Hydroxyapatite deposition disease (HADD) is a relatively common disorder characterised by periarticular deposition of calcium hydroxyapatite, most frequently involving tendons of the glenohumeral joint. Although calcific deposits typically produce a self-limiting inflammatory response, mass-like presentations at atypical sites can mimic trauma, infection or soft tissue neoplasia, posing a significant diagnostic challenge. We report an unusual presentation of mass-like calcific tendinitis within the flexor carpi ulnaris (FCU) tendon of the distal forearm-an atypical location for HADD presenting

OBJECTIVES: To assess the effectiveness and safety of anakinra in crystal-related disease (CRD) flares, and to identify factors associated with treatment failure and risk of infection.
METHODS: We conducted a single-centre, retrospective, observational study including adult patients treated with anakinra for a CRD flare between January 2011 and July 2024. Patients were identified in a hospital clinical data warehouse using an electronic query. CRD was confirmed by the presence of crystals in synovial fluid or by characteristic radiological features. Clinical and biological data were collected

PURPOSE: To present a series of 37 cases diagnosed with ultrasound (US) of symptomatic hydroxyapatite deposition disease (HADD) involving the wrist and hand.
MATERIALS AND METHODS: Reports from two institutions were searched using the keywords "HADD hand/wrist" or "calcium deposit hand/wrist". The US examinations were performed with a similar standardized approach, focusing on the region of pain. B-mode and Doppler sonograms were acquired. Each author reviewed the cases to ensure that only cases consistent with HADD were included.
RESULTS: A total of 37 patients were identified, mostly women

INTRODUCTION: Calcific tendinitis (hydroxyapatite deposition disease) most commonly affects the shoulder and peri-trochanteric hip region. Involvement of the linea aspera is uncommon and may mimic aggressive pathology due to associated cortical reaction and marrow edema.
CASE REPORT: A 53-year-old female presented with predominantly right-sided upper thigh pain. Radiographs demonstrated amorphous calcification along the lateral proximal femur at the linea aspera. Magnetic resonance imaging showed a calcific focus with marked adjacent marrow and soft-tissue edema on the right, while the contra

Calcific tendinitis is a common musculoskeletal disorder characterized by deposition of calcium hydroxyapatite within or around tendon tissue. Although it has traditionally been regarded as a structural tendon pathology, growing clinical evidence suggests that symptom severity, particularly pain, is more closely related to inflammatory activity and disease stage than to the size or precise anatomical location of calcific deposits. In routine practice, patients most often seek medical attention during sudden and intense pain episodes, typically occurring in the resorptive phase. Current diagnos

INTRODUCTION: Hydroxyapatite deposition disease (HADD) is caused by the presence of hydroxyapatite crystals in periarticular spaces oftentimes leading to inflammation, pain, and decreased range of motion.
CASE REPORT: A 40-year-old right hand dominant female presented with three days of atraumatic right shoulder pain. Radiographs of the right shoulder were negative. Computed tomography revealed a hydroxyapatite deposit adjacent to the acromioclavicular joint. The patient was managed with nonsteroidal anti-inflammatory drugs and a referral to orthopedic surgery.
CONCLUSION: Many instances of

Ultrasound (US) is a valuable tool in the evaluation of arthritis both for diagnosis and treatment response. Pertinent findings such as joint effusions, synovitis, bursitis, bone erosions, tenosynovitis, and enthesitis can all be readily evaluated sonographically. In this article, we describe specific considerations in the US evaluation of rheumatoid arthritis, psoriatic arthritis, juvenile idiopathic arthritis, crystalline arthritis (gout, calcium pyrophosphate deposition disease, and hydroxyapatite deposition disease), septic arthritis, and osteoarthritis with attention to certain differenti

Crystal arthropathies of the hand encompass a wide spectrum of disorders characterized by the deposition of various crystals within joints and soft tissues, often leading to inflammatory and degenerative changes. Due to their ability to mimic other rheumatic diseases, accurate diagnosis is challenging and relies heavily on imaging. This review highlights the pathophysiologic features and typical manifestations of key crystal arthropathies-calcium pyrophosphate crystal deposition, gout, hydroxyapatite deposition disease, oxalosis, amyloidosis, and rare mimickers such as ochronosis-focusing on t

Hydroxyapatite crystal deposition disease (HADD) poses diagnostic challenges in the emergency department (ED) as it may clinically present similarly to infection and other musculoskeletal conditions. Misdiagnosis often leads to unnecessary treatments and resource over-utilization. This review article provides an overview of HADD in seven patients who presented to the ED secondary to an acute presentation of this disease process. HADD is a prevalent pathology, which commonly involves the shoulder, followed by the hip, elbow, wrist, and knee. Predisposing risk factors, such as diabetes and certa

Vesico-acetabular fistula formation is a rare condition typically associated with total hip arthroplasty. Clinical features can include pain of the hip and flank, haematuria, and dysuria. We report the case of a 67-year-old female with a past medical history of bilateral pelvic fractures and calcium hydroxyapatite deposition disease of the hip joint, who developed vesico-acetabular fistula in the absence of surgery. This was then complicated by septic arthritis. We highlight the ultrasound, computed tomography (CT) and magnetic resonance imaging (MRI) findings of the case and review the previo

OBJECTIVE: To evaluate the prevalence of shoulder adhesive capsulitis (AC) signs on MR studies of patients with various common shoulder conditions.
METHODS: MR images of 316 patients were retrospectively evaluated. Patients were divided into three groups: controls (66 patients), clinical AC (63 patients), and study group (187 patients). The final diagnosis of AC was reached clinically. The study group was composed of patients with focal and massive rotator cuff tears, active hydroxyapatite deposition disease, fractures around the shoulder, and post-surgery. The following AC signs were evaluat

Painful os peroneum syndrome (POPS) is one of the uncommon causes of lateral foot pain in orthopaedic practice. POPS encompasses a spectrum of pathologies such as an acute or a chronic fracture, peroneus longus tenosynovitis, and attrition or partial rupture of the peroneus longus tendon. Herein, we reported the first case of POPS secondary to hydroxyapatite deposition disease in a 32-year-old female patient. The clinical presentation, the role of radiological interventions, particularly ultrasound-guided barbotage in its diagnosis and effective management, is highlighted.

Hydroxyapatite deposition disease is a common musculoskeletal pathology that often affects rotator cuff tendons in the shoulder joint. Here, we present the first documented case of acute calcific enthesitis involving the lateral patellar retinaculum, specifically the lateral patellofemoral ligament (LPFL). A 35-year-old male presented with insidious onset atraumatic pain persisting for 3 months on the lateral aspect of the knee, intensifying over the past week. Clinical examination revealed focal tenderness in the anterolateral region of the knee, with no signs of patellar instability. Magneti

Foot pain is one of the most common presenting complaints in orthopaedic clinical practice and can be attributed to a multitude of pathologies in the various osseous structures, ligaments, and tendons of the foot. The spring ligament complex (SLC) between the calcaneum and navicular supports the talus and plays a major role in the static stability of the medial longitudinal arch of the foot. Although calcific ligamentous enthesopathy around the ankle has been described in the literature, we report the first case of its kind affecting the SLC in a 51-year-old male with medial foot pain and no h

OBJECTIVE: To determine the cost-effectiveness of rotator cuff hydroxyapatite deposition disease (HADD) treatments.
METHOD: A 1-year time horizon decision analytic model was created from the US healthcare system perspective for a 52-year-old female with shoulder HADD failing conservative management. The model evaluated the incremental cost-effectiveness ratio (ICER) and net monetary benefit (NMB) of standard strategies, including conservative management, ultrasound-guided barbotage (UGB), high- and low-energy extracorporeal shock wave therapy (ECSW), and surgery. The primary effectiveness out

Hydroxyapatite deposition disease (HADD) represents a multifaceted condition characterized by the accumulation of hydroxyapatite crystals in soft tissues, leading to subsequent inflammation and discomfort. The intricate etiology of HADD is the subject of this comprehensive review, which encompasses an in-depth analysis of the four proposed pathogenic mechanisms and a deliberation on the predisposing factors that instigate the development of this disease. In order to provide a thorough understanding of the disease's progression, this manuscript delineates the stages of HADD-those preceding calc

Hydroxyapatite deposition disease (HADD) is a common localized disorder characterized by deposition of hydroxyapatite in soft tissues. These hydroxyapatite deposits can be found in the periarticular soft tissues such as bursae, joint capsules, tendon sheaths, and ligaments as well as within the tendons themselves, and intra-articular involvement has also been described [1,2]. We present a case of a 50-year-old female with acute symptoms of carpal tunnel syndrome secondary to partially liquified, mass-like, inflammatory calcific peri-arthritis. The case is supplemented with the use of multimoda

Hydroxyapatite deposition disease (HADD) is a mostly uniarticular, self-limiting condition caused by deposition of hydroxyapatite (HA) crystals in tendons or in the peritendinous soft tissues. Commonly, the glenohumeral joint is affected. More rarely, the HA depot can be cause of a carpal tunnel syndrome due to an acute inflammatory reaction and space-occupying soft tissue oedema. We report a case of acute HA depot located at the volar site of the right wrist with affection of the deep flexor tendons and intraosseous migration into the lunate bone in a 50-year-old female. There are two main go

In a collection of 6 young binturongs (Arctictis binturong), 3 presented with anorexia, lethargy, and footpad swelling diagnosed by surgical biopsy as calcinosis circumscripta. Despite supportive care over the next 4 years, affected binturongs had progressive weight loss, hyperphosphatemia, and hyperkalemia and developed large radiodense deposits in tissues adjacent to appendicular joints, thoracolumbar vertebrae, and the sternum. Two binturongs died and necropsies showed severe periarticular mineral deposition with fibrosis and granulomatous inflammation. Additionally, binturongs had minerali

Calcific tendonitis of the gluteus medius and minimus tendons is a rare complication of hydroxyapatite deposition disease, with bilateral involvement even more so. Although patients can be asymptomatic, there is often an acute-on-chronic presentation of pain. We present a case of bilateral calcific tendonitis of the gluteus medius and minimus tendons on a background of previous bilateral rotator cuff calcific tendonitis in a middle-aged woman. This patient's long-standing history of multi-focal involvement required a multidisciplinary approach between orthopedics, rheumatology, and musculoskel
