Recent Advances in the Pharmacological Management of Pediatric Spondyloarthritis.

Juvenile spondyloarthritis (JSpA) refers to a heterogeneous group of inflammatory diseases affecting peripheral joints and the axial skeleton during childhood and adolescence. Enthesitis-related arthritis (ERA) and juvenile psoriatic arthritis (JPsA), subtypes of juvenile idiopathic arthritis (JIA) under the International League of Associations for Rheumatology (ILAR) classification, are included in this group. They may be associated with human leukocyte antigen (HLA) B27 positivity. Apart from musculoskeletal involvement, pathologies that accompany the disease, such as psoriasis, uveitis, and

The promise of biomarkers in Psoriatic Arthritis: Moving towards precision medicine.

Psoriatic arthritis is a heterogeneous disease where delayed diagnosis and variable treatment responses remain significant challenges. This review addresses how interconnected metabolic, immunological, and genetic pathways provide opportunities for precision medicine. We examine a distinct metabolic endotype involving altered apolipoproteins and gut metabolites, alongside immunological signatures such as pathogenic tissue-resident memory T cells. Genetic and multi-omic analyses, supported by machine learning, highlight susceptibility loci and signaling pathways that refine disease heterogeneit

Prevalence of interstitial lung disease in patients with psoriatic arthritis: a systematic review and meta-analysis.

BACKGROUND: Interstitial lung disease (ILD) is not classically considered an extra-articular manifestation of psoriatic arthritis (PsA). However, emerging evidence suggests that pulmonary involvement may be more common than previously recognized. The prevalence of ILD in PsA remains poorly defined. OBJECTIVES: To estimate the pooled prevalence of ILD among patients with PsA and to evaluate associated risk factors and comparative risk with other populations. METHODS: We conducted a systematic review and meta-analysis registered in PROSPERO. MEDLINE (PubMed), EMBASE (Scopus), and the Cochrane

Cancer risk of Janus kinase inhibitors and other advanced therapies in immune-mediated inflammatory diseases: a systematic review and Bayesian network meta-analysis of RCTs.

OBJECTIVES: The Oral Rheumatoid Arthritis Trial Surveillance trial identified increased malignancy risk with tofacitinib vs tumour necrosis factor inhibitors (TNFis) in rheumatoid arthritis (RA), leading to class-wide regulatory warnings for Janus kinase inhibitors (JAKis). Comparative cancer risk vs other advanced therapies and across immune-mediated inflammatory diseases (IMIDs) remains uncertain. The objective was to estimate relative and absolute cancer risk associated with JAKi vs other advanced therapies across IMIDs. METHODS: MEDLINE, Embase, Cochrane Library, and ClinicalTrials.gov we

Predicting and preventing clinical psoriatic arthritis.

Psoriatic arthritis (PsA) develops in up to 30% of patients with psoriasis (PsO) and causes pain, disability and impaired quality of life despite effective therapies. Because PsO usually precedes PsA, psoriasis provides an identifiable at-risk population in whom prevention and interception strategies can be tested. This narrative review summarizes risk factors and predictors of transition from PsO to PsA, including genetic background, psoriasis severity and distribution, nail disease, obesity, mechanical stress and trauma, subclinical enthesitis, musculoskeletal symptoms and candidate biomarke

Excess cardiovascular morbidity in psoriatic arthritis and cardioprotective effects of biologic dmards: a propensity-matched analysis.

To quantify the cardiovascular burden in patients with psoriatic arthritis (PsA) compared to matched controls, and to assess whether biologic disease-modifying antirheumatic drugs (bDMARDs) reduce cardiovascular risk in PsA. We conducted a retrospective cohort study using the TriNetX Global Health Research Network (2002-2025). Adults with PsA (N = 123,031) were 1:1 propensity-score matched to controls without PsA (N = 123,031) on demographics, comorbidities, and medications. Within the PsA cohort, bDMARD-treated patients (N = 44,870) were matched to conventional synthetic DMARD-treated patient

Baseline pain, fatigue, and sleep quality predict 12-week pain improvement in inflammatory arthritis: retrospective real-world analysis of a digital health application cohort.

Despite advances in pharmacotherapy, many patients with inflammatory arthritis (IA) experience residual pain even in remission. Digital health applications (DHAs), when combined with machine learning, may help identify predictors of pain relief in real-world care. To identify predictors of clinically meaningful pain relief in IA using DHA data. We retrospectively analyzed 914 adults with rheumatoid arthritis, psoriatic arthritis, or spondyloarthritis using the Mida Rheuma App. Patients were categorized according to sufficient pain relief, defined as ≥ 30% reduction in pain intensity (visual an

Using causal machine learning and real world data to improve dose response decision making for secukinumab in psoriatic arthritis.

Personalized treatment in psoriatic arthritis (PsA) remains challenging, particularly in guiding dose escalation decisions. We applied a causal machine learning framework to real-world data from the AQUILA study to evaluate the impact of secukinumab dose escalation (150 mg to 300 mg) on disease activity and health-related quality of life (HR-QoL). Using double machine learning, we estimated both average and patient-level conditional treatment effects (CATEs) based on 42 baseline variables, including demographics, laboratory values, disease activity, HR-QoL, comorbidities, and prior treatments.

Psoriatic arthritis monocyte DNA methylomes bridge joint and skin inflammatory pathways across rheumatoid arthritis and psoriasis.

OBJECTIVES: Immune-mediated inflammatory diseases share clinical, genetic, and immunological features, but their specific molecular drivers are not fully defined. Psoriatic arthritis (PsA) exemplifies this overlap, bridging the joint pathology of rheumatoid arthritis (RA) with the skin involvement of psoriasis (PsO). We investigated shared and disease-specific epigenomic traits across the RA-PsA-PsO spectrum, focusing on monocyte DNA methylation and transcriptional programmes. METHODS: We compared genome-wide DNA methylation profiles from peripheral blood monocytes from patients with RA, PsA,

Difficult to treat disease in psoriatic arthritis- is it different from axial spondyloarthritis?

Despite major therapeutic advances, a substantial proportion of patients with spondyloarthritis (SpA), including psoriatic arthritis (PsA) and axial spondyloarthritis (axSpA), experience persistent symptoms, functional limitations, and reduced quality of life. Historically, heterogeneous terminology has limited comparability across studies for this population. To address this gap, three international initiatives, GRAPPA, EULAR, and ASAS, have proposed consensus definitions for two nested states: a broad category of "difficult-to-manage" (D2M) or "complex-to-manage" (C2M) disease and a more str

Cardiovascular Outcomes Associated With Biologic Therapy in Inflammatory Dermatologic Diseases: A Systematic Review.

Chronic systemic inflammation is characteristic of many inflammatory dermatoses, with psoriasis showing the closest association. This inflammation is closely linked to atherosclerosis and adverse cardiovascular events. Given that the same pro-inflammatory cytokines targeted by biologic therapies in skin disease are also central to cardiovascular pathophysiology, these treatments may attenuate vascular inflammation with potential implications for cardiovascular risk. This systematic review provides a qualitative analysis of the evidence on the cardiovascular effects of biologic therapies in der
