Influence of the shared epitope on the effectiveness of biologic antirheumatic agents in European patients with rheumatoid arthritis.

BACKGROUND: For rheumatoid arthritis (RA), the HLA-DRB1 'shared epitope' (SE) allele has been proposed as a predictive biomarker for the effectiveness of mainly abatacept and other biologic disease-modifying antirheumatic drugs (bDMARDs). OBJECTIVE: To explore the relative impact of SE positivity on the effectiveness of different bDMARDs in European patients with RA. METHODS: Nested cohort study in the Swiss (Swiss Clinical Quality Management) and Danish (DANBIO) rheumatology registries. Patients with RA who initiated bDMARD treatment and had available DNA samples to define SE were included.

Development and validation of a multi-institutional electronic juvenile idiopathic arthritis phenotype.

BACKGROUND: Ascertaining a true cohort of juvenile idiopathic arthritis (JIA) patients using claims and electronic health record (EHR) data is challenging due to clinical complexities. This study aims to develop a computable JIA phenotype using large databases with multi-site validation and replication. METHODS: This multi-institutional study used reporting resources at three sites identified through the Maternal and Pediatric Precision in Therapeutics Hub. Subjects with ICD-9 and ICD-10-CM codes for JIA and/or psoriatic juvenile arthropathy and age ≤20 at first eligible encounter were includ

Emotional unawareness in psoriatic arthritis: decoding the impact of alexithymia on pain perception, disease activity, and treatment response.

OBJECTIVES: Psychological distress influences pain and disease severity perception among patients affected by psoriatic arthritis (PsA). Alexithymia, a personal trait characterised by the difficulty in recognising and articulating emotions, has been observed in other rheumatic diseases, but its role in PsA remains underrecognised. We investigated the prevalence of alexithymia in PsA and its association with disease activity, psychological burden, and treatment complexity. METHODS: A cross-sectional observational study was conducted across three Italian rheumatology centers, enrolling PsA pati

Ixekizumab in children with active psoriatic and enthesitis-related juvenile idiopathic arthritis (COSPIRIT-JIA): a multicentre, open-label, 16-week, Bayesian trial including a randomised reference group to adalimumab.

BACKGROUND: Juvenile psoriatic arthritis and enthesitis-related arthritis are two categories of juvenile idiopathic arthritis (JIA). Despite available treatments including non-steroidal anti-inflammatory drugs, glucocorticoids, conventional synthetic disease-modifying antirheumatic drugs (DMARDs), and biological DMARDs, a substantial proportion of people do not adequately respond to treatment or do not have long-lasting clinical remission. The aim of this trial was to show the efficacy and safety of ixekizumab in children with active enthesitis-related arthritis and juvenile psoriatic arthriti

Turner syndrome as a state of increased autoimmune susceptibility: evidence from rheumatic diseases.

Turner syndrome (TS) is a chromosomal disorder of females due to the partial or complete absence of an X chromosome. Growing evidence suggests a significant association between TS and autoimmune disorders, especially rheumatic diseases like juvenile idiopathic arthritis (JIA) and systemic lupus erythematosus (SLE). Recognition of this association is important for early detection, timely intervention, and avoidance of complications. The aim of this narrative review is to analyze the association between TS and rheumatic diseases in terms of epidemiological, clinical, and pathophysiological aspec

Gender differences in health-related quality of life in axial spondyloarthritis, psoriatic arthritis and systemic sclerosis: a scoping review.

BACKGROUND: Axial spondyloarthritis (AxSpA), psoriatic arthritis (PsA) and systemic sclerosis (SSc) substantially affect health-related quality of life (HRQoL). Gender-related factors may influence health behaviours, coping and access to care. Yet, studies often do not distinguish sex from gender or specify how these variables are measured, limiting insight into gender as a multidimensional construct. OBJECTIVES: To map how gender is conceptualised and operationalised in HRQoL studies among patients with AxSpA, PsA and SSc, and to summarise HRQoL differences between binary sex/gender groups.

Impact of IL-23 inhibition versus methotrexate on select major fracture risk and progression to joint arthroplasty in psoriatic patients.

INTRODUCTION: Psoriasis is a chronic inflammatory skin disease linked to skeletal complications such as osteoporosis and fractures. Whether IL-23 inhibition via risankizumab (RZB) confers different long-term bone and joint outcomes than the conventional systemic agent methotrexate (MTX) remains uncertain. We aimed to compare patient characteristics between psoriatic adults initiating MTX versus RZB and to evaluate fracture and joint arthroplasty outcomes at 90 days, 2 years, and 5 years. METHODS: We performed a retrospective cohort study using the TriNetX network. Adults ≥18 years with psoria

Herring roe PLs promote SPM biosynthesis in macrophages and a keratinocyte/fibroblast coculture as a model of psoriasis.

Psoriasis is a chronic immune-mediated inflammatory disease affecting the skin, presenting with both local and systemic inflammation as part of its pathophysiology. An oil rich in phospholipids extracted from herring roe has been shown to have immunomodulatory properties and to improve clinical symptoms and impact inflammatory cytokine pathways in psoriasis in a clinical trial. The lipidic nature of herring roe oil (HRO) and its high content of PUFAs suggest the involvement of lipid mediator pathways for the observed alleviation of psoriatic inflammation. Of particular interest is the superfam

Atypical Vesicular Skin Eruption Following a Low-Dose Methotrexate Dosing Error: A Case Report.

Methotrexate is widely used to manage various autoimmune conditions, including psoriasis and psoriatic arthritis. Although weekly low-dose regimens are generally well-tolerated, dosing errors, particularly daily instead of weekly administration, can result in acute methotrexate toxicity with mucocutaneous and hematologic complications. We present a case of clinically significant methotrexate toxicity manifesting with painful oral mucositis and vesicular cutaneous lesions on the right lateral abdomen and right supraclavicular region in a 68-year-old Caucasian female patient with psoriasis and p

Investigating the relationship between psoriatic arthritis, multiple long-term conditions, treatment burden and adverse clinical outcomes: A systematic review protocol.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory musculoskeletal disease associated with psoriasis, affecting an estimated 0.13% of adults worldwide. People living with PsA often experience multiple long-term conditions (MLTCs) or multimorbidity (the presence of two or more long-term health conditions), such as hypertension, diabetes, obesity, and metabolic syndrome. Multimorbidity and treatment burden which is the workload of healthcare experienced by individuals and its impact on wellbeing may exacerbate poor health outcomes and complicate disease management in PsA. However, t
