Whole-body magnetic resonance imaging in rheumatology: advancements, key applications and future perspectives.

Whole-body magnetic resonance imaging (WB-MRI) is widely used in rheumatology to assess peripheral and axial joints and entheses throughout the body. Despite some limitations, it has potential in determining the overall inflammatory burden, tracking disease progression, and evaluating treatment response. It is used in the evaluation of idiopathic inflammatory myositis (IIM) or in the pediatric population where it is becoming the gold standard for the diagnosis and monitoring of conditions, such as juvenile idiopathic arthritis (JIA) and chronic recurrent multifocal osteomyelitis (CRMO). In add

Temporomandibular joint manifestation of autoimmune diseases.

This research compilation examines the frequency and consequences of abnormalities in the temporomandibular joint (TMJ) associated with various autoimmune diseases, with an emphasis on arthritic conditions. When the immune system of the body unintentionally targets its own tissues, autoimmune illnesses arise. This causes inflammation and damage to several organs, including joints. This study describes how rheumatoid arthritis affects the TMJ and emphasize how it affects oral dysfunction; provide a preliminary report on TMJ involvement in Psoriasis, highlighting the need of early diagnosis and

Osteopathia striata with cranial sclerosis, associated with juvenile idiopathic arthritis: A case report and review of literature.

BACKGROUND: Osteopathia striata with cranial stenosis (OSCS) is a rare genetic disorder (Mendelian Inheritance in Man: 300373) inherited in an X-linked dominant pattern. It is classified as a form of skeletal dysplasia and is characterized by linear striations of bone sclerosis, primarily affecting the long bones. OSCS may present as an isolated condition or as part of broader genetic conditions such as Horan-Beighton and Goltz syndromes. Thus far, approximately 100 cases have been reported. CASE SUMMARY: Herein, we report the case of an 11-year-old girl with OSCS in association with juvenile

An etiopathogenesis of juvenile idiopathic arthritis: the protein-homeostasis-system hypothesis.

The initiation of juvenile idiopathic arthritis (JIA) may be associated with an infection caused by unidentified pathogens. The prevalence or incidence rates of JIA differ markedly among populations. The constituent of microbiota of human species is influenced by age during childhood and differs among ethnic groups. On occasion, some strains in microbiota can invade the host and elicit inflammatory immune reactions, and dysbiosis has been observed in JIA. The microbial-infected cells contain inflammation-inducing substances including pathogen-origin substances such as toxins and pathogen-assoc

Moving towards optimized treatment for children and adolescents with juvenile idiopathic arthritis in sustained remission randomized to continue stable treatment, methotrexate withdrawal or tumor necrosis factor inhibitor withdrawal: study protocol for the Norwegian multi-center MOVE-JIA trial.

BACKGROUND: Juvenile idiopathic arthritis (JIA) used to be a joint-destroying disease, but thanks to modern treatment strategies and medications, many patients with JIA today reach inactive disease. However, once disease remission is achieved, there is a lack of knowledge and recommendations regarding maintenance therapy. Drug-free remission is the ultimate goal in JIA, but withdrawal of medications increases the risk of disease flare. Clinical approaches vary widely, underscoring a need for knowledge about maintenance treatment strategies that allow for safe tapering and withdrawal of medicat

Mechanobiologic modulation of TMJ loading in juvenile idiopathic arthritis.

OBJECTIVE: TMJ arthritis is common in juvenile idiopathic arthritis (JIA). Condylar height loss can shift mandibular loading and increase compressive TMJ stress, which may persist even when synovitis is controlled with biologics. We hypothesize that light elastics delivered via anchorage devices (TADs) can partially unload the joint. METHODS: This hypothesis driven biomechanical model integrates condylar support loss, lever geometry, hyperdivergent susceptibility, and feasibility of routing low forces through a perioral soft tissue corridor. A cephalad inter implant elastic vector of ~25 to 5

Case Report: A case of autoinflammatory disease with a novel NLRP12 variant-clinical presentation and successful treatment with baricitinib.

NLRP12-associated autoinflammatory disease (NLRP12-AID) is a rare monogenic disorder. The p.Glu619Gln (c.1855G > C) variant in NLRP12 is classified as a variant of uncertain significance (VUS), with no previously reported clinical cases. We describe a 16-year-old Chinese girl with a 9-year history of periodic high fevers (>39 °C), cold-triggered urticarial rashes, and polyarticular arthralgia. Previous misdiagnoses included recurrent infections and juvenile idiopathic arthritis. Laboratory tests showed elevated levels of interleukin-6 (IL-6) and acute-phase reactants. A lymph node biopsy confi

Transition of serum-free IL-18 levels in systemic juvenile idiopathic arthritis measured by a novel detection method.

BACKGROUND: Systemic juvenile idiopathic arthritis (sJIA) is a pediatric-onset autoinflammatory disease that may progress to fatal macrophage activation syndrome (MAS). Free interleukin-18 (IL-18), which is unbound to IL-18 binding protein (IL-18BP), may reflect its biological activity, but its accurate measurement remains challenging and its clinical usefulness in sJIA remains unclear. METHODS: We generated a NanoLuc fused IL-18BP plasmid, expressed and highly purified the protein with the silkworm protein expression system, and used it to develop an Enzyme-Linked Immunosorbent Assay for mea

Inosine-Triphosphate-Pyrophosphatase Activity as a Potential Predictor of Methotrexate Remission in Juvenile Idiopathic Arthritis.

OBJECTIVE: Methotrexate (MTX) is the first-line therapy for juvenile idiopathic arthritis (JIA), but up to 40% of patients do not respond to it. Low inosine triphosphate pyrophosphatase (ITPA) activity has been associated with reduced clinical remission. We investigated the role and underlying mechanisms of ITPA in vitro. METHODS: ITPA enzymatic activity was measured by high-performance liquid chromatography with ultraviolet detection in erythrocyte lysates from patients with JIA (enrolled in Trieste, Italy; Florence, Italy; and Kansas City, Missouri) receiving weekly MTX (10-15 mg/m2) for at

Challenges in functional validation and mechanistic interpretation of a novel LACC1 variant in familial juvenile arthritis.

In their recent publication, Alblooshi et al. report the identification and preliminary functional analysis of a novel LACC1 variant, p.Asp220Asn, associated with familial juvenile idiopathic arthritis. While their study provides a valuable genetic association, this letter offers a critical appraisal to contextualize the findings and propose essential future directions. We contend that the functional characterization, while indicative of a loss-of-function, is insufficient to distinguish it from a potential dominant-negative mechanism, a distinction with paramount implications for genetic coun

Metabolic and Anthropometric Alterations in Juvenile Idiopathic Arthritis: A Focus on Cardiometabolic Risk and Non-Invasive Evaluation Methods.

Juvenile idiopathic arthritis (JIA) is the most prevalent chronic rheumatologic condition in childhood, with an incidence that continues to rise worldwide. Despite substantial progress in therapeutic strategies over the past two decades, JIA remains a major health concern. Beyond joint inflammation and musculoskeletal impairment, accumulating evidence indicates that JIA is associated with metabolic disturbances and altered body composition, which may predispose affected children to an elevated cardiovascular risk in the long term. The objective of this review is to synthesize current knowledge
