema, and pruritus were assessed at the start of and 4, 7, 14, and 21 days after the start of active treatment.

The folic acid antagonists, aminopterin and then methotrexate, have been the primary systemically administered drugs for the treatment of severe psoriasis for 25 years. Methotrexate was finally approved by the FDA 5 years ago and was recently estimated to be used for approximately 25,000 psoriatic patients annually. Results of treatment of psoriasis and various keratinizing dermatoses with an orally effective, aromatic retinoid are presented. Results in 39 of 55 psoriatic patients ranged from satisfactory to excellent. All side effects are restricted to hypervitaminosis A. The therapeutic margin of this new retinoid is clearly better than that of natural vitamin A and vitamin A acid preparations and other previously tested synthetic retinoids. Even after long-term treatment of 14 patients for more than 2 years no side effects of vital importance have as yet appeared.

In earlier studies we have shown that there is a significant prolongation of DNA-synthesis time (ts) in the epidermal cells of fully developed psoriatic lesions. The present study shows that this prolongation is to be observed even in very early plaques. The prolongation of ts precedes the development of acanthosis.
