Gait and Plantar Pressure Analysis in Charcot Neuroarthropathy: A Clinic-Based Biomechanical Study.

INTRODUCTION: Charcot Neuroarthropathy (CN) is a progressive joint disorder resulting from peripheral neuropathy and repetitive microtrauma. Management prioritizes offloading to prevent deformity, ulceration, infection, and amputation. While three-dimensional gait analysis can be useful for disease staging and guiding management, its cost and limited accessibility limit routine clinical use. This study evaluated plantar pressure and two-dimensional gait kinematics in CN patients using a relatively low-cost clinic-based approach. METHODS: Following institutional review board approval, this cro

Systemic Sclerosis Presenting With Long-Standing Peripheral Neuropathy and Delayed Multisystem Manifestations: A Case Report.

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder characterized by immune dysregulation, microvascular injury, and progressive fibrosis involving the skin and internal organs. Although Raynaud phenomenon and cutaneous sclerosis are the hallmark manifestations, atypical presentations may significantly delay diagnosis. Peripheral neuropathy is an uncommon neurological manifestation and rarely precedes the diagnosis of SSc by several years. We report the case of a 62-year-old female who presented with a 20-year history of progressive distal paresthesia involving both upper

Musculoskeletal Histopathology of Klippel-Trénaunay Syndrome: A Clinicopathologic Study of 21 Cases With Long-Term Orthopedic Follow-Up.

Klippel-Trénaunay syndrome (KTS) is a rare congenital vascular-overgrowth disorder within the broader spectrum of PIK3CA-related overgrowth syndromes. Although clinical, radiologic, and orthopedic manifestations are well described, the histologic spectrum of KTS-associated pathology remains under-characterized. Clinicopathologic findings from 21 patients with clinically established KTS were reviewed, including 18 with longitudinal clinical follow-up and 3 consultation cases of rare malignant mesenchymal neoplasms. Cases were identified from institutional pathology and clinical databases, and a

Charcot Neuro-osteoarthropathy: Critical Concepts and Practical Management Pearls for Dermatologists.

Charcot neuro-osteoarthropathy (CNO), formerly known as Charcot's arthropathy, is a progressive joint and bone disorder primarily affecting the bones of the feet. It usually occurs secondary to neuropathic damage from conditions like diabetes and leprosy. This chronic condition evolves through acute inflammatory and later chronic phases, characterized by joint destruction and instability. Pathophysiological mechanisms involve neurotraumatic and neurovascular theories, along with newer insights such as altered neuropeptide signaling, including calcitonin gene-related peptide, and dysregulation

In Situ Genetic Engineering Facilitates MMP-9-Responsive TIMP1 Release Through Proteolytic Cleavage to Boost Diabetic Wound Healing.

Diabetic Foot Ulcer (DFU), a severe chronic diabetes complication with low healing and high recurrence rates, is a major global health challenge. Overexpression of matrix metalloproteinase-9 (MMP-9) and the consequent MMP-9/TIMP1 (tissue inhibitor of metalloproteinase 1) imbalance delays healing by degrading the extracellular matrix, impairing granulation tissue formation, and exacerbating inflammation. Conventional therapies do not dynamically respond to fluctuating protease levels in chronic wounds. This study introduces an MMP-9-responsive protein release system (M9RR), where the therapeuti

Persistent Hyperprocalcitoninemia as the Initial Diagnostic Clue to Sporadic Medullary Thyroid Carcinoma.

UNLABELLED: Medullary thyroid carcinoma (MTC) is a rare neuroendocrine tumour arising from parafollicular C cells, and serum calcitonin remains its most reliable biochemical marker. Procalcitonin (PCT), widely used in the evaluation of bacterial infections, is usually undetectable under non-infectious conditions. Persistent elevation of PCT in the absence of clinical or microbiological evidence of infection is uncommon and may represent an early clue to underlying endocrine malignancy. We report the case of a 48-year-old woman who presented with markedly elevated PCT levels (>20 ng/ml) detecte

Micro/nanoplastics pollution: emerging challenges for aquatic animals and food crops.

IMPORTANCE: How complications associated with chronic venous insufficiency (CVI) develop is not clear. The central source of the complications is likely a dysfunction of the calf muscle pump, which includes veins and their valves, the gastrocnemius and other lower leg and foot muscles as well as the nerves supplying the muscles, and ankle mobility limitations. The least well-studied source of complications is the relationship between range of ankle movement (ROAM), neuropathy, and the clinical severity of the disease. OBJECTIVE: To study sensory neuropathic changes and ankle mobility in patie

Dual mobility total hip arthroplasty vs. bipolar hemiarthroplasty in treating patients with displaced femoral neck fractures: a systematic review and meta-analysis.

BACKGROUND: Total hip arthroplasty (THA) has been reported with superior outcomes in hip pain, reoperation rate, function, but higher rate of dislocation compared with bipolar hemiarthroplasty (BHA) for treatment of displaced femoral neck fractures (DFNFs). A dual mobility cup (DMC) has been designed to reduce the rates of dislocation and re-operation. However, whether DMC-THA achieves better clinical outcomes than BHA remains ambiguous. Therefore, we have conducted this meta-analysis to investigate the clinical outcomes of DMC-THA vs. BHA in the treatment of DFNFs. METHOD: We identified stud

Anti-Integrin αvβ6 Autoantibodies as a Biomarker for Ulcerative Colitis in Patients With Axial Spondyloarthritis.

OBJECTIVE: Anti-integrin αvβ6 autoantibodies (anti-αvβ6) have recently been described as informative biomarkers for ulcerative colitis (UC), demonstrating a high sensitivity and specificity compared to patients without inflammatory bowel disease (IBD). Given the association between axial spondyloarthritis (axSpA) and IBD, we sought to evaluate the potential of anti-αvβ6 as a biomarker in axSpA patients with or without concurrent IBD. METHODS: Using an established ELISA protocol, we measured anti-αvβ6 levels in sera of patients with (1) axSpA and UC, (2) axSpA and Crohn disease (CD), (3) axSpA

Individualised Wound Treatment Strategies for Skin Lesions Caused by Calcium Gluconate Extravasation in Neonates: A Case Report.

Extravasation of calcium gluconate solution is a rare yet serious complication in neonatal intensive care units (NICUs), capable of causing significant skin necrosis, ulceration and long-term functional impairment. This study reports a case of such injury and describes an effective, individualised strategy for wound management. An 18-day-old, 4200 g male infant, diagnosed with hypocalcaemia (serum calcium: 1.6 mmol/L) at a hospital in China, experienced calcium gluconate extravasation following peripheral intravenous supplementation. This complication resulted in localised skin swelling, ulcer

Analysis of common differential gene expression between rheumatoid arthritis and ulcerative colitis.

OBJECTIVE: This study employs bioinformatics analysis with the objective of identifying commonly differentially expressed genes (DEGs) in ulcerative colitis (UC) and rheumatoid arthritis (RA), as well as exploring their underlying molecular mechanisms. By doing so, it aims to provide a theoretical basis for investigating the potential associations between these two diseases and developing novel therapeutic strategies. MATERIALS AND METHODS: We downloaded multiple gene expression datasets for Rheumatoid Arthritis (RA) and Ulcerative Colitis (UC) from the Gene Expression Omnibus (GEO) database.

Dismantling the Necroptotic Engine: An Oral Theranostic Nanosponge for Ulcerative Colitis.

Ulcerative Colitis (UC) treatments often lack target specificity and have significant side effects. A key pathological driver is the excessive necroptosis of intestinal epithelial cells (IECs), which disrupts the gut barrier. To address this, we designed an intelligent oral theranostic nanoplatform, CurN@I, to dismantle the necroptosis-inflammation axis. CurN@I consists of a barium sulfate (BaSO4) core for computed tomography (CT) imaging, encapsulated within a pH-responsive silk protein nanosponge. This scaffold is co-loaded with a necroptosis inhibitor (Necrostatin-1s, Nec-1s) and an antioxi
