Disrupting the adiposopathy-inflammation loop: a translational immunometabolic framework for inflammatory disease.

Adipose tissue dysfunction is increasingly recognized as a biologically active contributor to chronic inflammatory disease beyond its traditional role in energy storage. In the context of obesity, adiposopathy is characterized by persistent immunometabolic alterations involving adipokine dysregulation, macrophage infiltration, insulin resistance, and chronic low-grade inflammation. These processes intersect with inflammatory signaling pathways central to immune-mediated diseases, supporting the concept of an immunometabolic phenotype in which metabolic dysfunction and immune activation converg

Obstructive Sleep Apnea as a Modifiable Contributor to Chronic Pain in Rheumatologic Disease: The Potential Role of Continuous Positive Airway Pressure Therapy.

Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder marked by recurrent episodes of upper-airway obstruction, leading to intermittent hypoxia, recurrent arousals, and fragmented sleep. It is highly prevalent worldwide and remains underdiagnosed, particularly when symptoms such as fatigue, poor concentration, and nonrestorative sleep overlap with obesity, chronic pain, depression, or inflammatory disease. These diagnostic challenges are clinically relevant in rheumatology, where pain, fatigue, poor sleep, and immune activation frequently coexist. This narrative review exa

The clinical characteristics, mechanism and management of immune checkpoint inhibitor-related arthritis.

BACKGROUND: Immune-related adverse events, notably arthritis (irAE-arthritis), frequently occur in patients receiving immune checkpoint inhibitors. Arthritis severity varies from mild to severe, adversely impacting quality of life. Despite reports in clinical trials and real-world studies, the pathophysiology and optimal management of irAE-associated arthritis (irAE-arthritis) are still unclear. METHODS: From the inception to September 25, 2025, a search was conducted on PubMed, EMBASE, and MEDLINE for case reports/series on irAE-arthritis. FINDINGS: The most common rheumatic irAEs were arth

Risk of nonmelanoma skin cancer in patients with psoriatic arthritis and according to bDMARD treatment-a cohort study from the Nordic Arthritis Research Collaboration.

OBJECTIVES: To investigate if psoriatic arthritis (PsA) itself and if biological disease-modifying antirheumatic drugs (bDMARDs) used in PsA are associated with increased risks of nonmelanoma skin cancer (NMSC). METHODS: From nationwide health and clinical rheumatology registers in 4 Nordic countries, we identified patients with PsA who between 2010 and 2021 started a: (i) tumour necrosis factor inhibitor (TNFi), or (ii) non-TNFi bDMARD, or were (iii) b/targeted synthetic (ts)DMARD naïve. For Sweden and Denmark, we identified incident patients with PsA (2010-2021) and corresponding general po

Long-Term Effectiveness of Repeated, Single-Day Intravenous Ketamine Infusions for Severe Arthritis: A Retrospective Cohort Study.

BACKGROUND: Ketamine, either adminstered as an infusion or orally as an analgesic, was considered only to be a drug for neuropathic pain. More recently it has been speculated to be efficacious for other types of pain, including nociceptive pain. OBJECTIVES: We aimed to examine the long-term effects of single-day ketamine infusions for treating diffuse chronic pain caused by rheumatoid arthritis, osteoarthritis, or psoriatic arthritis. STUDY DESIGN: Our study analyzed the records of a cohort of 38 consecutive patients treated with repeated single-day intravenous ketamine infusions for chronic

Perioperative Management of Antirheumatic Medications and Their Effects on Wound Healing in Orthopaedic Surgery.

Orthopaedic surgeons frequently treat patients on antirheumatic medications for rheumatoid arthritis, psoriatic arthritis, lupus, and other inflammatory conditions. When these patients require surgery, surgeons face competing therapeutic priorities: continuing medications risks impaired wound healing and infection, whereas discontinuing them may trigger disease flares and functional decline. Management decisions are further complicated by inconsistent guidelines and limited data on newer biological therapies. Antirheumatic medications disrupt wound healing at multiple stages, some affect early

Dual‑Energy CT in Inflammatory Arthritis: A Comprehensive Review.

Inflammatory arthritis and crystal-associated arthropathies frequently present with overlapping clinical features, creating diagnostic and phenotypic challenges when conventional imaging findings are inconclusive. Dual-energy computed tomography (DECT) extends standard CT by enabling material-specific characterization, allowing differentiation of monosodium urate and calcium-containing deposits, and supporting selected applications beyond crystal detection. This narrative review synthesizes evidence from a curated set of PubMed-indexed studies to examine DECT principles, workflow consideration

Bimekizumab and progressive pulmonary fibrosis in severe psoriasis: a case report.

Psoriasis is a chronic immune-mediated inflammatory disease frequently associated with systemic comorbidities, including an increased prevalence of interstitial lung diseases (ILDs). Progressive fibrosing ILDs are characterized by declining respiratory function and high morbidity, and emerging evidence suggests that type-17 immune pathways may contribute to both psoriatic inflammation and pulmonary fibrosis. Bimekizumab, a monoclonal antibody selectively targeting IL-17A and IL-17F, has demonstrated high efficacy in psoriasis, although its potential impact on fibrotic lung disease remains uncl

A Case Series of Raynaud's Phenomenon in the Setting of Immune Checkpoint Inhibitors.

Although immune checkpoint inhibitors (ICIs) have improved oncological outcomes, they can cause a myriad of inflammatory conditions known as immune-related adverse events (irAEs). Raynaud's phenomenon (RP) is a rare rheumatologic irAE that may arise de novo or present as an exacerbation of preexisting disease. This case series aims to describe ICI-induced RP in patients without a rheumatological diagnosis seen at a large, specialized cancer center. Our case series describes five patients with ICI-induced RP who were evaluated by rheumatology at a cancer center during the past 3 years where the

Nrf2 as a redox checkpoint in autoimmune joint inflammation: microenvironmental redox control across the arthritis spectrum.

Arthritis comprises a spectrum of immune-mediated joint disorders, with rheumatoid arthritis (RA) representing prototypic autoimmunity and psoriatic arthritis (PsA) and ankylosing spondylitis (AS) spanning an autoinflammation-autoimmunity continuum. Across this spectrum, oxidative stress and inflammatory signaling reinforce each other within synovial/entheseal niches, sustaining immune activation and progressive structural damage. Excess reactive oxygen species (ROS) injure chondrocytes and synoviocytes, activate NF-κB and the NLRP3 inflammasome, and reprogram stromal-immune interactions; infl

Clinical-psychosocial archetypes predict short-term outcomes in inflammatory arthritis: an unsupervised segmentation study.

BACKGROUND: Heterogeneity in inflammatory arthritis (IA) outcomes limits the effectiveness of non-individualized treatment approaches, and digital health platforms can capture psychosocial and behavioral signals that may stratify responses beyond diagnosis or baseline severity. OBJECTIVE: To identify clinically interpretable patient clusters and evaluate their associations with 12-week outcomes in IA, testing whether cluster membership adds information beyond demographics, diagnosis, and baseline symptom burden. METHODS: We retrospectively analyzed the use of a CE-certified rheumatology appl

TaILOR: a randomised trial to compare the clinical and cost-effectiveness of a patient-initiated follow-up (PIFU) strategy compared to standard care pathways in people with inflammatory arthritis: a study protocol.

BACKGROUND: Inflammatory arthritis comprises lifelong conditions, such as rheumatoid arthritis, axial spondyloarthritis and psoriatic arthritis that require long-term treatment and regular monitoring. People with inflammatory arthritis usually require long-term treatment with immune-suppressing medications and are typically reviewed in outpatient clinics every 6-12 months, accounting for 1.3 million appointments/year in the UK. The National Health Service (NHS) is implementing a move to patient-initiated follow-up (PIFU) where patients request appointments as required. To date, there are small
