Decreased levels of sCD21 and sCD23 in blood of patients with systemic-juvenile arthritis, polyarticular-juvenile arthritis, and pauciarticular-juvenile arthritis.

A soluble form of CD21 (sCD21) and CD23 (sCD23) is released from the surface of human white blood cells upon shedding of the extracellular domain. sCD21 circulates in a complex with cleavage fragments of C3 and sCD23, which were previously identified as ligands of membrane and soluble CD21. sCD21 seems to be a marker of chronic inflammatory disease. To assess the sCD21 and sCD23 status in patients with subsets of juvenile arthritis (JA), we determined plasma levels sCD21 and sCD23. Plasma sCD21 levels were significantly decreased in all JA subtypes (O-JA P < 0.0068; P- and S-JA P < 0.0001) com

Juvenile arthritis, HLA-A2 and binding of DEK oncogene-peptides.

Previous studies have shown that susceptibility to Pauciarticular Juvenile Arthritis is associated with HLA-A*0201. Recently, autoantibodies against the protein of the DEK oncogene have been found in sera of patients with this disease. If T cells are involved in the pathogenesis of joint lesions, it is possible that they target autoantigens presented by HLA-A*0201. Therefore, we investigated whether DEK-derived peptides can bind efficiently to HLA-A*0201. Nonameric peptides selected considering anchor positions 2 and 9, and preferred amino acids at other positions, were incubated either with t

Spondyloarthropathies in circumpolar populations of Chukotka (Eskimos and Chukchi): epidemiology and clinical characteristics.

OBJECTIVE: We sought to determine the prevalence of HLA-B27 and the prevalence and nature of spondyloarthropathies in the native residents of Eastern Chukotka, Siberia, Russia. METHODS: We conducted a cross sectional study with medical examination and interview of 464 of 640 (72.5%) native residents of 2 Eastern Chukotka settlements, Sireniki and Novo-Chaplino. RESULTS: Among them were 212 Eskimos, 98 Chukchi and 154 subjects of mixed race. There were 2 subjects (0.4%) with ankylosing spondylitis, 2 with Reiter's syndrome, 2 with undifferentiated spondyloarthropathy and 1 with late onset pau

[Intra-articular steroids in chronic juvenile arthritis].

The use of intraarticular steroids was evaluated in 15 children with chronic pauciarticular juvenile arthritis. Of the 25 joints injected, 23 responded favorably. At the 6-month check-up, 65% remained in remission. One year after injection, 42% maintained a good response. The beneficial effect of the injection was not associated with age, sex, erythrosedimentation or arthritis duration. A small atrophic lesion in the area of the injection in one patient was the only complication.

Antibodies to lipid A in pauciarticular juvenile arthritis: clinical studies.

IgG antibodies to monophosphoryl lipid A (MPL) are more concentrated in synovial fluids than in the blood of children with pauciarticular juvenile arthritis. Correlations between IgG anti-MPL levels and numbers of inflamed joints were highly significant in 2 of 10 patients followed for 8 years and suggestive in 2 patients followed for shorter periods. A fifth patient had a correlation between IgM anti-MPL and joint count.

The association of pauciarticular juvenile arthritis and myasthenia gravis.

We encountered two children with fluorescent antinuclear antibody-positive pauciarticular juvenile arthritis who later developed myasthenia gravis. Acetylcholine receptor binding, blocking, and modulating antibodies, retrospectively tested on frozen serum, yielded negative results before the onset of myasthenic symptoms but all yielded strongly positive results coincident with the onset of weakness. In both children, myasthenia gravis responded to thymectomy, and one child had a beneficial response to plasmapheresis. Although, to our knowledge, only two patients with juvenile arthritis and mya

C3 activation products correlate with antibodies to lipid A in pauciarticular juvenile arthritis.

IgM antibodies to monophosphoryl-lipid A were found to be elevated in sera from children with all forms of juvenile arthritis (JA) and systemic lupus erythematosus. Of more interest, in patients with pauciarticular JA, IgG antibody titers to monophosphoryl-lipid A were found to be correlated with the C3a concentration and the C3d:C3 ratio. Although the full specificity of these antibodies is unknown, they are the first that have been found to be correlated with complement activation products in any form of JA.

The effect of juvenile inflammatory synovial fluid on in vitro cartilage.

Inflammatory and noninflammatory juvenile synovial fluid (SF) samples were examined in vitro for their effect on cartilage. SF from 3 of 6 patients with pauciarticular juvenile arthritis (JA), 7 of 7 patients with polyarticular JA and 2 of 2 patients with ankylosing spondylitis had significantly increased proteoglycan (PG) releasing activity in 4 day cultures of living but not of freeze-killed cartilage. Noninflammatory SF did not cause increased PG release. Cartilage collagen content was not significantly altered by either inflammatory or noninflammatory SF. Our in vitro results showed PG rel

In vitro responses of lymphocytes in juvenile arthritis: effects of time and concentration of PHA-P.

The effects of concentration of PHA-P and time in culture on cells from patients with systemic, polyarticular and pauciarticular juvenile arthritis have been studied. Responses to suboptimal concentrations are depressed at 3 days, particularly of systemic and polyarticular patients, but normal by 5 days and probably prolonged at 7 days. The responses approach normal with 5 micrograms/ml PHA-P. This delay also occurs in cells from children with cystic fibrosis and acute pneumonia and so is not disease specific. Kinetic studies show a slower recruitment of cells. The delay is corrected by 1 micr

Pauciarticular juvenile arthritis.

Pauciarticular juvenile arthritis begins insidiously, involves no more than five joints, causes few systemic symptoms, and has a relatively mild course. The disease may recur at the same site or in previously uninvolved joints. It generally runs its course in four years. The authors believe that synovectomy is contraindicated in children suspected of having this mild form of arthritis unless joint destruction is evident or symptoms progress for more than three years.
