Drug retention of TNF versus IL-23 inhibitors in psoriatic arthritis: a multicenter real-life cohort study.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory disease characterized by musculoskeletal and cutaneous involvement. While TNF inhibitors (TNFi) are commonly used as first-line biologic therapies, treatment failure is frequent. IL-23 inhibitors (IL23i) represent an alternative mechanism of action (MoA), but direct real-world comparisons between these drug classes remain limited. OBJECTIVES: To compare the effectiveness of TNFi versus IL23i in real-life PsA patients. DESIGN: This multicenter retrospective observational study is part of the BIRRA (BIologics Retention Rate Assess

Tumor necrosis factor-alpha-mediated inflammatory bone loss: Pathogenic mechanisms and therapeutic potential of its inhibitors.

Bone remodeling preserves skeletal integrity through the balanced actions of bone-resorbing osteoclasts and bone-forming osteoblasts, regulated by mechanical, endocrine, and immune signals. Tumor necrosis factor-alpha (TNF-α), a key pro-inflammatory cytokine, disrupts this balance by enhancing osteoclast differentiation and activation in synergy with receptor activator of nuclear factor κB ligand, leading to net bone loss, microarchitectural deterioration, and increased fracture risk. Epidemiological studies consistently demonstrate that TNF-α-mediated inflammatory diseases are associated with

Real-world performance of EULAR and GRAPPA definitions of difficult-to-treat psoriatic arthritis within the ORCHESTRA cohort: an observational cohort study.

BACKGROUND: Psoriatic arthritis (PsA) is a complex inflammatory disease with significant musculoskeletal, dermatological, and comorbidity burden. Despite therapeutic advances, many patients remain difficult to treat (D2T). Conventional definitions often overlook distinctions between inflammatory and non-inflammatory drivers, risking inappropriate treatment escalation. Two consensus frameworks-the European Alliance of Associations for Rheumatology (EULAR) and the Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA)-have recently been proposed to better capture this co

Mesenchymal stromal cell-derived extracellular vesicles (MSC-EVs) as a novel topical immunomodulatory therapy for psoriasis: bridging the therapeutic gap in moderate disease.

Psoriasis is a chronic, immune-mediated inflammatory disease that affects approximately 2%-3% of the global population, and remains a major dermatologic and psychosocial burden. Despite advances in biologics targeting interleukin 17 (IL-17) and interleukin 23 (IL-23) pathways, effective and accessible treatment options for moderate psoriasis are lacking. Topical therapies and phototherapy are often inadequate, while systemic agents and biologics are limited by toxicity, high cost, and restricted reimbursement criteria, leaving patients with moderate disease without adequate therapeutic options

Assessment of PPD Conversion Rate in Patients Receiving Tumor Necrosis Factor-alpha Inhibitor Drugs.

BACKGROUND & OBJECTIVE: Tumor necrosis factor-alpha (TNF-α) inhibitors are widely used in rheumatologic diseases but may increase the risk of primary tuberculosis (TB) infection or reactivation. Purified protein derivative (PPD) conversion is an important indicator of latent TB in immunosuppressed patients. This study aimed to determine the rate of PPD conversion and associated factors in patients receiving anti-TNF therapy. METHODS: This prospective study included adults with rheumatologic diseases who initiated anti-TNF therapy between March 2021 and September 2023. Patients with prior TB,

Dermal mesenchymal stem cells promote angiogenesis in HMEC-1 via activation of the angiopoietin 1/Tie2 pathway in psoriasis.

BACKGROUND: Abnormal proliferation of microvessels in the superficial dermis is a key pathological feature of psoriasis. The angiopoietin 1 (Ang1)/Tie2 axis regulates angiogenesis and is closely associated with the cellular microenvironment. Our previous studies demonstrated that dermal mesenchymal stem cells (DMSCs) derived from psoriatic lesions can promote angiogenesis in human umbilical vein endothelial cells; however, the link between this proangiogenic effect and the Ang1/Tie2 axis remains unclear. OBJECTIVE: To investigate the potential role of Ang1 in DMSC-mediated promotion of angiog

Economic Impact of Timely Systemic Treatment of Psoriatic Disease in Comorbidity Risk Reduction: Literature Review and (Early) Economic Assessment.

INTRODUCTION: Psoriatic disease, including psoriasis and psoriatic arthritis, is associated with increased risk of comorbidities (cardiovascular disease, mental health disorders, type 2 diabetes mellitus and obesity), placing substantial burden on patients and health systems. We aimed to quantify the lifetime economic burden of these four comorbidities in populations with moderate-to-severe psoriasis and psoriatic arthritis. We subsequently estimated the potential comorbidity-related gross cost offsets from timely systemic therapy across three diverse country settings (Denmark, the USA and Vie

Pemigatinib-Associated Psoriasis in a Patient With Cholangiocarcinoma.

Psoriasis is a chronic autoimmune skin disease that may be triggered or exacerbated by medications, including targeted oncologic therapies. Pemigatinib is a fibroblast growth factor receptor inhibitor used for the treatment of cholangiocarcinoma. We present the case of a 61-year-old woman with unresectable cholangiocarcinoma and lung metastases who developed psoriatic plaques after four cycles of pemigatinib. She had a remote history of limited psoriasis but no other new triggers. Her presentation included occipital scalp and leg plaques. Topical therapies were prescribed; however, follow-up r

High Patient Willingness to Grant Broad Consent for Real-World Data Use in Rheumatology-Implications for Real-World Data Platform Governance: Cross-Sectional Study.

BACKGROUND: Medical real-world data (RWD) are often siloed across organizations, making them inaccessible for research. Unlocking these data could advance clinical research and patient care. The pan-European Data Nexus platform (DNP) links RWD, facilitating its use, for example by artificial intelligence (AI) tools, to support the generation of real-world evidence. In Europe, particularly Germany, the secondary use of health data is governed by stringent regulatory requirements, including informed consent. OBJECTIVE: This study evaluated the informed broad consent form for the RWD (Data Nexus

New Tuberculosis Infection and Reactivation of Tuberculosis in Patients with Psoriasis or Psoriatic Arthritis Receiving IL-23 Inhibitors: A Systematic Literature Review.

Biologic agents, including the interleukin-23 (IL-23) inhibitors (IL-23is) guselkumab, risankizumab, and tildrakizumab, are effective therapies for the treatment of psoriasis (PsO) and psoriatic arthritis (PsA). However, the risk of latent TB infection (LTBI) reactivation or new tuberculosis (TB) infection is of question, given documented evidence of occurrence of cases in patients with PsO and PsA treated with biologics with other mechanisms of action, and the relatively high prevalence of TB in some Asian Pacific (APAC) countries. We conducted a systematic literature review to summarize publ

Ixekizumab Treatment Persistence in Psoriatic Arthritis: Response to Joaquin et al.

We appreciate the interest of Joaquin et al in our recent study on the treatment persistence of ixekizumab in psoriatic arthritis. In response, we clarify that our study utilized prospectively collected real-world clinical data from a multicenter observational cohort, rather than administrative databases. We emphasize the importance of persistence, as assessed through the Drug Retention Rate (DRR), a validated measure in observational studies of biologic therapies. The DRR reflects a complex interplay of factors, including efficacy, safety, and patient choice, making it a valuable indicator of

The potential role of the gut microbiota in the development of autoantibodies associated with Spondyloarthritis: a narrative review.

Spondyloarthritis (SpA) is a set of immune-inflammatory conditions characterized by musculoskeletal and extra-articular manifestations. Increasing evidence indicates that alterations in the gut microbiota (dysbiosis) may influence both mucosal and systemic immune responses, potentially contributing to the loss of tolerance and the development of autoantibodies in SpA.This narrative review examines the current evidence linking gut dysbiosis to autoantibody development in SpA, with particular focus on ankylosing spondylitis (AS) and psoriatic arthritis (PsA).We summarized key mechanistic pathway

Lymphocyte-Platelet and Neutrophil-Platelet Aggregates are Associated with Dysregulated Platelet Inflammatory Pathways in Psoriasis.

BACKGROUND: Psoriasis is a chronic immune-mediated inflammatory disease associated with heightened cardiovascular risk. Platelets are increasingly implicated in this link through their capacity to amplify vascular inflammation and interact with leukocytes. Circulating leukocyte-platelet aggregates are elevated in psoriasis, although the biological significance of these aggregates remains incompletely understood. We investigated whether increased leukocyte-platelet aggregates is associated with alterations in the platelet transcriptomic profile in psoriasis. METHODS: Leukocyte-platelet aggrega
