Introduction. Hemophagocytic lymphohistiocytosis (HLH), a rare but potentially fatal disease, is characterized by excessive immune activation and cytokine release which stimulates bone marrow macrophages to engulf hematopoietic cells. HLH could be secondary to infections: viral, fungal, and bacterial; malignancies and autoimmune diseases. The diagnosis of HLH is usually delayed due to the presence of non-specific symptoms at presentation. This delay contributes to increased mortality. Cases and review. A 9-y-old boy with severe juvenile rheumatoid arthritis who had been recommended for bilateral hip replacement received treatment for more than 8 y from an interprofessional group of clinicians. The clinicians provided chiropractic, naturopathic, and acupuncture care, in addition to pediatrics, rheumatology, orthopedics, and physical therapy care. This process was not easy; the patient used crutches for 2 y during treatment and experienced severe pain secondary to multiple joint pathology. He is now aged 17 y, and he regularly plays golf and basketball with his own hips.

OBJECTIVE: To evaluate the incidence and prevalence of juvenile idiopathic arthritis (JIA) in Olmsted County, Minnesota in 1994-2013 and trends in juvenile rheumatoid arthritis (JRA) in 1960-2013.
METHODS: Cases of arthritis in 1994-2013 were identified by diagnosis code with medical chart review to confirm diagnosis separately for JIA and JRA. Overall incidence rates with 95% confidence intervals (95% CIs) were age and sex adjusted to the 2010 US white population.
