Sex Hormones Regulate T-Cell Responses Associated With Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disorder characterized by sustained synovial inflammation contributing to bone erosion and loss of joint function. The pathological response of infiltrated T cell subsets, followed by the formation of ectopic lymphoid microstructures in the synovial tissue, promotes RA progression and exacerbates disease severity. Two major hallmarks of RA pathogenesis are dysregulated peripheral tolerance and aberrant pro-inflammatory responses due to the secretion of pro-inflammatory cytokines within the RA synovium. Interestingly, regulatory T cells (Tregs)

Shared pathogenic genes and therapeutic targets in periodontitis and systemic juvenile idiopathic arthritis: An integrative bioinformatics and machine learning study.

Periodontitis (PD) and systemic juvenile idiopathic arthritis (sJIA) are chronic inflammatory diseases with potential clinical links, yet their shared molecular mechanisms remain unclear-patients with JIA face elevated periodontal risk, the temporomandibular joint affected in JIA shares inflammatory pathways with PD, Th17 cells drive both conditions, and glucocorticoid therapy may exacerbate periodontal vulnerability. This study aimed to explore the shared mechanisms and potential therapeutic targets of PD and sJIA using genetic expression data from the GEO database and performed comprehensive

Clinical trials and new therapies in Sjögren's disease.

Despite major advances in understanding Sjögren's disease (SjD), no approved disease-modifying therapies have been approved. Management remains largely symptomatic with the use of off-label immunomodulators for systemic features. This gap highlights the substantial unmet need for targeted treatment. Encouragingly, the therapeutic landscape is shifting. Several phase 2 trials have shown meaningful efficacy signals, and for the first time, phase 3 programs with ianalumab and telitacicept have demonstrated positive outcomes, particularly when systemic activity is assessed using the EULAR Sjögren'

Bloodstream Infection in an Immunocompromised Host After Unusual Presentation of Photo Recall Dermatitis.

BACKGROUND Photo recall dermatitis (PRD) is a phototoxic eruption that occurs in areas of previous ultraviolet-damaged skin following systemic administration of a drug, most commonly chemotherapeutic medications. Although generally considered a self-limited dermatologic phenomenon, its implications in immunocompromised patients have not been well characterized. Secondary infection complicating PRD has not been previously established in the literature. This case is unique because the patient had an unusual rash while neutropenic, then developed a bloodstream infection (BSI), with the skin as he

Utility of the Early Sjögren Antibody Panel as a Diagnostic Marker for Sensory Neuropathy.

INTRODUCTION/AIMS: .: Primary Sjögren syndrome (SS) is a chronic autoimmune disease characterized frequently by sensory neuropathy (SN). Carbonic Anhydrase-6 (CA-6), Parotid Secretory Protein (PSP), and Salivary Protein-1 (SP-1) antibodies (Early Sjögren antibodies [ESA]) are found in 45% of SS patients who lack traditional antibodies (Ro/La). But there is a lack of information regarding whether ESA are associated with SN, and if there are any identifying characteristics associated with ESA in SN. Our goal was to close this knowledge gap. METHODS: All SN patients tested for ESA from May 2023-

Targeting of NLRP3 Gln624/Ser658 by Carabrone attenuates inflammatory diseases.

The NOD-like receptor family pyrin domain-containing protein 3 (NLRP3) inflammasome plays a crucial role in host defense; however, its aberrant activation leads to excessive release of pro-inflammatory cytokines, triggering inflammatory responses and tissue damage in human diseases. In this study, the inhibitory effect and anti-inflammatory potential of carabrone on the NLRP3 inflammasome were systematically evaluated. Carabrone suppressed lipopolysaccharide (LPS) + ATP/Nigericin-induced IL-1β secretion, Caspase-1 activation, and apoptosis-associated speck-like protein (ASC) speck formation. M

Pure Motor Axonal Neuropathy Associated with Sjögren's Syndrome: A Rare Case Report.

BACKGROUND: Peripheral nervous system involvement is a recognized extraglandular manifestation of Sjögren's syndrome (SjS), most commonly presenting as sensory or mixed sensorimotor axonal neuropathies. Pure motor axonal neuropathy is exceptionally rare in this context and may closely mimic anterior horn cell disorders, posing significant diagnostic and therapeutic challenges. CASE PRESENTATION: We report the case of a 47-year-old man with long-standing sicca symptoms who developed progressive, asymmetric motor weakness affecting all four limbs without sensory involvement. Neurological examin

Neutrophil Fate and Function in Gout: From Sterile Inflammation to Resolution.

Gout is a prototypical sterile inflammatory disease caused by the deposition of monosodium urate (MSU) crystals in joints and periarticular tissues. Although acute gout flares are characterized by rapid and intense neutrophil-dominated inflammation, they often resolve spontaneously, suggesting that neutrophils may contribute not only to inflammatory amplification but also to endogenous resolution. In this review, we summarize current evidence on the multifaceted roles of neutrophils in gout across different stages of disease. We discuss how MSU crystals activate innate immune signaling and pro

Coexistence of Sjögren's disease and Takayasu arteritis in a woman: a case-based review.

Sjögren's disease (SjD) is a chronic systemic autoimmune disorder primarily affecting the exocrine glands, while Takayasu arteritis (TAK) is a large-vessel vasculitis involving the aorta and its major branches. Although SjD frequently overlaps with other autoimmune diseases, its coexistence with TAK has been reported only rarely. A 47-year-old Turkish woman with seropositive SjD presented with constitutional symptoms, upper extremity claudication, and asymmetric peripheral pulses. Laboratory evaluation revealed elevated inflammatory markers while antiphospholipid antibodies were negative. Comp

Common Skin Diseases and Metabolic Syndrome: A Proinflammatory Chemokine Perspective.

Skin diseases frequently coexist with other disorders, such as metabolic syndrome, diabetes mellitus, depression, psoriatic arthritis, and cardiovascular disease. Altered levels of distinct chemokines, like CCL5/RANTES, CXCL12/SDF-1a, CCL7/MCP-3, CCL2/MCP-1, CXCL1/GROa, and the eotaxin family, contribute to the development and/or exacerbation of inflammation, which is a common feature of numerous skin diseases as well as metabolic syndrome. The pathological and molecular connections between chronic inflammatory skin diseases and metabolic syndrome are increasingly recognized as being driven by

Tissue-Specific Expansion of Age-Associated B Cells via IFN-γ and IL-21 Within Salivary Glands in Sjögren Disease.

Sjögren disease (SjD) is an autoimmune disorder that predominantly affects the exocrine glands, and advancing age is recognized as an important risk factor for its development. However, the mechanisms linking age and disease progression remain poorly understood. Age-associated B cells (ABCs), a subset of B cells that increase with age, have been implicated in autoimmune responses, but their role in SjD pathogenesis has not been fully clarified. In this study, we examined labial salivary glands (LSGs) from 44 SjD patients and 11 non-SjD sicca controls. T-bet+ CD20+ ABCs were detected infiltrati

Real-world Clinical and Diagnostic Features of Patients With Isolated Anti-SSB Antibodies Compared With Those With Combined Anti-SSA and Anti-SSB Antibodies.

BACKGROUND/OBJECTIVE: Anti-SSA and/or anti-SSB antibodies are commonly seen in Sjögren disease but lack disease specificity. Isolated anti-SSB may have a limited association with Sjögren, while dual anti-SSA/SSB positivity may indicate more severe disease. This study compared clinical and serologic features of patients with isolated anti-SSB versus dual anti-SSA/SSB positivity in a real-world setting. METHODS: A retrospective medical records review study from January 1, 2018, to December 31, 2022, was conducted. Adults with positive serologic testing for anti-SSA and anti-SSB antibodies were

Dynamic progression of ectopic lymphoid structure formation in lacrimal glands of a Sjögren's disease murine model.

BACKGROUND: Sjögren's Disease (SjD) is a chronic autoimmune condition characterized by lymphocytic infiltration of lacrimal glands (LG) and salivary glands (SG). In SG, these immune structures have properties of ectopic lymphoid structures (ELS) and appear to play a critical role in disease pathology. While the presence of ELS in patients' SG biopsies is linked to disease severity, their presence and composition in LG has not been well characterized. METHODS: The properties and time course of apparent ELS development in LG from the male non-obese diabetes free (NOR) sub-strain of the Non-Obes
