INTRODUCTION: Juvenile idiopathic arthritis-associated uveitis (JIA-U) is an important cause of childhood and adult visual impairment. Advances in surveillance approaches and treatment have improved outcomes, yet delayed detection and treatment-refractory disease remain substantial challenges. This review summarises recent developments across disease stratification, imaging, pharmacologic management, and psychosocial support, following the natural history of disease from inception to adulthood.
MAIN BODY: Risk-stratified screening based on antinuclear antibody status, JIA subtype, and age at

Advances in medications to treat paediatric rheumatic conditions including juvenile idiopathic arthritis (JIA) and systemic lupus erythematosus (SLE) have transformed outcomes in well-resourced settings. Access to therapeutic agents that are an accepted standard of care in many regions of the world remains unequal. Here we review access to medications through a global paediatric rheumatology lens, examining challenges from drug development to delivery. The high cost of novel medications, combined with complex research and regulatory environments, market forces, the rarity of conditions and pol

INTRODUCTION: Synovial fibroblasts (S-Fib) are recognized as key drivers of joint inflammation in adult arthritis. Moreover, data are limited on the processes driving monocyte activation in oligoarticular juvenile idiopathic arthritis (oJIA). Therefore, we aimed to explore if S-Fib from patients with oJIA induce monocyte activation. METHODS: S-Fib were isolated from the synovial fluid (SF) of n = 13 patients with oligoarticular juvenile idiopathic arthritis (oJIA) and n = 4 patients with rheumatoid arthritis (RA) to be used as a disease control. Healthy adult S-Fib were purchased. The interact

Despite major scientific advances, delivering high-quality care for children with inflammatory rheumatic and musculoskeletal diseases remains challenging. The field of paediatric rheumatology lies at the intersection of different disciplines, and requires excellent highly specialised medical expertise based on rapidly deepening knowledge in rheumatology and immunology. At the same time, this field relies on compassionate paediatricians understanding the needs of developing children as a vulnerable population. Training pathways and professional recognition vary widely between countries, and for

INTRODUCTION: Juvenile idiopathic arthritis (JIA) is the most common chronic rheumatic disease in children, with varying clinical and immunological features. Over the last 20 years, treatment options for JIA have significantly expanded, with the development of biologic and targeted synthetic disease-modifying antirheumatic drugs (bDMARDs and tsDMARDs), which have substantially improved clinical outcomes.
AREAS COVERED: This review provides a comprehensive assessment of bDMARDs and tsDMARDs in the management of JIA, focusing on their therapeutic efficacy, long-term safety profiles, and evolvin

BACKGROUND: We aimed to investigate the prevalence and risk factors of acute kidney injury (AKI) at juvenile idiopathic arthritis (JIA) onset and its impact on long-term kidney outcomes.
METHODS: In this multicenter study, we retrospectively reviewed 192 children diagnosed with JIA (1998-2025). AKI was defined according to KDIGO serum creatinine criteria. Kidney damage (KD) was defined as chronic kidney disease (CKD) and/or hypertension at follow-up. Logistic and Cox regression analyses were used to identify risk factors and to calculate odds ratio (OR) and hazard ratio (HR), respectively. Ka

BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis syndrome is a rare autosomal recessive disorder with camptodactyly, non-inflammatory arthropathy, coxa vara, and pericarditis. Symptoms usually begin in early childhood with swelling of interphalangeal joints, wrists, and knees. Pericarditis is rare. It is caused by proteoglycan 4 gene mutations and often misdiagnosed as juvenile idiopathic arthritis due to overlapping features.
CASE PRESENTATION: A 4-year-old boy presented with 6 months of knee and wrist swelling and nocturnal pain. His parents were first-degree cousins, and a mate

Arthritis, an acute inflammatory disease affecting single or multiple joints, causes irreversible damage to cartilage and bone, leading to substantial pain and economic burden. Current treatments lack specificity. Curcumin, with its anti-inflammatory and antioxidant properties, shows promise in arthritis treatment, yet its poor water solubility and low bioavailability hinder clinical use. This study investigates the efficacy of a nanocarrier-based curcumin delivery system for non-infectious arthritis. Four types of nanocarriers-liposomes, biomolecule-based nanoparticles, nanoemulsions, and imp

BACKGROUND: Juvenile Idiopathic Arthritis (JIA) includes a subset of oligoarticular patients who later extend to a polyarticular course (extended-to-be, ETB), resulting in worse outcomes. Predictors of extension remain poorly defined. We investigated whether interleukin-6 (IL-6) and fibroblast-like synoviocytes (FLS) contribute to this progression. METHODS: Synovial fluid samples from persistent oligoarticular patients who remained persistent (≤ 4 joints involved) throughout their disease course and ETB patients were analyzed using protein antibody arrays and ELISA. Importantly, ETB samples we

OBJECTIVE: The physician global assessment of lung disease (PGALD) is a recently proposed disease activity measure for patients with systemic juvenile idiopathic arthritis-associated lung disease (SJIA-LD). This study evaluates the reliability and construct validity of the PGALD.
METHODS: Fifty-seven pediatric rheumatologists and pulmonologists with experience caring for children with SJIA-LD were invited to rate 20 clinical vignettes using the PGALD, a 10-point Likert scale (0 = inactive SJIA-LD; 10 = highly active SJIA-LD). Raters were also asked to rate a subset of eight repeat vignettes.

OBJECTIVE: Although early diagnosis improves long-term outcomes, patients with juvenile idiopathic arthritis (JIA) often experience prolonged, circuitous paths to diagnosis. To inform diagnostic improvement, we sought to characterize health care utilization in the year preceding diagnosis.
METHODS: We identified 10,021 patients with an incident diagnosis of JIA and 20,042 age- and sex-matched healthy controls in the Merative MarketScan administrative datasets (2014-2022). Using negative binomial or hurdle models, we calculated incidence rate ratios (IRRs) comparing outpatient, inpatient, and

BACKGROUND: Paediatric-onset Still’s disease is a severe autoinflammatory disorder marked by high fever, systemic inflammation, rash, organomegaly, and arthritis. Pulmonary complications, once anecdotally reported, have been increasingly observed in paediatric Still’s disease. A more severe lung involvement, Still-associated lung disease, has emerged in the last ten years, including interstitial lung disease, pulmonary hypertension, and pulmonary alveolar proteinosis (PAP). This complication carries significant morbidity and mortality. Its mechanism remains poorly understood; current biologic

BACKGROUND: Asthma and juvenile idiopathic arthritis (JIA) are common pediatric inflammatory conditions that may share immunologic and environmental risk factors. Previous studies have reported inconsistent associations and often lacked rigorous control for confounding. This study investigated whether asthma is associated with increased odds of JIA in a large, population-based sample. METHODS: We analyzed data from 169,786 participants aged 0–17 years from six cycles of the National Survey of Children’s Health (2016–2021). Parent-reported and provider-diagnosed asthma and JIA were identified u

BACKGROUND: Children with a chronic inflammatory disease (CID) are at increased risk of cardiovascular disease and show an impaired flow-mediated dilation (FMD) response compared to their healthy peers, making it critical to monitor their cardiovascular health over time. FMD is a repeatable measure of vascular function in healthy children and adults. However, repeatability can vary in adult populations with disease and inflammation, and the repeatability of FMD in children with a CID has not been explored.
OBJECTIVES: The objective of this study was to assess the repeatability of brachial art

The gut microbiota, a vast community of symbiotic microorganisms inhabiting our gut, has been recognized as a key-lever for human health, shaping immune system resilience and being essential for immunological homeostasis throughout the life course. Gut microbiota composition may influence both initiation and/or perpetuation of intestinal inflammation, but recent research has highlighted its contribution to both rising and progression of protean non-intestinal inflammatory diseases: indeed, a perturbation of host-associated microbiota during critical developmental stages like early childhood ca

OBJECTIVES: Limited information is available on patients with non-systemic juvenile idiopathic arthritis (nsJIA) receiving biologics in Japan. The types of biologics, treatment duration, prior and concomitant treatments, administration routes (intravenous [IV] or subcutaneous [SC] injection), and patient characteristics were investigated.
METHODS: We investigated nsJIA, excluding psoriatic arthritis and ankylosing spondylitis, using data from a Japanese hospital claims database (2008-2024).
RESULTS: Of the 4191 nsJIA patients in the database, 965 (23.0%) received one or more biologics. For t
