Diseases and Mortality in Confiscated Birds and Reptiles Housed in a Wildlife Rescue Center Under the CITES Directive.

The number of confiscated CITES-listed animals has increased dramatically worldwide, creating significant health, logistical, and resource challenges for responsible authorities. Rescue centers represent a scientific and humanitarian response to this challenge, providing solutions through rehabilitation, research, and environmental education. This postmortem survey provides information on disease and mortality during a four-year period, in confiscated CITES-listed birds and reptiles housed in an authorized rescue center. A total of 29 animals (17 birds and 12 reptiles) were examined by necrops

Associations of Semaglutide With Skeletal Outcomes in People With Obesity, With and Without Type 2 Diabetes: A Target Trial Emulation.

AIMS: To evaluate the associations between semaglutide initiation and long-term skeletal outcomes in people with obesity, stratified by type 2 diabetes (T2D) status, using target trial emulation. MATERIALS AND METHODS: This retrospective cohort study used the TriNetX federated electronic health record network. People with obesity initiating semaglutide were matched 1:1 via propensity score matching (215 covariates) to those initiating active comparators or usual care. The with-T2D cohort (n = 19 824-93 519 matched pairs per comparison) was followed for 3 years; the without-T2D cohort (n = 10

Real-world efficacy and safety of immune checkpoint blockades in advanced urothelial carcinoma with histologic variant subtypes: the pembroblad study.

BACKGROUND: Histologic variants (HV) of urothelial carcinoma (UC-V) and non-urothelial carcinomas (NUC) of the bladder and urinary tract are aggressive malignancies with poor prognosis and limited response to platinum-based chemotherapy. Despite their biological particularities and distinct immune microenvironments, these HV subtypes are under-represented in immune checkpoint blockade (ICB) trials. METHODS: PEMBROBLAD is a French national multicenter retrospective study conducted across 24 centers. We included patients with advanced UC-V or NUC who received second-line anti-PD-(L)1 monotherap

A Rare Case of Septic Arthritis Secondary to Actinomyces europaeus Infection.

Actinomyces europaeus is an uncommon cause of human infection and a rare cause of septic arthritis. We report an 81-year-old woman with diabetes mellitus, chronic kidney disease, gout, and dementia who developed septic arthritis of the distal interphalangeal joint of the right third finger during hospitalization for Escherichia coli bacteremia. Computed tomography demonstrated erosive joint destruction with an adjacent abscess. Surgical incision, drainage, and arthrotomy revealed septic arthritis with associated abscess formation and degeneration of the terminal extensor tendon. Operative cult

Rheumatoid Arthritis Mimickers Presenting With Polyarticular Arthritis: A Case Series Highlighting Diagnostic Pitfalls.

Rheumatoid arthritis (RA) is the most common chronic inflammatory arthritis and a leading cause of disability worldwide. However, several diseases may mimic RA, particularly in patients presenting with polyarticular pain or seronegative inflammatory arthritis. Misdiagnosis may result in inappropriate immunosuppressive therapy and delayed treatment of the underlying disease. Recognizing these mimickers remains a critical challenge in rheumatology practice. We conducted a retrospective case series of patients evaluated at a tertiary rheumatology center between January 2021 and December 2025. Pat

Racial Disparities in Severe Cutaneous Adverse Reactions: A Report of Two Cases on Preventable Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis Prescription Errors Through Medication Stewardship.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe, life-threatening cutaneous adverse reactions often triggered by medications. Common medications implicated in SJS and TEN include antibiotics, anticonvulsants, allopurinol, and nonsteroidal anti-inflammatory drugs (NSAIDs). Polypharmacy, particularly inappropriate or unnecessary medication use, increases the risk of severe adverse drug reactions. We present a case report discussing two patients with SJS/TEN secondary to non-indicated medication administration. The first patient was a 28-year-old male with antibioti

SGLT2 Inhibitors in Nephrolithiasis: Emerging Evidence and What Urologists Need to Know.

BACKGROUND: Nephrolithiasis frequently coexists with type 2 diabetes, obesity, gout, chronic kidney disease, and other cardiometabolic disorders. Sodium-glucose cotransporter 2 (SGLT2) inhibitors may influence stone formation through effects on glucosuria, natriuresis, urine volume, urinary citrate, pH, urate handling, and relative supersaturation. This review aimed to examine the current mechanistic, biochemical, and clinical evidence linking SGLT2 inhibition to nephrolithiasis, with particular attention to its relevance for urological practice. METHODS: A clinically oriented narrative revie

Gout-Related Medication Use After Initiating Sodium-Glucose Cotransporter 2 Inhibitors in Patients With Gout and Type 2 Diabetes: Population-Based Target Trial Emulation Studies.

OBJECTIVE: Sodium-glucose cotransporter 2 inhibitors (SGLT2is) lower serum urate and are associated with a lower risk of recurrent gout flares. We used target trial emulation to compare rates of allopurinol initiation and use of anti-inflammatories (high-dose glucocorticoids, nonsteroidal anti-inflammatory drugs [NSAIDs], colchicine) and diuretics (prototypic serum urate-raising medication) among patients with gout using SGLT2is versus dipeptidyl peptidase 4 inhibitors (DPP-4is) (primary comparator), with glucagon-like peptide 1 receptor agonists (GLP-1RAs) as an alternative comparator. Use of clustering techniques for clinical and epidemiological research: practical tips using an example from rheumatology.

Clinical and epidemiological researchers may want to identify or determine groups with similar characteristics that exist within a dataset or population. For this purpose, clustering techniques can be applied. However, clustering can be complex, and results can be misleading if the methods are not applied with fidelity. The aim of this paper is to provide practical guidance to (clinical) researchers in rheumatology and other fields who wish to explore or confirm (sub) groups within their study population, and are considering to apply clustering techniques to (patient) data. We discuss when clu

[Gout of the manubriosternal joints: A case report].

Gout is a metabolic disorder caused by abnormalities in purine metabolism and/or impaired uric acid excretion. It is characterized by the deposition of monosodium urate crystals in joints or connective tissues, leading to recurrent episodes of acute arthritis. Gout can be categorized into two major types based on its etiology: primary and secondary. The majority of gout cases are associated with impaired uric acid excretion. The natural course of gout can be divided into three stages: (1) asymptomatic phase; (2) acute arthritis phase and intercritical phase; (3) chronic gouty arthritis and top

Toxic Epidermal Necrolysis: A Rare Complication of Allopurinol.

Toxic epidermal necrolysis (TEN) is a rare, life-threatening, mucocutaneous disorder characterized by extensive epidermal detachment and necrosis, often triggered by medications. Prompt recognition and management are critical to improving outcomes. The present article reports a case of an 80-year-old patient, with a history of gout and essential hypertension, who presented to the emergency department with pyrexia and a generalized macular rash characterized by dark centers and vesicles, accompanied by severe pruritus. The patient had recommenced allopurinol therapy one week prior, following an

Monitoring and Management of Uric Acid Therapy in Gout and Chronic Kidney Disease: A Single-Center Retrospective Study.

Background Gout commonly coexists with chronic kidney disease (CKD) due to reduced renal excretion of uric acid (UA). Guidelines recommend regular monitoring and dose adjustment of urate-lowering therapy (ULT), but the rate of adherence to these guidelines is not well established. Our study aimed to determine adherence to ULT guidelines in gout patients at our institution. In particular, we sought to assess the effect of CKD as well as other comorbidities on the prevalence of ULT guideline adherence. Methods We conducted a retrospective cohort study of 5,985 gout patients at our institution in

Design of a randomised, placebo-controlled, double-blind multicentre study assessing the effect of colchicine on the incidence of knee or hip replacements in symptomatic knee or hip osteoarthritis: the ECHO trial.

INTRODUCTION: Osteoarthritis (OA) is a multifactorial disease in which low-grade inflammation is considered to play a pivotal role. Although colchicine is a widely used anti-inflammatory drug in the treatment of gout, its effect in OA is still disputed due to inconsistent results of short-term clinical trials. Therefore, we aim to evaluate the effect of long-term colchicine 0.5 mg once daily on the incidence of knee or hip replacements in patients with knee or hip OA. METHODS AND ANALYSIS: The ECHO trial is a prospective, multicentre, randomised, double-blind, placebo-controlled, phase III tr

Efficacy of Colchicine in Reducing Cardiovascular Events in Patients with Acute Coronary Syndrome: A Systematic Review.

Acute coronary syndrome (ACS) remains one of the leading causes of cardiovascular morbidity and mortality, largely due to the high incidence of recurrent adverse events. Colchicine, a traditional anti-inflammatory drug primarily used for the treatment of gout, has emerged as a potential therapeutic option in cardiovascular care due to its ability to stabilize atherosclerotic plaques. This systematic review aimed to evaluate the efficacy of colchicine in reducing cardiovascular events and inflammatory biomarkers in patients with ACS. Eight studies published between 2013 and 2024 were included,
