BACKGROUND: Systemic juvenile idiopathic arthritis (SoJIA) lacks specific biomarkers for objective disease activity assessment. Conventional inflammatory markers have limited accuracy for monitoring disease fluctuation. Few studies have integrated lipid metabolic and immune parameters to support SoJIA activity assessment, underscoring the need for simple and efficient biomarker combinations. This study aims to characterize differences in immune parameters between children with active SoJIA and healthy controls; compare the lipid profiles among children with active SoJIA, the same children in r

BACKGROUND: This cross-sectional study examined the relevance and impact of fear of disease progression (FoP) to parents of youth with two common childhood chronic conditions—juvenile idiopathic arthritis (JIA) and type 1 diabetes mellitus (T1DM).
METHODS: 124 parents (57 JIA, 67 T1DM; 107 mothers, 17 fathers) of youth aged 2–18 years (M=11.36, SD=3.90) were recruited through social media and patient organisation groups. Parents completed online questionnaires assessing fear of their child’s disease progression (FoP), responses to their children's symptoms and their own psychopathology. In ad

OBJECTIVES: Antenatal glucocorticoids are effective in reducing the risk of neonatal respiratory distress syndrome; however, the potential long-term adverse effects of antenatal glucocorticoids on the development of pediatric autoimmune diseases remain largely unexplored. This study investigated the associations of antenatal glucocorticoids with pediatric autoimmune diseases, specifically juvenile idiopathic arthritis (JIA), Kawasaki disease (KD), and systemic lupus erythematosus (SLE), during the first five years of life.
METHODS: We conducted a nationwide cohort study from 2008 through 2021

Progressive encephalomyelitis with rigidity and myoclonus (PERM) represents the most severe subtype of stiff-person spectrum disorders and is exceedingly rare in children. We report a 14-year-old female patient who presented with chronic joint pain and axial rigidity as the initial manifestations, with a disease course of two years, during which she had been diagnosed with juvenile idiopathic arthritis but lacked typical peripheral arthritis signs and showed poor response to therapy. The patient exhibited paroxysmal axial rigidity, autonomic dysfunction, and sleep disturbances. Although typica

Juvenile idiopathic arthritis (JIA) is a common inflammatory rheumatic disease of childhood, yet the mechanisms driving immune activation in affected joints remain unclear. In this study, we analyzed synovial fluid (SF) immune cells from patients with JIA using two complementary methodologies, single-cell multiomics and flow cytometry. SF displayed a markedly altered immune landscape compared with blood. Both myeloid and plasmacytoid dendritic cells were enriched in SF, and SF monocytes exhibited signs of macrophage differentiation. B cells were strikingly rare in SF, and the few B cells prese

PURPOSE: To identify factors associated with adalimumab initiation in non-infectious uveitis.
METHODS: This was a retrospective case-control study using multicentric data of 3204 patients. The primary outcome was whether adalimumab was initiated, and the secondary outcome was time to initiation. To investigate factors influencing the primary outcome, propensity score matching (1:3) of adalimumab recipients (n = 53, cases) to non-recipients (n = 159, controls) was done to reduce confounding by age, sex, race, anatomical category of disease, and follow-up time. Subsequently, subgroup analysis w

OBJECTIVES: Uveitis, an inflammation of the uveal tissue, can cause permanent vision loss. A previous study revealed elevated levels of cathepsin S in the tear proteomic analyses of patients with juvenile idiopathic arthritis-associated uveitis. This study investigates the effects of a cathepsin S inhibitor on inflammation in a rat model of uveitis.
METHODS: Forty female rats were divided into five groups: Group I (healthy control), Group II (vehicle group), Group III (Z-FL-COCHO-treated group), Group IV (EIU group), and Group V (lipopolysaccharide [LPS] and Z-FL-COCHO-treated group). Uveitis

INTRODUCTION: Monoclonal anti-tumor necrosis factor (anti-TNF) agents have substantially improved the management of juvenile idiopathic arthritis (JIA)-associated uveitis. However, some patients remain refractory despite multiple biologic therapies. We aimed to evaluate the outcomes of abatacept in patients with chronic JIA-associated uveitis refractory to synthetic DMARDs, anti-TNF agents, and tocilizumab at a tertiary referral center and to review the available pediatric literature.
METHODS: This retrospective observational series included patients with chronic JIA-associated uveitis treate

BACKGROUND: Juvenile idiopathic arthritis (JIA) treatment relies on biologic and conventional disease-modifying anti-rheumatic drugs (b/csDMARDs), but pediatric-specific bone and growth safety remains incompletely characterized. Standard FAERS disproportionality analyses in pediatric polypharmacy may overstate signals through co-medication coding. We aimed to identify drug-specific bone-toxicity (Composite A) and growth/developmental (Composite B) signals and to test whether they survive a Primary-Suspect (PS) restriction designed to isolate reporter-attributed reactions.
METHODS: Pre-specifi

Blau syndrome is a rare granulomatous autoinflammatory disease caused by NOD2 mutations and characterised by dermatitis, arthritis and uveitis. We report a middle childhood boy initially diagnosed with oligoarticular juvenile idiopathic arthritis (JIA) because arthritis was the predominant early manifestation. He had papular skin lesions in infancy and periarticular swelling and tenosynovial cysts, later developed persistent arthritis and recurrent fever. Genetic testing identified a heterozygous NOD2 variant (c.1001G>A, p.Arg334Gln [R334Q]), confirming Blau syndrome. Despite ongoing methotrex

OBJECTIVE: To compare the clinical characteristics and outcomes of children with juvenile idiopathic arthritis (JIA) treated with biologic drugs in traditional clinical trials with the real-world data (RWD) in the CARRA Registry.
METHODS: This was a retrospective observational study. We reviewed data from 3192 children in the CARRA Registry with JIA exposed to adalimumab or canakinumab, 2 historical trials of polyarticular JIA participants exposed to adalimumab (n = 171 and 32), and 1 trial for canakinumab in systemic JIA (n = 43). We made matched and unmatched comparisons of demographic info

BACKGROUND: Temporomandibular joint (TMJ) involvement in rheumatic inflammatory arthropathies (RIA) has been sparsely described, and evidence regarding local treatment options for affected TMJs remains limited.
OBJECTIVES: To perform a systematic review (SR) regarding local treatments for TMJ involvement in RIA and evaluate their effects on pain management, disease improvement, quality of life, health economics, and adverse events.
METHODS: According to the PICO framework, patients with RIA were defined as the population, local TMJ treatment as the intervention, and alternative treatments, p

Whole-body magnetic resonance imaging (WBMRI) has emerged as the primary imaging tool for multifocal inflammatory and infectious musculoskeletal diseases. By extending coverage from the vertex to the toes in a single, radiation-free examination, WBMRI enables the detection of clinically occult lesions, disease-specific pattern recognition across bone marrow, joints, entheses, and muscles, and objective monitoring of disease activity and treatment response. This review describes the WBMRI findings-with detailed imaging features and distribution patterns-in chronic nonbacterial osteitis, SAPHO s

OBJECTIVE: Juvenile idiopathic arthritis (JIA) is a heterogeneous group of childhood-onset arthritides, many of which require sustained pediatric rheumatology care and transition to adult rheumatology care. We examined longitudinal retention in rheumatology care from diagnosis through adulthood.
METHODS: We conducted a population-based longitudinal study of an inception cohort of children/adolescents diagnosed with JIA between 2010 and 2018 and followed until 2024. For each follow-up year, we determined whether patients were engaged in care (at least one pediatric rheumatologist visit), lost

OBJECTIVES: To develop and validate interferon-γ-based early prediction models for MAS in pediatric SJIA patients.
METHODS: We conducted a retrospective multicentre study of hospitalized SJIA patients complicated with MAS across seven tertiary paediatric centres in China between January 2020 and December 2024. Demographic, clinical, laboratory, and cytokine data were extracted from electronic medical records. Univariate and multivariate logistic regression analyses identified independent risk factors. A nomogram and cytokine-based predictive models were constructed and externally validated.
