Single-cell analysis reveals CXCL16+ dendritic cells and clonally expanded GZMK+ CD4 T cells driving joint inflammation in psoriatic arthritis.

OBJECTIVE: Psoriatic arthritis (PsA) is a chronic immune-mediated arthritis affecting approximately 30% of individuals with psoriasis. The mechanisms driving PsA joint inflammation remain incompletely defined. METHODS: To address this gap, we performed multimodal single-cell sequencing of immune cells from matched peripheral blood and synovial fluid (SF) from 6 PsA patients, with comparisons to a rheumatoid arthritis (RA) patient and 6 healthy controls (HC). RESULTS: SF CD4 and CD8 T cells displayed elevated expression of TOX, PDCD1, CTLA4, GZMK, CXCR3, and CXCR6, reflecting a tissue-specifi

Beyond the pelvis: redefining endometriosis as a systemic inflammatory disorder. A systematic review and meta-analysis.

BACKGROUND: Endometriosis has traditionally been defined as a disease localized to the pelvis. However, its clinical presentation is often accompanied by systemic comorbidities that extend far beyond the pelvic cavity, a phenomenon that challenges the traditional disease paradigm. OBJECTIVE AND RATIONALE: This study aimed to comprehensively assess the associations between endometriosis and autoimmune diseases, allergic conditions, chronic pain syndromes, and endocrine disorders through a systematic review and meta-analysis, thereby providing evidence for redefining endometriosis as a systemic

Decoding autoimmune diseases through the adaptive immune receptor repertoires.

The adaptive immune receptor repertoire (AIRR), composed of T-cell receptors (TCRs) and B-cell receptors (BCRs), underpins antigen-specific immunity through extensive diversity generated by V(D)J recombination and junctional modification. Advances in high-throughput AIRR sequencing have enabled comprehensive characterization of clonal diversity, gene usage patterns, and antigen-driven selection at unprecedented resolution. In autoimmune diseases, breakdown of immune tolerance permits the persistence and expansion of autoreactive lymphocyte clones, and is associated with characteristic repertoi

Generative AI in clinical practice: how should rheumatologists evaluate and monitor emerging tools?

Generative artificial intelligence (AI) is increasingly being used in rheumatology practice, to assist with tasks such as documentation, record summarization, and clinical decision support. Rheumatologists need a practical approach to determining what evidence and oversight each application requires. This narrative review proposes a framework based on two dimensions: contextual complexity and influence on patient care. Greater contextual complexity can make errors harder to detect, whereas greater influence on care increases the potential consequences of an undetected error. We examine evidenc

A subunit vaccine protects A129 mice from death and joint swelling against lethal Chikungunya virus challenge.

INTRODUCTION: Chikungunya virus (CHIKV) is primarily transmitted to humans through bites of infected Aedes mosquitoes, causing febrile illness as well as acute or chronic arthritis. With increasing reports in previously non-endemic regions, it poses an emerging threat to public health and a growing risk to travelers. Despite the recent approval of live-attenuated and virus-like particle-based vaccines, a safe, efficacious, and cost-effective vaccine remains urgently needed. The envelope glycoproteins E1 and E2, which play crucial roles in viral attachment and entry, are the major immunodominan

Investigating the association between interferons and flares of systemic lupus erythematosus at a single cell level.

BACKGROUND: Elevated levels of interferons (IFN) are a hallmark of Systemic Lupus Erythematosus (SLE) and are linked to disease flares, but the precise mechanisms underlying this remain unclear. We examined the association between IFN-stimulated proteins (ISPs) and immune changes in flaring and quiescent SLE patients at single-cell resolution. METHODS: Using CyTOF, six ISPs were measured in PBMC populations from 18 healthy controls, 24 quiescent SLE patients, and 46 recently flaring SLE patients. Individual ISPs and a composite ISP score were assessed. RESULTS: Analysis of 29 immune populati

Targeted protein degradation via chimeric antigen receptor (CAR)-mediated antigen endocytosis in T cells.

Targeted protein degradation (TPD) holds significant therapeutic potential over conventional biologics. However, existing platforms are constrained by their reliance on host protein degradation machinery and limited durability, rendering them inadequate for chronic diseases requiring sustained treatment. Here, we constructed a chimeric antigen receptor (CAR) incorporating the TNFR1 ectodomain as the antigen-binding domain (TNFR1T cells) and used CRISPR-mediated knockout of BCOR and ZC3H12A to generate persistent TNFR1TIF cells. TNF binding, endocytosis, and degradation were assessed in vitro b

Chylothorax vs. Pseudochylothorax: Cholesterol Crystal Finding in Pleural Effusion.

Chylothorax and pseudochylothorax are both rare forms of pleural effusion characterized by a milky appearance of the pleural fluid. Despite their similarities, they have distinct etiologies, clinical implications, and medical management strategies. Pleural fluid analysis of cholesterol and triglyceride levels is crucial to differentiate between them, establish a proper diagnosis, and guide clinical evaluation and treatment. This case report describes an uncommon presentation of pseudochylothorax associated with long-standing rheumatoid arthritis (RA) in a patient with no prior history of pleur

Bacterial extracellular vesicles in inflammatory diseases: emerging roles and mechanisms.

Bacterial extracellular vesicles (BEVs), nanoscale membrane-bound particles secreted by bacteria, have emerged as pivotal mediators of host-microbe communication, particularly in the context of inflammatory diseases. By delivering diverse bioactive cargos including proteins, lipids, nucleic acids and metabolites, BEVs orchestrate complex immunological responses, playing dual roles in both the exacerbation and resolution of inflammation. This review presents a comprehensive overview of the biogenesis, classification and molecular composition of BEVs, with emphasis on their immunomodulatory func
