Leveraging the PEDSnet clinical research network and electronic health record data to enhance efficiency of trial enrollment for a rare pediatric rheumatic disease.

BACKGROUND: We evaluated the PEDSnet clinical research network for study enrollment of juvenile spondyloarthritis, a rare rheumatic disease that includes enthesitis-related arthritis (ERA) and psoriatic arthritis (PsA). METHODS: An electronic health record (EHR)-based typology was developed by an interdisciplinary team to query EHR data for a spectrum of pediatric rheumatic diseases (2009–2023) from 8 PEDSnet centers. The prevalence, characteristics and drug exposures for juvenile spondyloarthritis was explored to gauge feasibility of leveraging the network for study enrollment. Next, the typo

PRO-KIND consensus protocol for classification, monitoring, and therapy in pediatric rheumatology: persistent oligoarticular juvenile idiopathic arthritis.

Protocols concerning classification, monitoring and treatment were developed for the oligoarticular form of juvenile idiopathic arthritis (JIA) as part of a consensus process. The aim was to establish standardized, evidence-based protocols for managing persistent oligoarticular JIA. The group of authors initially formulated 23 statements and circulated them in an online survey to medical members of the Society for Paediatric and Adolescent Rheumatology (GKJR). A total of 80 of the 124 paediatric and adolescent rheumatologists took part in the survey, which corresponds to just under 65% of the

Deep learning and radiomics-based system for early diagnosis of hip synovitis in juvenile idiopathic arthritis.

OBJECTIVE: Juvenile Idiopathic Arthritis (JIA) frequently affects children's hips, causing severe progression, but early hip synovitis lacks obvious symptoms and is hard to detect via conventional ultrasound, delaying diagnosis. magnetic resonance imaging (MRI), though accurate, is costly and inaccessible for routine use. This study aims to develop an automatic identification system for the early diagnosis of hip synovitis in JIA through the integration of deep learning and radiomics techniques. METHODS: A YOLO-JIA model specifically designed for the automatic segmentation of hip ultrasound i

Causal relationship between body mass index and risk of juvenile idiopathic arthritis: A 2-sample Mendelian randomization study.

The association between body mass index (BMI) and juvenile idiopathic arthritis (JIA) has been suggested, but the causal relationship remains unclear. Mendelian randomization (MR) offers a tool to address this causal question using genetic data. This study aims to investigate the causal relationship between BMI and JIA, providing genetic evidence to inform clinical prevention and treatment strategies. A total of 35 BMI-related single nucleotide polymorphisms were included as instrumental variables. Inverse variance weighting analysis revealed a significant positive association between increase

Altered B cell activation contributes to the immunopathogenesis of childhood arthritis-associated uveitis.

In Juvenile Idiopathic Arthritis (JIA), the most common childhood rheumatic disease, many patients also develop uveitis (JIA-uveitis), risking life-long vision loss. The mechanisms driving uveitis development in JIA remain understudied. Here, we demonstrate that peripheral blood CD19+IgD-CD27- double negative type 1 (DN1) B cells are elevated in JIA-uveitis compared to JIA patients without eye disease (JIA). The B cell receptor (BCR) repertoire was also more clonal and somatically hypermutated in JIA-uveitis and antigen-activated B cells infiltrated chronically inflamed JIA-uveitis eyes. Featu

Atypical Early Presentation of Multicentric Carpotarsal Osteolysis: Ankle Pain and Cavovarus Deformity Without Osteolysis.

Multicentric carpotarsal osteolysis (MCTO) is a rare skeletal dysplasia classically associated with carpal and tarsal osteolysis. Its presentation is highly variable, ranging from subtle musculoskeletal deformities to progressive bone loss and renal involvement. We report a nine-year-old female who presented with progressive ankle pain, cavovarus foot deformity, and gait disturbance. Initial radiographs demonstrated osteopenia and bilateral pes cavus without frank osteolysis. Genetic testing identified a pathogenic MAFB variant, confirming the diagnosis of MCTO. This case highlights the broad

Superb microvascular imaging ultrasound of the knee in patients with juvenile idiopathic arthritis-a repeatability study.

INTRODUCTION: Juvenile idiopathic arthritis (JIA) is the commonest rheumatologic disease in children and frequently affects the knee joint. Synovial inflammation and tenosynovitis are key pathological features, and ultrasound plays an increasingly important role in their assessment. Superb Microvascular Imaging (SMI) is a novel Doppler technique with enhanced sensitivity to low-velocity microvascular flow, but evidence on its repeatability in JIA remains limited. This study aimed to evaluate intra- and inter-observer repeatability of knee SMI in children with JIA. METHODS: In this prospective

Serum bile salt-stimulated lipase levels associates with systemic inflammation and declines with effect of treatment in juvenile idiopathic arthritis.

BACKGROUND: Juvenile idiopathic arthritis (JIA) is among the most common autoimmune diseases in children, yet its pathophysiology remains incompletely understood. Reliable biomarkers are needed to assess disease activity and guide therapy. Bile salt-stimulated lipase (BSSL), originally identified as a lipolytic enzyme, has recently been implicated in inflammatory conditions such as rheumatoid arthritis (RA) and psoriatic arthritis (PsA) in adults. Elevated serum BSSL levels correlate with markers of inflammation and decrease following positive response to anti-inflammatory treatments, suggesti

From Juvenile Idiopathic Arthritis to Pachydermoperiostosis: A Journey to an Unexpected Rare Diagnosis.

Pachydermoperiostosis, also known as primary hypertrophic osteoarthropathy or Touraine-Solente-Golé syndrome, is a rare genetic disorder that represents a small fraction of hypertrophic osteoarthropathy cases. It typically begins during adolescence, affects males more frequently and follows either an autosomal dominant or recessive inheritance pattern. The disease is characterized by the triad of pachydermia, periostosis and digital clubbing, often accompanied by hyperhidrosis, seborrhea, cutis verticis gyrata and joint effusions. Although articular involvement is usually non-erosive, the diso

Hip Joint Synovial Cavity Thickness in Early Juvenile Idiopathic Arthritis Without Effusion: A Cross-Sectional Ultrasound Study.

Background: The clinical meaning of hip joint synovial cavity thickness (HJSCT) on ultrasound (US) in juvenile idiopathic arthritis (JIA) without effusion is uncertain. Methods: In this cross-sectional study, we analyzed 369 children (187 JIA; 182 controls) undergoing hip US at a referral center in Kraków, Poland. JIA examinations were performed upon initial referral, early in the care pathway. We excluded patients with hip effusion and pre-existing inflammatory, traumatic or degenerative hip pathology. HJSCT was defined as the distance from the outer capsule margin to the femoral neck cortex.
