Two-Stage vs Single-Stage Ilizarov Application in High-Risk Patients: Impact on Early Complications and Resource Utilization.

BACKGROUND: Patients treated with an Ilizarov external fixator often feature risk factors that impair wound healing such as diabetes, peripheral arterial disease, obesity, and renal insufficiency. A 2-staged approach, with initial bony correction (Kirschner wire or hybrid) followed by delayed Ilizarov application by 5 days, is intended to support wound healing by allowing for reduction of soft tissue swelling and enhancing postoperative wound care. We hypothesized that this method is associated with fewer wound complications but increased use of resources. METHODS: A retrospective review of 9

USP30 inhibits wound healing in diabetic foot ulcers by promoting TRAF6-induced ferroptosis and angiogenesis inhibition.

BACKGROUND: Diabetic foot ulcers (DFUs) frequently complicate diabetes. Ubiquitin-specific protease 30 (USP30), a key deubiquitinating enzyme, is implicated in diverse diseases. This study explores USP30's effects and mechanisms on DFUs. METHODS: Tumor necrosis factor receptor-associated factor 6 (TRAF6), USP30, GPX4, and SLC7A11 protein levels were determined using western blot. TRAF6 mRNA level was assessed using real-time RT-qPCR. Cell proliferation and mortality were evaluated via the 5-Ethynyl-2'-deoxyuridine assay and TdT-mediated dUTP nick end labeling, respectively. Intracellular ferr

PathMED: an R toolkit for single-sample molecular scoring and machine learning with omics data.

MOTIVATION: Molecular scoring is a popular approach for studying pathway-level functional alterations with omics data. Using molecular scores for tasks such as single-sample molecular characterisation, phenotype prediction or disease stratification has several advantages compared to using omics data directly. Molecular scores provide biological interpretability and are more generalisable across datasets, facilitating data integration and machine learning applications. However, numerous scoring methods are available through different software packages, and currently there is a lack of tools to

[Minimally invasive surgery for Charcot arthropathy].

In Germany, 9.3 million people are affected by type 2 diabetes mellitus [27, 29]. Diabetic foot syndrome encompasses the structural and functional damage to the foot that occurs as a result of this underlying condition and can progress to Charcot foot in advanced stages. This complex clinical condition requires specialized, experienced treatment, with the preservation of the limb and its function as the primary therapeutic goal. Conventional open surgical procedures have so far been only moderately successful due to high complication rates. In contrast, minimally invasive surgery (MIS) offers

Association between inflammatory bowel disease and ankylosing spondylitis: Mendelian randomization of a different database and meta-analysis.

Inflammatory bowel disease (IBD) and ankylosing spondylitis (AS) are common autoimmune disorders that have a serious impact on the physical and mental health of patients. Some patients with AS also suffer from IBD, and some studies have shown that there is a link between the 2. However, the causal relationship between the 2 is not clear. This study aimed to infer a causal relationship between IBD and AS using Mendelian randomization (MR) analysis. We obtained genome-wide association study data related to IBD and AS from the Integrative Epidemiology Unit database and the Finnish database to inf

Shengji Yuhong Ointment accelerates diabetic wound healing by suppressing AGEs/RAGE-related inflammation and restoring HIF-1α/VEGF-mediated angiogenesis.

ETHNOPHARMACOLOGICAL RELEVANCE: Shengji Yuhong Ointment (SJYHO) is a classic external formula in traditional Chinese medicine surgery, which was first recorded in Wai Ke Zheng Zong. It is commonly used for conditions such as chronic non-healing sores and ulcers, failure of necrotic tissue to slough off, poor granulation tissue formation, and chronic refractory wounds. Diabetic wounds are long exposed to a pathological microenvironment characterized by hyperglycemia, chronic inflammation, oxidative stress, and impaired angiogenesis. This shares intrinsic connections with the traditional indicat

Review: advances in snail mucus-inspired biomaterials for diabetic wound repair.

Diabetic wounds are a class of chronic non-healing wounds driven by multiple interacting factors. Conventional therapies remain limited in their ability to actively modulate the wound microenvironment and promote coordinated tissue regeneration. In this context, naturally derived biomaterials with combined biological activity and material functionality have attracted increasing attention. Among them, snail mucus has emerged as a promising source of inspiration because of its multicomponent composition and potential roles in hydration, adhesion, barrier protection, antimicrobial activity, immun

Lactate dehydrogenase to albumin ratio as a convenient parameter for predicting the severity of Behçet disease: a cross-sectional study.

BACKGROUND: We investigated whether the ratio of lactate dehydrogenase (LDH) to albumin could be used as a diagnostic marker for Behçet's disease compared to the healthy population and as a marker to assess the severity and activity of the disease. METHODS: The research was conducted as a cross-sectional study. Patients aged 18-69 years presenting to the Behçet's disease Outpatient Clinic over a three-year period and healthy individuals with a similar age and gender distribution formed the two study groups. LDH and albumin levels, and the LDH/albumin ratio were compared between the groups. R

Early-onset systemic lupus erythematosus with discoid rash and chondritis.

Juvenile systemic lupus erythematosus (jSLE) is a rare disorder that presents with greater severity compared to adult systemic lupus erythematosus. It typically manifests in adolescent females with constitutional symptoms, multisystem involvement, and a malar rash. In this case, a 9-year-old African American pre-pubertal girl presented with a progressive facial and scalp rash, weight loss, night sweats without fever, and abdominal pain following recent penicillin exposure. On laboratory results and clinical examination, leucopenia, auricular chondritis, palatal ulceration, and marked facial ra

Behçet-like Syndromes: A Comprehensive Review.

BACKGROUND: Behçet-like syndrome (BLS) refers to the presence of Behçet's disease (BD) features occurring in association with distinct clinical-pathological conditions such as inborn errors of immunity, myeloproliferative disorders, infections, or drug exposure. BLS may differ clinically from BD and is increasingly recognized as a separate entity. Distinguishing BLS from primary BD is essential for appropriate management, and studying BLS may provide insights into BD pathogenesis. OBJECTIVES: To summarize clinical features, treatments, and genetic abnormalities reported in BLS, we reviewed al

Bowel-Associated Dermatosis and Arthritis Syndrome (BADAS) - A Literature Review With Diagnostic and Therapeutic Implications and a Report of Two Cases of BADAS Associated With Inflammatory Bowel Disease.

Bowel-associated dermatosis-arthritis syndrome (BADAS) is a rare neutrophilic dermatosis characterized by recurrent erythematous macules, papules or vesicopustules accompanied by fever, malaise, arthralgia and peripheral arthritis. Although originally described after intestinal bypass procedures, BA-DAS is increasingly recognized in inflammatory bowel disease (IBD), yet remains underdiagnosed and lacks practical guidance. We reviewed published IBD-associated BADAS cases to define clinical patterns and therapeutic options, and report two additional cases managed by an interdisciplinary team. Ac

Using Remote Behavioral Skills Training to Teach University Students Literature Searching Skills.

BACKGROUND: The presence of bacteria in bile is an important factor in the formation of pigment gallstones. The bile of healthy people is sterile and bacteria in the biliary system come from endogenous infection from the gut. Yet, the route of bacterial translocation into the bile duct is still unclear. Theoretically, two routes exist: one is through the intestinal barrier and the other is by direct reflux from the sphincter of Oddi. This study was undertaken to explore the relationship between the effectiveness of intestinal barrier and the formation of pigment gallstones in hamsters. METHOD

Differential diagnosis of solitary rectal ulcer syndrome and early rectal cancer via endorectal ultrasound: a retrospective matched case-control study.

BACKGROUND: Solitary rectal ulcer syndrome (SRUS) is a rare chronic rectal disease with nonspecific symptoms and endoscopic findings that overlap with rectal cancer, leading to misdiagnosis in up to 64%. Biopsy is often limited by insufficient depth. Data on the use of endorectal ultrasound (ERUS) for differentiating SRUS from early rectal cancer (ERC) are scarce. Thus, this study aims to identify ERUS features that differentiate SRUS from ERC (T1 and T2 stages). METHODS: Twenty-nine patients who were pathologically diagnosed with SRUS between 2015 and 2024 were enrolled, whereas ERC patients
