Cost-Utility and Budget Impact Analysis of Tumor Necrosis Factor-Alpha Inhibitors for the Treatment of Refractory Nonsystemic Juvenile Idiopathic Arthritis in Thailand.

OBJECTIVES: To evaluate the cost-utility and budget impact of introducing individual tumor necrosis factor-alpha inhibitors, including adalimumab, etanercept, infliximab, and golimumab, as add-on therapies for patients aged 2 years or older with refractory nonsystemic juvenile idiopathic arthritis in Thailand. METHODS: Decision-tree and Markov models were used to conduct a cost-utility analysis from a societal perspective to estimate lifetime costs and health outcomes. Direct medical and nonmedical costs were obtained from a hospital database and a previous study, respectively. Health-related

Effects of active action observation on cognitive, emotional, motor, and somatosensory outcomes in adolescents with juvenile idiopathic arthritis: a prospective exploratory case series.

BACKGROUND: Juvenile Idiopathic Arthritis (JIA) is a heterogeneous pediatric rheumatic disease characterized by persistent pain, functional limitations, and psychosocial difficulties that frequently persist despite optimized pharmacological treatment. Active Action Observation (AAO), which involves observing and then executing goal-directed movements, has been shown to engage mirror-neuron and visuomotor networks. This approach has shown promise in neurological and orthopedic rehabilitation, but has not been studied in adolescents with JIA. OBJECTIVE: To assess the feasibility, acceptability,

Effects of very early arthritis on pain, behavior, and hippocampal TNF/BDNF in rats.

BACKGROUND: Juvenile idiopathic arthritis (JIA) is the most common childhood chronic arthritis, and pain may persist despite controlled inflammation, potentially due to central sensitization. This study aimed to evaluate the effects of very early-onset arthritis on pain, behavior, and cognition using a collagen-induced arthritis model in juvenile rats. METHODS: Thirty-six three-week-old male Wistar rats were divided into control, sham (saline), and arthritis (type II collagen with incomplete Freund's adjuvant) groups. Disease severity was monitored via joint thickness and VAS. Pain (hot plate

A Narrative Review of Secukinumab in Spondyloarthritis and Psoriatic Arthritis: Treating the Whole Patient.

Spondyloarthritis (SpA), including axial spondyloarthritis (axSpA) and psoriatic arthritis (PsA), comprises a diverse but interrelated family of inflammatory diseases characterized by inflammation of the joints and spine. Signs and symptoms of SpA vary widely, as patients experience a diverse spectrum of musculoskeletal and extra-musculoskeletal disease manifestations. Interleukin (IL)-17 is an inflammatory cytokine common to the pathogenesis of SpA disease development and progression. The IL-17A inhibitor secukinumab has been well established in clinical trials and real-world practice as an e

Single-centre observational study of anti-adalimumab antibody development in children and young people treated for juvenile idiopathic arthritis and/or non-infectious uveitis.

BACKGROUND: Adalimumab is an effective first-line biologic therapy used to treat children and young people (CYP) with juvenile idiopathic arthritis (JIA) and non-infectious uveitis (NIU). However, the development of neutralising antibodies may compromise adalimumab's efficacy. This study investigated the incidence and clinical relevance of anti-adalimumab antibody (AAA) development in CYP treated with adalimumab for JIA and/or NIU. METHODS: A retrospective observational study was conducted within a specialist paediatric rheumatology service at a tertiary UK centre. CYPs receiving adalimumab b

A proposal for the implementation of telemedicine in Japan for rheumatoid arthritis, juvenile idiopathic arthritis, and systemic lupus erythematosus: Review article.

Telemedicine is a promising solution for addressing regional disparities, and for improving access to specialized care for patients with rheumatic diseases in Japan. This review provides a comprehensive overview of the current challenges, proposed clinical models, and future directions for implementing telemedicine for these conditions. Drawing from the European Alliance of Associations for Rheumatology points to consider and national surveys, we highlight the potential of hybrid care models, the importance of digital literacy, and the need for interdisciplinary collaboration in this field. Etiologies of Wrist Arthritis in the Young Patient.

Most young patients with wrist arthritis will present with a history of trauma as their inciting event. Common causes of wrist arthritis in young patients are intercalated segment pathologies such as scapholunate advanced collapse and scaphoid nonunion advanced collapse, postdistal radius fracture arthritis, and pisotriquetral arthritis. Less common causes of wrist arthritis in young patient are nontraumatic in nature and encompass a wide range of etiologies that are idiopathic (eg, Kienbock and Preiser disease), congenital (Madelung deformity), metabolic (chondrocalcinosis), and inflammatory

Clinical predictors of relapse and severe disease phenotype in children with non-systemic juvenile idiopathic arthritis.

UNLABELLED: The aim of this study was to identify the predictors of relapse and severe disease in non-systemic juvenile idiopathic arthritis (JIA), a heterogeneous childhood disease. Patients with JIA were grouped based on relapse status, and those with ≥ 2 relapses requiring biologics were classified as severe disease phenotype. A total of 142 patients (63.4% female) were included in the study. Seventy-three patients (51.4%) experienced at least one relapse after achieving remission, who were significantly characterized by female gender, younger age at diagnosis, positive ANA test, and longer

The gut-joint axis in juvenile idiopathic arthritis.

Juvenile idiopathic arthritis (JIA) is an umbrella term for a group of oligo- and poly-arthritic conditions that develop before 16 years of age. While the exact pathophysiology of JIA is still unknown, both genetic and environmental factors are known to contribute to the development and severity of all JIA subtypes. Of the possible environmental factors involved, there is growing evidence that changes in intestinal homeostasis, or a ‘gut-joint axis’, can be observed in both animal models of arthritis and in adult arthritis. However, fewer studies have focused on whether similar processes contr

Methotrexate Therapy in Juvenile Idiopathic Arthritis: No Clinically Relevant Pulmonary Impairment but Frequent Transient Liver Enzyme Elevations in a Longitudinal Single-Center Pediatric Cohort of 274 Children Over 30 Years.

OBJECTIVE: Methotrexate (MTX) is a cornerstone in treating juvenile idiopathic arthritis (JIA). The long-term impact of MTX, particularly on pulmonary and liver function, remains a concern. We longitudinally examined the effects of MTX on pulmonary and hepatic function in the largest reported cohort to date. METHODS: In this retrospective, single-center study, patients with JIA receiving MTX (10-15 mg/m2) between 1993 and 2023 were analyzed. Pulmonary function tests (PFTs), including body plethysmography and diffusing capacity for carbon monoxide (DLCO), were performed annually (±3 months) st

Performance evaluation of an adalimumab biosimilar autoinjector and its reference device: comparative use human factors study in patients with rheumatoid arthritis and juvenile idiopathic arthritis.

A Comparative Use Human Factors study was conducted to assess whether the injection success rate (SR) with biosimilar adalimumab-fkjp (Bios-ADA) autoinjector (Hulio®) is comparable to reference adalimumab (Ref-ADA) autoinjector (Humira®), accounting for design differences, including the absence of an activation button, to evaluate usability in the target patient population. A counterbalanced, multicenter study was conducted under simulated conditions, with patients completing multiple steps to administer the medication (40 mg Bios-ADA or Ref-ADA) into an injection pad using both devices. Modeling, Simulation, and Extrapolation to Optimize Pediatric Development Plan: Sarilumab Polyarticular-Course juvenile arthritis Application.

Polyarticular-course juvenile idiopathic arthritis (pcJIA) is a chronic condition that manifests before the age of 16 years, with pathology similar to that of adult rheumatoid arthritis (RA). Sarilumab is an interleukin-6 receptor inhibitor approved for RA and pcJIA. To obtain the approval for pcJIA, a single-arm, multiple-dose phase 2 study was conducted to determine the sarilumab dose for pcJIA. The population pharmacokinetics analysis of the phase 2 dose-finding portion data showed comparable pharmacokinetics; exposure–response analyses demonstrated similar or greater efficacy (JIA-ACR30/50

Sociodemographic and psychosocial correlates of self-compassion in adolescents with physical health conditions.

BACKGROUND: Adolescents with physical health conditions (PHCs) face the dual demands of both normative developmental and disease-related burdens. Self-compassion (SC) represents a potentially important resource, yet little is known about its correlates. This study investigated sociodemographic, health-related, and interpersonal predictors of compassionate self-responding (CS) and uncompassionate self-responding (UCS) among adolescents with PHCs. METHODS: In total, 498 adolescents (ages 12–21) with type 1 diabetes, cystic fibrosis, and juvenile idiopathic arthritis completed measures of SC, dep

Septic arthritis in neonatal and juvenile New World camelids (5 cases).

Septic arthritis (SA) in camelids is infrequently reported in the literature. The purpose of this study was to describe the clinical presentation, etiologies, diagnostics, treatment and short- and long-term outcome in neonatal and juvenile New World camelids (NWCs) with SA. Two neonatal alpaca crias were diagnosed with SA associated with failure of transfer of passive immunity and neonatal sepsis. Three juvenile camelids, 1 alpaca, and 2 llamas were presented with an infection of a single joint with associated lameness. All animals were treated with systemic and intra-articular antimicrobials.
