Detection of COX-1 and COX-2 isoforms in synovial fluid cells from inflammatory joint diseases.

OBJECTIVE: To investigate the expression of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) in cells from synovial fluid (SF) of patients with acute or chronic arthritis. METHODS: SF was obtained from eight patients with acute crystal-induced arthritis, nine with rheumatoid arthritis and four with psoriatic arthritis. COX-1 and COX-2 gene expression was studied by reverse transcriptase-polymerase chain reaction (RT-PCR). Protein expression was detected by Western blotting and immunocytochemistry. RESULTS: There was expression of COX-1 mRNA in all and COX-2 mRNA in most of the SF sample

Dactylitis: implications for clinical practice.

OBJECTIVES: To assess the specificity of dactylitis for the diagnosis of spondyloarthropathy, sarcoidosis, and gout; and to characterize dactylitis specifically associated with gout. METHODS: Dactylitis was prospectively assessed among all individuals presenting to the Arthritis Center of Northeast Ohio from 1986 to 1996. RESULTS: Dactylitis was observed in 12% of individuals with spondyloarthropathy, 17% with sarcoidosis, and 5% with gout, but not in 96 patients with rheumatoid arthritis or in 2,434 patients with osteoarthritis, neck or back pain, or collagen vascular diseases. Among indivi

Coexisting psoriatic arthritis, gout, and chondrocalcinosis.

The authors describe the case of a 63-year-old Caucasian man with a history of psoriatic arthritis who subsequently developed gout and chondrocalcinosis. This is the first report documenting the simultaneous occurrence of psoriatic arthritis, gout, and chondrocalcinosis in a single patient. The relations of these rheumatic diseases are discussed.

Calcium pyrophosphate dihydrate crystal deposition disease after anterior cruciate ligament reconstruction.

We report the case of a 34-year-old woman who presented with calcium pyrophosphate dihydrate (CPPD) crystal deposition disease shortly after anterior cruciate ligament (ACL) reconstruction using a polyester artificial ligament (Leeds-Keio; Neoligaments, Leeds, England). The patient had earlier undergone a medial collateral ligament repair of a sprain to her right knee incurred while skiing. Nine years later, she underwent ACL reconstruction. Seventeen months after ACL reconstruction, calcification was observed on radiographs of the medial and lateral menisci. Based on these calcifications and

Prevalence and patterns of tendon calcification in patients with chondrocalcinosis of the knee: radiologic study of 156 patients.

The presence or absence of tendon calcification was studied at six anatomic sites: Achilles, gastrocnemius, quadriceps, triceps (elbow), triceps long head (shoulder), and rotator cuff. The morphology of the calcifications was categorized in 156 patients with chondrocalcinosis in the knee. Achilles tendon, gastrocnemius, and quadriceps tendon calcifications were most common, ranging from 21%-25% of our patient population was thin linear bands. Triceps calcification at the elbow, rotator cuff calcifications, and long head of triceps tendon calcification were less common.

Chronic tophaceous gout in the elderly.

Gout in the elderly seems to represent a special subgroup. The presentation is often atypical and drug treatment poses special difficulties. We present two elderly patients with large subcutaneous tophi leading to grotesque deformities. Their clinical features, risk factors, diagnosis and treatment are discussed. Although the appearance and radiographic changes caused by tophi are characteristic, the definitive diagnosis of tophaceous gout is made by aspiration and crystal analysis, as is illustrated by colour photographs.

Cutaneous deposition diseases. Part II.

Part II of the cutaneous deposition disorders focuses on cutaneous calcification and ossification, alkaptonuria and ochronosis, and gout. These disorders have in common the deposition of materials in the dermis or subcutis and often involve metabolic defects in hormonal and enzymatic regulation. The pathogenesis, clinical findings, and treatment of these diseases are discussed. Both the histologic and ultrastructural findings are reviewed.

Restoration of the uricosuric effect of probenecid after triglycylvasopressine administration in a gouty patient.

A 35-year-old patient with severe gout and mild renal insufficiency presented very low urinary urate excretion. Volume expansion induced by fludrocortisone combined or not with a uricosuric drug (Benzbromarone) was unable to significantly increase his urate excretion. A combined Probenecid (PB) and Pyrazinamide (PZA) test was performed. These drugs being considered to affect renal tubular reabsorption or secretion. No significant modification of uric acid fractional excretion (FE.uric acid) was observed after PB and PZA. When the same test was performed after the administration of Triglycyl-ly

Chondrocalcinosis is a feature of Gitelman's variant of Bartter's syndrome. A new look at the hypomagnesemia associated with calcium pyrophosphate dihydrate crystal deposition disease.

The occurrence of chondrocalcinosis in patients with Bartter's syndrome has been reported as a typical example of hypomagnesemia-associated calcium pyrophosphate dihydrate crystal (CPPD) deposition disease. However, hypomagnesemia is a feature of Gitelman's variant of Bartter's syndrome, whereas serum magnesium levels are normal in Bartter's syndrome strictly speaking. We managed four patients with chondrocalcinosis and hypomagnesemia who met criteria for Gitelman's disease, including hypomagnesemia, hypokalemia with normal or high urinary potassium excretion, hypocalciuria, and normal blood p

Acute neck pain and fever as the first manifestation of chondrocalcinosis with calcification of the transverse ligament of the atlas. Five case-reports with a literature review.

We report five cases of acute neck pain with fever in patients with diffuse articular chondrocalcinosis and computed tomography evidence of calcification of the transverse ligament of the atlas. A review of the relevant literature illustrated the high frequency of this condition, the variability of clinical presentations ranging from episodes of acute neck pain with fever to compression of the proximal spinal cord, the value of computed tomography for determining the site of the calcific deposits and assessing the lesions, and the possibility of deposition of both calcium pyrophosphate dihydra
