Recognition of Juvenile Ankylosing Spondylitis in an Adolescent Female Presenting to a Chiropractor.

Juvenile ankylosing spondylitis, a form of axial spondyloarthritis (axSpA), is an uncommon cause of chronic back pain in adolescents. We describe a 15-year-old female with a three-year history of chronic lumbosacral and hip pain, prolonged morning stiffness, nocturnal pain, and limited response to multiple conservative treatments, including physical therapy, medication, and injections. Previous lumbar magnetic resonance imaging (MRI) was virtually normal. On chiropractic evaluation, the combination of symptoms, chronicity despite conservative care, and lack of explanatory structural changes in

Evaluating juvenile ankylosing spondylitis: A possible case from the central plains of Ming-Qing China.

OBJECTIVE: To evaluate axial and peripheral joint lesions in a late adolescent/young adult skeleton and assess whether they support a diagnosis of juvenile-onset ankylosing spondylitis. MATERIALS: Individual M55, a possible male aged 16.5-20 years, recovered from a Ming-Qing period tomb (14th-20th century CE) at the Dabuzi cemetery, Shaanxi, China. METHODS: Macroscopic observation was used to record skeletal pathologies. Differential diagnosis focused on inflammatory spondyloarthropathies and related arthropathies. RESULTS: The preserved thoracic and lumbar vertebrae show multi-level poster

Advancing Juvenile Spondyloarthritis: Closing Knowledge Gaps with New Axial JSpA Classification Criteria.

PURPOSE OF REVIEW: Until recently, the absence of validated, pediatric-specific classification criteria for juvenile spondyloarthritis (JSpA) limited targeted clinical trials evaluating treatment efficacy and advancements in understanding the natural history in pediatric-onset disease. There is an urgent need for efficacy and effectiveness studies in this understudied group. RECENT FINDINGS: Most children with JSpA continue to experience disease activity despite current therapies and generally have worse outcomes than those with other juvenile arthritis forms. Fewer than 20% achieve remission

A case of juvenile-onset ankylosing spondylitis effectively treated with tumour necrosis factor-alpha inhibitor agents.

A 15-year-old girl had experienced hip pain at 11 years of age. At 15 years of age, the patient complained of persistent generalised pain. Her rheumatoid factor and serum matrix metalloproteinase-3 levels were below standard values; there were no inflammatory responses, and the human leukocyte antigen test was negative for B27 and positive for B52 and B62. The bath ankylosing spondylitis disease activity index (BASDAI) value was 8.0 at the time of induction and 3.1 at 6 months after the introduction of adalimumab (at a dose of 40 mg). The BASDAI value improved with an increase in the dose of a

Age- and Microbiota-Dependent Cell Stemness Plasticity Revealed by Cattle Cell Landscape.

BACKGROUND/AIMS: Open chamber systems for measuring transepidermal water loss (TEWL) have limitations related to ambient and body-induced airflows near the probe, probe size, measurement sites and angles, and measurement range. The aim of the present investigation was to develop a closed chamber system for the TEWL measurement without significant blocking of normal evaporation through the skin. Additionally, in order to use the evaporimeter to measure evaporation rates through other biological and non-biological specimens and in the field applications, a small portable, battery-operated device

A Case of Chronic Deep Infrapatellar Bursitis Complicated by Patellar Tendinopathy and its Evaluation With Musculoskeletal Ultrasound.

Knee pain is a very common complaint in routine physiatry and orthopedic practice. While bursitis is a well-known and common cause of knee pain, deep infrapatellar bursa (DIPB) involvement is relatively less common. Inflammation of DIPB occurs commonly due to either direct trauma or overuse, but other rare causes have also been reported in the literature including infection, juvenile idiopathic arthritis, gout, and juvenile ankylosing spondylitis. We report a case of chronic inflammation of DIPB caused by direct trauma and associated with patellar tendinopathy. Additionally, we describe the ch

Do geography and ethnicity play a role in juvenile Spondyloarthritis? A multi-center binational retrospective study.

BACKGROUND: Observations among Israeli pediatric rheumatologists reveal that pediatric Juvenile Spondyloarthritis (JSpA) may present differently compared to patients from the United States (US). This study is aimed to compare the demographic and clinical variables of Israeli and US JSpA patients upon presentation. METHODS: We performed a retrospective, cross-sectional, multicenter comparison of JSpA patients among 3 large Israeli pediatric rheumatology centers and a large US pediatric rheumatology center. Unexpected increase of aortic stiffness in juvenile Spondyloarthropathies.

Juvenile spondyloarthropathy is an umbrella term for a group of childhood rheumatic diseases that can cause chronic arthritis extending to the axial skeleton before the age of 16. Although ankylosing spondylitis has aortic involvement as one of its most important effects, this relationship has not been extensively studied in children with juvenile spondyloarthropathy. Here, a cross-sectional study of the elastic properties of the aorta of 43 patients with juvenile spondyloarthropathy and 19 healthy controls is reported. Aortic stiffness assessed by echocardiography was used to predict the pres

Bone tumors developed in patients with juvenile inflammatory arthritis after anti-TNFα therapy.

Administration of anti-TNFα agents has become a mainstay in the treatment of chronic inflammatory arthritis such as rheumatoid arthritis (RA) and spondyloarthritis. Adverse events, including infections and allergic reactions, have been reported. Malignancies are rare but potentially life threatening. The existence of bone tumor in those patients is very rare, only five cases of bone tumors were mentioned in juvenile idiopathic arthritis (JIA) in the literature. We describe three patients in whom bone neoplasms developed after years of anti-TNFα therapy for JIA or juvenile ankylosing spondyliti

Classifications and imaging of juvenile spondyloarthritis.

Juvenile spondyloarthritis may be present in at least 3 subtypes of juvenile idiopathic arthritis according to the classification of the International League of Associations for Rheumatology. By contrast with spondyloarthritis in adults, juvenile spondyloarthritis starts with inflammation of peripheral joints and entheses in the majority of children, whereas sacroiliitis and spondylitis may develop many years after the disease onset. Peripheral joint involvement makes it difficult to differentiate juvenile spondyloarthritis from other juvenile idiopathic arthritis subtypes. Sacroiliitis, and e

The Disease of the Italian Poet Giacomo Leopardi (1798-1837): A Case of Juvenile Ankylosing Spondylitis in the 19th Century?

Some authors sustained that the pessimistic thought of the Italian writer and philosopher Giacomo Leopardi (1798-1837) may be attributed to his unhappy life, characterized by several health problems. His philosophical theories appear as the result of depressive and melancholic state, related to his precarious health conditions, so limiting their intrinsic values. Several authors formulated various hypotheses on the diseases that Leopardi suffered from and postulated different theories on the cause of his early death. This article assumed that Leopardi may have been affected by juvenile ankylos

Recurrent macrophage activation syndrome since toddler age in an adolescent boy with HLA B27 positive juvenile ankylosing spondylitis.

Recurrent macrophage activation syndrome (MAS) is very rare. We present the case of an adolescent boy with human leukocyte antigen (HLA) B27-positive ankylosing spondylitis (AS), who experienced episodes of recurrent MAS since he was a toddler. A 16-year-old boy was admitted because of remittent fever with pancytopenia and splenomegaly after surgical intervention for an intractable perianal abscess. He had been diagnosed with hemophagocytic lymphohistiocytosis (HLH) 4 different times, which was well controlled with intravenous immunoglobulin and steroids since the age of 3. We were unable to i

Biologic agents in juvenile spondyloarthropathies.

The juvenile spondyloarthropathies (JSpA) are a group of related rheumatic diseases characterized by involvement of peripheral large joints, axial joints, and entheses (enthesitis) that begin in the early years of life (prior to 16(th) birthday). The nomenclature and concept of spondyloarthropathies has changed during the last few decades. Although there is not any specific classification of JSpA, diseases under the spondyloarthropathy nomenclature umbrella in the younger patients include: the seronegative enthesitis and arthropathy (SEA) syndrome, juvenile ankylosing spondylitis, reactive arth

Differential plasma microRNAs expression in juvenile idiopathic arthritis.

OBJECTIVES: To identify potential novel biomarkers for juvenile idiopathic arthritis (JIA), we evaluated the correlation between plasma expression levels of specific miRNAs and disease characteristics of JIA. METHODS: Differentially expressed miRNAs in JIA plasma were identified by microarray analysis. Five candidate plasma miRNAs with differential expression were further evaluated by qRT-PCR. The correlation between the expression of candidate plasma miRNAs and clinical parameters of JIA patients was assessed. RESULTS: The expression of miR-16, miR-146a, and miR-223 was higher, and miR-132

Juvenile ankylosing spondylitis in Turner syndrome.

Juvenile ankylosing spondylitis (JAS) is a chronic autoimmune disorder which causes considerable morbidity when left untreated; it occurs predominantly in men. We describe an Asian Indian woman who had JAS with phenotypic features of Turner syndrome (TS) and was found to be a mosaic for 45, X/46, X, psu idic (X) (p11) by karyotyping and fluorescence in situ hybridization (FISH) studies of peripheral blood. The absence of Y chromosome material was confirmed by FISH. Haplo-insufficiency of the X chromosome can predispose to autoimmunity. To the best of our knowledge, this is the first report of

Pediatric fibromyalgia.

Fibromyalgia (FM) is currently defined as chronic widespread pain (CWP) with allodynia or hyperalgesia to pressure pain. It is classified as one of the large group of soft-tissue pain syndromes. Pain is the cardinal symptom of FM; however, most patients also experience additional symptoms such as debilitating fatigue, disrupted or non-restorative sleep, functional bowel disturbances, and a variety of neuropsychiatric problems, including cognitive dysfunction, anxiety and depressive symptoms. Its pathogenesis is not entirely understood, although it is currently believed to be the result of a ce

Diagnosis and treatment of enthesitis-related arthritis.

Juvenile idiopathic arthritis (JIA) is a chronic, inflammatory disease of unknown etiology. The enthesitis-related arthritis (ERA) JIA category describes a clinically heterogeneous group of children including some who have predominately enthesitis, enthesitis and arthritis, juvenile ankylosing spondylitis, or inflammatory bowel disease-associated arthropathy. ERA accounts for 10%-20% of JIA. Common clinical manifestations of ERA include arthritis, enthesitis, and acute anterior uveitis. Axial disease is also common in children with established ERA. Treatment regimens for ERA, many of them base
