Background: The Mediterranean diet (MedDiet) has been associated with anti-inflammatory effects and potential benefits in several chronic conditions. However, adherence to the MedDiet and its relationship with lifestyle factors and disease severity across different rheumatological diseases remain poorly characterized. Objectives: This study aimed to evaluate differences in MedDiet adherence among patients with rheumatoid arthritis (RA), psoriatic arthritis (PsA), and fibromyalgia (FM), and to explore its association with cardiovascular comorbidities, bowel habits, and disease-related outcomes.

OBJECTIVES: IL-17A inhibitors are therapeutic options in PsA, but response is not universal. Evidence from other inflammatory arthritides suggests differential gene expression may predict the outcomes. This study aimed to identify transcriptomic predictive biomarkers of response in PsA patients commencing secukinumab.
METHODS: Participants were recruited to OUtcomes of Treatment in Psoriatic Arthritis Study Syndicate (OUTPASS), a prospective observational cohort study of patients with PsA initiating advanced therapeutics. Samples for this analysis were chosen based on extreme phenotype respon

BACKGROUND: Psoriasis is recognized as a systemic inflammatory disease associated with metabolic dysregulation. Understanding these metabolic changes may reveal biomarkers to elucidate disease mechanisms and predict comorbidities. While previous studies have identified psoriasis-associated metabolites, findings are often limited by sample sizes and lack validation.
OBJECTIVES: To identify circulating metabolites associated with psoriasis, including disease severity and psoriatic arthritis. Further, we investigated whether the metabolic signature was disease-specific compared to other immune-m

Risk of serious infections with biological drugs for psoriasis (PsO) and psoriatic arthritis (PsA) remains uncertain, particularly with long-term exposure and in older adults. We aimed to assess the comparative risk of serious infections among biological agents used for PsO/PsA. Using the Italian VALORE distributed database (2015-2021), we conducted a retrospective cohort study replicating the EXCEED trial (adalimumab vs. secukinumab), followed by a second-stage cohort extending follow-up, restricting analyses to older patients, and comparing other biological drugs. For replication, new users

Autoantigens play a critical role in triggering autoimmunity in autoimmune diseases. Psoriasis, a systemic autoimmune skin disease, has an incompletely defined spectrum of autoantigens in terms of both identity and function. Here, through integrated analysis of bulk transcriptomic, DNA methylomic, and immunopeptidomic datasets, we identify 10 psoriasis-associated autoantigens that are reactivated by DNA hypomethylation. Among them, transcobalamin 1 (TCN1) is a keratinocyte-secreted autoantigen that is internalized by antigen-presenting cells and presented via human leukocyte antigen class II m

INTRODUCTION/OBJECTIVES: Psoriatic arthritis (PsA) is a heterogeneous inflammatory disease within the spectrum of spondyloarthritis (SpA) that shares genetic and clinical features with other subtypes. HLA-B27 is a well-established genetic risk allele for axial SpA, while its role in PsA is less clear. This study aimed to investigate the association between HLA-B27 and the clinical, laboratory, and therapeutic features of a real-world PsA cohort.
METHOD: We performed a retrospective analysis of 333 patients with PsA who fulfilled the CASPAR criteria and had documented HLA-B27 status. Demograph

BACKGROUND AND AIMS: Cardiovascular disease (CVD) is the leading cause of death in inflammatory arthritis. Epicardial fat has emerged as a novel marker of CVD. Therefore, we examined the association of epicardial fat volume (EFV) with disease activity and other atherosclerotic CV markers in patients with inflammatory arthritis.
METHODS: Patients with rheumatoid arthritis (RA) (n = 114) and spondyloarthritis (SpA), including ankylosing spondylitis (n = 28) and psoriatic arthritis (n = 100) were recruited. EFV was assessed from coronary artery calcium scoring (CACS) computed tomography scans. A

INTRODUCTION: People with IA may suffer from pain of differing aetiologies and subtypes including nociceptive joint pain, neuropathic pain of carpal tunnel syndrome or nociplastic pain from concomitant fibromyalgia. Lack of precise measurement tools to identify nociplastic pain influences as a contextual factor potentially all outcomes in collected clinical trials as residual pain might impact various measurements in IA. The OMERACT 2025 pain SIG discussed, developing a scoping review from protocol, to identify an instrument to measure nociplastic pain in IA and a contextualised domain definit

OBJECTIVE: Glucagon-like peptide 1 receptor agonists (GLP-1RAs) and sodium-glucose cotransporter 2 inhibitors (SGLT2i) facilitate weight loss and exhibit immunomodulatory effects, but their impact on the risk of developing autoimmune rheumatic diseases (ARDs) is unclear. We compared ARD incidence following initiation of GLP-1RAs or SGLT2i versus a weight-neutral comparator (dipeptidyl peptidase 4 inhibitors [DPP4i]).
METHODS: We performed a population-based cohort study using administrative health data from a Canadian province with universal health care. We included adults with type 2 diabete

BACKGROUND: Chronic spontaneous urticaria (CSU) is a refractory dermatosis characterized by recurrent wheals and pruritus. Although omalizumab is recommended for patients failing antihistamine therapy, some patients remain unresponsive. Tofacitinib is an oral JAK inhibitor that exerts immunosuppressive effects by blocking the key intracellular inflammatory signaling pathway (JAK-STAT). Currently approved for the treatment of rheumatoid arthritis, psoriatic arthritis, ulcerative colitis, and other conditions, it is also widely used in clinical practice for atopic dermatitis, vitiligo, and refra

In the clinical context of the new medical concept of diseases related to the Major Histocompatibility Complex class I (MHC-I-opathy), contrasting data are available on the possible association among HLA-B*51, Behcet's disease (BD), and spondyloarthritis (SpA). The aim of this retrospective study on a cohort of HLA-B*51-positive patients who were clinically observed for almost 5 years is primarily to evaluate which classification criteria they satisfy among BD, axial (ax) or peripheral (p) SpA, and psoriatic arthritis (PsA). Furthermore, we characterized the possible impact of different arthri

OBJECTIVES: Efficacy thresholds allow interpretation of randomised clinical trials (RCTs) and establishment of treatment targets. To assess the need for more personalised treatment goals, minimal clinically important improvement (MCII) thresholds in psoriatic arthritis (PsA) clinical and patient-reported outcome measures were determined by treatment history and baseline disease activity.
METHODS: This post hoc analysis pooled data from 3 RCTs in participants with active PsA receiving guselkumab every 8 week (Q8W), or placebo with W24 transition to guselkumab Q8W. MCII thresholds for clinical

Plain radiographs have been widely used in psoriatic disease (PsD) for diagnosis, classification, and monitoring of structural damage. However, with the emergence of other imaging modalities such as ultrasound, magnetic resonance imaging, and computed tomography, the necessity of plain radiographs in PsD is debatable. At the Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) 2025 annual meeting in Bogotá, Colombia, Drs. William Tillett and Arthur Kavanaugh were invited to debate the topic "Plain radiography in psoriatic disease: is it still necessary?" Each present

BACKGROUND: Many patients with psoriasis suffer from post-inflammatory pigmentary alteration (PIPA), which remains an unmet treatment need. Eruptive lentiginosis (EL) is a rare form of PIPA where multiple dark brown macules arise within a resolving psoriatic plaque. We present a case of a 68-year-old male with a history of chronic plaque psoriasis and prothrombin deficiency, who developed extensive EL following ixekizumab treatment. In addition, this article will discuss the current literature on risk factors, clinical outcomes, and treatments for EL.
METHODS: The systematic literature review

BACKGROUND: The first phase of the Psoriasis Longitudinal Assessment and Registry (PSOLAR) provided safety, demographic, and other outcomes data from >12,000 patients (>75,000 patient-years) eligible to receive biologics for psoriasis. In 2018, PSOLAR enrollment criteria were updated to specifically include patients receiving either guselkumab or an interleukin-17 inhibitor (IL-17i).
METHODS: Baseline characteristics are described for patients included in PSOLAR after adoption of Protocol Amendment 6 (March 26, 2018), which updated enrollment criteria to include patients exposed to guselkumab

OBJECTIVE: To assess whether early-life antibiotics are associated with spondyloarthritis (SpA) diagnosed by age 21.
METHODS: In this population-based, matched case-control study, we used Danish live births (1997-2023) restricted to those reaching the following minimum relevant ages by December 31, 2024: 1-21 years for psoriatic arthritis (PsA), and 6-21 years for peripheral/axial SpA and inflammatory bowel disease (IBD)-associated arthritis. Cases (n = 560) had physician-recorded SpA (peripheral/axial SpA, PsA, or IBD-associated arthritis). Controls were matched 1:50 by birth year, sex, and

The relationship between psoriatic arthritis (PsA) and Peyronie's disease (PD) remains poorly understood. PsA is a chronic immune-mediated inflammatory disease, whereas PD is a fibroproliferative disorder of the penile tunica albuginea that leads to penile curvature, pain, and sexual dysfunction. Both conditions involve chronic inflammation and dysregulated tissue remodeling. We report the case of a 42-year-old man with long-standing PsA who developed acute inflammatory PD shortly after discontinuing biologic therapy because of loss of insurance and medication nonadherence. The patient present
