BACKGROUND: Gout is a common inflammatory arthritis that affects ∼4% of the US population. Most patients with gout are >50 years of age and have multiple comorbidities. Gout is caused by the deposition of monosodium urate crystals in joints secondary to hyperuricemia. Gout typically presents as an acute painful inflammation (flare) involving one or more joint. Parathyroid dysfunction, leading to severe clinical symptoms and radiographic changes, has decreased over the last years due to routine laboratory checks including serum calcium levels. Thus, abnormal calcium levels are detected early in the course of the disease and the underlying cause treated accordingly. Hyperparathyroidism often leads to osteoporosis and low-trauma fractures. When evaluating secondary osteoporosis analysis of calcium, phosphate and intact parathyroid hormone levels are mandatory. Osteitis fibrosa cystica and brown tumors are less frequent findings of hyperparathyroidism.

We present a patient with gout and a total knee arthroplasty that failed secondary to infection. The English literature records only 5 other cases of gouty attacks after total knee arthroplasty. Symptoms, presentation, and laboratory tests do not definitively differentiate between infection and gout, but crystals in the joint aspirate are diagnostic. This case stresses the importance of recognizing crystalline arthropathy as a source of a painful knee and as a complicating factor during revision surgery.
