Ulcerative colitis and Crohn's disease are significant childhood illnesses. With their many extraintestinal manifestations, they may masquerade as fever of unknown etiology, arthritis, anorexia nervosa, growth hormone deficiency, collagen-vascular disease, idiopathic growth retardation and even irritable bowel syndrome of childhood. In any child who presents with growth failure and/or chronic abdominal pain with fever or weight loss, the diagnosis of inflammatory bowel disease must be considered. As in any other chronic disease of childhood, long-term management will often challenge the physic

Suppressor cell function of normal peripheral blood mononuclear cells was unaltered by preincubation with sera from patients with severe inflammatory bowel disease (IBD). These results suggest that, in contrast to juvenile rheumatoid arthritis and systemic lupus erythematosus, the decreased suppressor cell activity in IBD is not mediated by anti-suppressor cell antibodies.

In a large number of rheumatoid factor-negative arthritic conditions there is a number of diseases which are characterized by common clinical manifestations and the genetic property of the presence of B27. The term "B27-positive diseases" is based on a concept arising from intensive nosological studies and provides important suggestions for the study of the pathogenesis of a group of rheumatic diseases in that it emphasizes the significance of a genetic predisposition and makes an infectious etiology probable.

Most chronic arthritis in childhood is seronegative. Within "JRA" several distinct subgroups exist: one of these (pauciarticular disease type II) affects predominantly boys more than 8 years of age. It is clearly associated with sacroiliitis, HLA-B27, family history of spondyloarthropathy, and subsequent ankylosing spondylitis in an as yet underfined percentage of patients. This type of disease is probably classified appropriately with the spondyloarthropathies, although patients often may fulfill diagnostic criteria for "JRA" in the first years of their disease, and accounts for about 15% of

A group of diseases--ankylosing spondylitis, psoriatic arthritis, Reiter's disease, ulcerative colitis, Crohn's disease, juvenile chronic arthritis (Still's disease), Whipple's disease, and Behçet's syndrome, which previously were thought to be variants of rheumatoid arthritis, now appear to be distinct from that disease but linked together in a group termed "seronegative spondyloarthropathies." Detailed studies have been made of the clinical, radiologic, and serologic features of these individual diseases. Extensive family studies have been done epidemiologically and through examining pedigre

We studied the intestinal morphology, and the jejunal and rectal immunoglobulins of 16 children with partial IgA deficiency, defined as serum IgA concentration more than two standard deviations below the mean for age, but higher than the lower limit of sensitivity of single radial immunodiffusion (0.02 g/l). Five of the patients had been treated with phenytoin, 2 had juvenile rheumatoid arthritis, 2 had ulcerative colitis and 5 had recurrent upper respiratory tract infections. The jejunal morphology was normal in every case. In 6 cases normalization of serum IgA occurred during the follow-up,

Salicylates provide the backbone of therapy in juvenile rheumatoid arthritis. They are effective in controlling the disease approximately 75% of the time if they are properly used. Salicylate administration is relatively safe if carefully done. Serum salicylate levels should not exceed 30 mg/dl routinely. Patients, physicians, and parents should be alert to early clinical signs of toxicity. Chief hazards of chronic salicylate administration other than salicylism (which should be uniformly preventable) include gastric irritation with questionable relationship to peptic ulcer disease, and rare s

Eighty-three consecutive patients with rheumatoid factor-negative polyarthritis seen during a 1-year period were evaluated clinically, radiologically, and with the B27 test. Patients with definite spondylitis, juvenile chronic polyarthritis, a collagen disease, a known metabolic arthropathy, or primary generalized osteoarthritis were excluded. The patients could be classified into two groups independent of any knowledge of B27 testing. Twenty-five had a spondylitic "variant" syndrome. These could be diagnosed on clinical grounds, and included a male preponderance and a high frequency of B27 po

Corticosteroids, although potent suppressors of symptoms and signs of disease, are not effective in curing rheumatoid arthritis in adults or children. Prolonged use of corticosteroids is attended with multiple serious side effects. In JRA, the outlook for most children is good; potentially dangerous drugs should be avoided if at all possible. The indications for corticosteroid usage in JRA might be summarized as: Severe systemic disease unresponsive to salicylates Chronic iridocyclitis unresponsive to topical steroids. Only very rarely for severe arthritis per se; and then with the realization

Recent reports of and our own experience with biochemical alterations of liver function secondary to salicylate therapy stimulated this prospective study. Thirty-four children with juvenile rheumatoid arthritis, 6 children with acute cartilagenous necrosis of the hipfollowing slipped capital femoral epiphysis, and 2 children with ulcerative colitis and hip disease who were on salicylates were followed over a period of 1-27 months with serial determinations of serum glutamic oxaloacetic transaminase (SGOT), serum glutamic pyruvic transaminase (SGPT), lactic dehydrogenase (LDH), alkaline phospha
