Gout, calcium pyrophosphate dihydrate (CPPD) deposition disease, and calcium hydroxyapatite deposition disease (HADD) are the three most common crystal-induced arthropathies. Multimodality imaging may help in their diagnosis, and is useful for a precise and comprehensive assessment and grading of the related osteoarticular damage. Plain film radiography, due to its low cost and wide availability, is the first imaging technique to be used in crystal deposition diseases, providing well-known and specific findings for CPPD deposition disease and HADD, while it may undergrade the early osteoarticu

OBJECTIVE: The objective of our study was to characterize patterns of calcifications in synovial sarcoma.
CONCLUSION: The presence of fine, stippled calcifications in a soft-tissue mass should raise suspicion for synovial sarcoma. These calcifications can coalesce to a deposit that mimics hydroxyapatite deposition disease or soft-tissue osteosarcoma. Spicules of calcification were a less common finding in our series; the appearance of spicules in synovial sarcomas overlaps with that of other soft-tissue sarcomas such as soft-tissue osteosarcoma and liposarcoma.

The study design included a multidisciplinary examination of the mineral phase of ovine intervertebral disc calcifications. The objective of the study was to investigate the mineral phase and its mechanisms of formation/association with degeneration in a naturally occurring animal model of disc calcification. The aetiology of dystrophic disc calcification in adult humans is unknown, but occurs as a well-described clinical disorder with hydroxyapatite as the single mineral phase. Comparable but age-related pathology in the sheep could serve as a model for the human disorder. Lumbar intervertebr

Lansoprazole, a known H(+)/K(+)-ATPase inhibitor, is currently used as a therapeutical option for the initial treatment of gastroesophageal reflux disease. Recently, lansoprazole has been found to be an inhibitor of cytosolic PHOSPHO1 (a phosphatase which hydrolyses phosphocholine and phosphoethanolamine), providing a possible therapeutical target to cure pathological mineralization. Since PHOSPHO1 is present inside matrix vesicles, we tested the effect of lansoprazole on matrix vesicles containing several key enzymes for the mineralization process including tissue-nonspecific alkaline phospha

We report a case of acute carpal tunnel syndrome caused by periarticular calcification (hydroxyapatite deposition disease) around the wrist joint in a 64-year-old woman. She had acute severe pain, exacerbated by wrist movements and extension of the fingers. Her full blood count, urea, electrolytes, uric acid, calcium, phosphate, alkaline phosphatase, and thyroid function levels were all within normal ranges, and her serum was negative for rheumatoid factor. Computed tomography revealed lobulated calcification close to the volar capsule. She underwent an emergency surgical decompression of the

This article presents three patients with acute calcific periarthritis (ACP) of the hand and wrist. ACP is an unusual, painful, monoarticular, periarticular inflammatory process associated with juxtaarticular deposits of amorphous calcium hydroxyapatite. ACP is a distinct clinical subset of hydroxyapatite deposition disease. ACP has a high rate of misdiagnosis because of its rare occurrence and its clinical resemblance to other entities. Clinical presentation may simulate infection, and the associated periarticular calcifications may be mistaken for gout, pseudogout, or other entities. One thi

Crystal-induced arthropathies constitute a spectrum of inflammatory arthritides that is induced by cellular reaction to crystal deposition in and around joints. A variety of microcrystals may be deposited and can induce an inflammatory response. The three most common types of crystal-induced arthropathy are gout, calcium pyrophosphate dihydrate deposition disease, and calcium hydroxyapatite deposition disease. Each has a characteristic clinical presentation, crystal type that may be aspirated from affected tissues, and radiographic appearance. Each of these entities may occur as a primary abno

OBJECTIVE: To evaluate the ability of baseline hand radiographs to predict the diagnosis 2 years later in a cohort of patients with early arthritis.
METHODS: A total of 258 patients with arthritis onset within the previous year were evaluated. At baseline, all patients underwent a standardized evaluation including laboratory tests and radiographs. Hand radiographs were read by a blinded observer who used a standardized procedure for detecting features of crystal deposition diseases and rheumatoid arthritis (RA). After 30 +/- 11.3 months, the final diagnosis was established by a panel of rheum

Hydroxyapatite deposition disease (HADD) is a rare cause of carpal tunnel syndrome. This is a case report of a 55-year-old male who presented with symptoms of carpal tunnel syndrome and was subsequently found to have HADD. The diagnosis of HADD is made by the identification of poorly defined cloudy calcifications on radiography of the hand and wrist. Management is usually with nonsteroidal anti-inflammatory drugs, but if the pain is refractory, more invasive methods are utilized.

The authors report the magnetic resonance (MR) appearance of bone marrow edema associated with hydroxyapatite deposition disease without cortical erosion. Hydroxyapatite deposition disease may have bone marrow edema on MR imaging without radiographic evidence of cortical erosion, mimicking the appearance of fracture, neoplasm, or infection. Awareness of this association can prevent unnecessary additional imaging evaluation or biopsy.

Rapid developments in genetic analysis have enabled the dissection of a variety of arthropathies that are inherited in a Mendelian manner. These disorders include calcium crystal arthropathies such as calcium pyrophosphate dihydrate deposition (CPPD) disease and hydroxyapatite deposition disease. In CPPD disease, mutations in a recently discovered gene, ANKH, have been demonstrated in five affected families and may also be associated with the idiopathic deposition of calcium pyrophosphate dihydrate crystals. The product of ANKH appears to be involved in cellular transport of inorganic pyrophos

We describe a patient with acute calcific periarthritis in an unusual site, the ilio-femoral ligament. The clinical findings in this patient resembled those of septic arthritis. T2*-weighted images with gradient field echo clearly showed foci of low signal intensity in this region, corresponding to the calcification shown on plain radiographs, and increased signal intensity around the foci, representing edema. These signal intensities were diagnostic of the disease.

Basic calcium phosphate (BCP) crystals include partially carbonate-substituted hydroxyapatite, octacalcium phosphate, and tricalcium phosphate. They may form deposits, which are frequently asymptomatic but may give rise to a number of clinical syndromes including calcific periarthritis, Milwaukee shoulder syndrome, and osteoarthritis, in and around joints. Recent data suggest that magnesium whitlockite, another form of BCP, may play a pathologic role in arthritis. Data from the past year have provided further understanding of the mechanisms by which BCP crystals induce inflammation and degener

BACKGROUND: To the best of our knowledge, only one patient with calcium hydroxyapatite deposition disease (CHADD) of the longus colli muscle has been reported in the otolaryngology literature.
METHODS: Clinical findings and results of imaging studies in such a patient are reported.
RESULTS: Two particular imaging findings of this disorder are discussed. First, disappearance of calcifications over the short time frame of a week, accompanied by the resolution of clinical symptoms. Second, the presence of an extensive region of high signal intensity extending from the skull base to the lower bo

This 52-year-old woman developed crystal deposition disease involving the cervical vertebrae. She presented with symptomatic spinal cord compression secondary to extensive calcified lesions in the posterior elements of the cervical spine. Surgical decompression with posterior fusion was performed. Histological examination showed hardened deposits of calcium carbonate involving the soft tissue, and dissolution of the vertebral bone trabeculae. There was no inflammatory response to these deposits. One year postoperatively the patient developed severe pulmonary disease associated with the collage
