Joint pain: a practical guide to the diagnosis of arthritis.

Joint pain is one of the most common reasons for consultation in clinical practice, and its correct interpretation presents a diagnostic challenge due to the wide variety of causes and clinical presentations. The assessment of joint pain requires a structured clinical approach that allows differentiation between joint and extra-articular causes and distinguishes between inflammatory and non-inflammatory processes. A detailed medical history and targeted physical examination form the cornerstone of the diagnostic process, facilitating the identification of clinical patterns based on the number

Acute septic arthritis of the ankle in an immunocompromised patient with multiple myeloma: a case report.

INTRODUCTION: Septic arthritis is a rapidly progressive joint infection that can result in irreversible cartilage destruction, systemic sepsis, and increased mortality if not diagnosed and treated promptly. Immunocompromised patients, including those with hematological malignancies, are at significantly higher risk. CASE PRESENTATION: We present the case of immunosuppressed acute septic arthritis of the ankle in a patient with multiple myeloma. The diagnosis was made from the analysis of the aspirated joint fluid that contained pus along with Gram-positive cocci. The X-rays excluded the possi

Dysregulation of caspase-8 and caspase-9 in T-lymphocyte apoptosis: implications for pathogenesis, diagnosis, and therapeutic targeting in psoriatic arthritis.

BACKGROUND: Apoptotic changes in keratinocytes and alterations in T-lymphocytes were shown to have a role in the progression of psoriatic arthritis (PsA). OBJECTIVES: To determine the possible role of the apoptotic changes in peripheral T-lymphocytes in association with the activity of both caspase-8 and caspase-9, and the expression of B-cell lymphoma 2 (Bcl-2) and Annexin V in the pathogenesis of PsA. In addition, the relationships between these apoptotic-related markers, the clinical severity of PsA, and the expression of inflammatory cytokines tumor necrosis factor (TNF-α) and interleukin-

Case Report: Austrian syndrome with multifocal septic arthritis in the immunocompetent elderly: expanding the clinical spectrum.

INTRODUCTION: Austrian syndrome, defined by the triad of pneumococcal pneumonia, meningitis, and infective endocarditis (IE), represents a rare but severe form of invasive pneumococcal disease (IPD). Despite advances in vaccination and antimicrobial therapy, it remains associated with high morbidity and mortality due to diagnostic delays. We report a diagnostically challenging case of Austrian syndrome in an immunocompetent elderly male, complicated by multifocal septic arthritis and delayed recognition of IE. CASE DESCRIPTION: A 71-year-old man with no history of pneumococcal vaccination, sm

When one joint matters: the monoarticular blind spot in inflammatory arthritis.

Monoarthritis is a familiar-yet systematically under-theorised-scenario in rheumatoid arthritis and psoriatic arthritis, occurring both at onset and as residual synovitis that persists despite apparently successful treat-to-target care. Contemporary composite indexes, trial eligibility criteria, and guideline algorithms were built for polyarticular disease; as a result, these tools can numerically dilute the functional and prognostic weight of a single inflamed knee, ankle, or dominant wrist. This situation results in an uncomfortable paradox: overall disease activity can meet the predefined t

Surgical Management of Gonococcal Septic Arthritis: A Case Series and Literature Review.

INTRODUCTION: Purulent gonococcal septic arthritis (GSA) is a rare but clinically significant manifestation of disseminated gonococcal infection (DGI) with Neisseria gonorrhoeae (NG). Presenting distinctly from the arthritis-dermatitis syndrome associated with DGI, GSA poses a unique diagnostic challenge as it presents as monoarthritis or oligoarthritis without systemic features. Current guidelines do not distinguish between these presentations of gonococcal arthritis, and there are no existing recommendations on the surgical management of purulent GSA. CASE REPORT: We present a single-instit

[Gonococcal arthritis: neither so common nor so straightforward. Clinical description of cases diagnosed in northern Spain between 2013 and 2023].

INTRODUCTION: Given the current increase in the incidence of Neisseria gonorrhoeae infection, a rise in disseminated forms of the disease may also be expected. Within disseminated gonococcal disease, gonococcal arthritis has specific clinical, diagnostic and therapeutic characteristics. METHODS: A retrospective search was conducted for synovial fluid samples that tested positive for N. gonorrhoeae by culture or molecular biology techniques between 2013 and 2023 at several hospitals in northern Spain. A descriptive analysis was subsequently performed. RESULTS: Fifteen patients were analysed.

Bilateral knee lipoma arborescens mimicking persistent inflammatory arthritis in a patient with psoriatic arthritis.

Lipoma arborescens is a rare intra-articular lesion typically associated with chronic monoarthritis. We report on a man in his 40s with psoriatic arthritis, well controlled with secukinumab, who developed chronic bilateral knee effusion and soft, non-tender suprapatellar masses over several years despite low systemic disease activity. Magnetic resonance (MR) imaging revealed frond-like fatty proliferation with effusion, which was confirmed as lipoma arborescens on histopathology. Bilateral synovectomy restored full knee extension and improved mobility and function. MR imaging is an excellent n

Profiling immune check point inhibitors - Induced arthritis: A systematic review and pooled analyses of cohort studies.

BACKGROUND: The growing use of immune checkpoint inhibitors (ICIs) in the oncology field along with the potential development of the so-called immune-related adverse events (irAEs) has introduced new diagnostic and therapeutic challenges for rheumatologists. Rheumatic irAEs (Rh-irAEs), particularly those involving the musculoskeletal system, are relatively common and sometimes underdiagnosed, with an estimated prevalence of up to 7% of ICIs treated patients. In this context, we conducted a systematic review and pooled analyses of cohort studies to better characterize ICIs-induced-arthritis (IC

TAPE in inflammatory rheumatic monoarthritis: a proof-of-concept study.

OBJECTIVES: Transarterial periarticular embolization (TAPE) is a minimally invasive endovascular technique established for the treatment of knee osteoarthritis (OA). Its role in inflammatory rheumatic monoarthritis has not yet been investigated. This proof-of-concept study evaluated the feasibility, safety and clinical effects of TAPE in inflammatory rheumatic monoarthritis and compared outcomes with those in knee OA. METHODS: Seventeen joints in 14 patients (7 female, 7 male) were treated, including 11 patients with knee OA and three patients (6 joints) with inflammatory rheumatic joint dise

IgA Vasculitis Presenting As Isolated Monoarthritis Mimicking Transient Synovitis: A Diagnostic Challenge With Delayed Purpura.

IgA vasculitis (IgAV), previously known as Henoch-Schönlein purpura, is a small-vessel vasculitis that may involve the skin, joints, gastrointestinal tract, and kidneys. While purpura is a key clinical feature, presentations without rash at onset can occur and may lead to diagnostic uncertainty. We report the case of a two-year-old boy who presented with a three-day history of acute right knee pain, swelling, and reduced weight-bearing ability following a recent febrile respiratory illness. Ultrasound of the knee demonstrated mild periarticular fluid, and a provisional diagnosis of transient s

Cutaneous clues to a fungal culprit: disseminated Blastomycosis presenting as inflammatory monoarthritis - a Case Report.

Blastomycosis is a dimorphic fungal infection caused by Blastomyces dermatitidis and related species, classically presenting with pulmonary and cutaneous involvement. Musculoskeletal manifestations, particularly monoarthritis, are rare and diagnostically challenging. We present the case of a previously healthy 41-year-old man who developed nodular skin lesions, cough, pulmonary nodules and persistent left knee arthritis, initially thought to be due to pseudogout. The diagnosis of disseminated fungal infection was suggested by CT scan, skin biopsy, and confirmatory fungal culture that grew Blas

Isolated knee septic arthritis revealing disseminated coccidioidomycosis without apparent pulmonary involvement: A case report.

Coccidioidomycosis is an endemic fungal infection that rarely presents as isolated osteoarticular disease in immunocompetent hosts. We report a case of disseminated coccidioidomycosis manifesting as chronic septic arthritis of the tibiofemoral joint without apparent pulmonary involvement in a 32-year-old male. Diagnostic delay and initial immunosuppressive therapy contributed to disease progression. Histopathology confirmed the diagnosis and combined surgical debridement and prolonged antifungal therapy led to favorable outcomes. This case highlights the importance of considering fungal etiolo

Clinician Suspicion for Lyme Disease and Clinical Decision-Making in Children with Monoarthritis.

In this large multi-center cohort of children evaluated for Lyme disease in a Lyme-endemic emergency department, we assessed the diagnostic accuracy of clinician suspicion and subsequent clinical decision-making for children presenting with monoarthritis. Among 1,582 children with monoarthritis evaluated for Lyme disease, 623 (39%) had Lyme arthritis and 32 (2%) had septic arthritis. Overall, 313 (20%) had an invasive joint procedure (arthrocentesis or arthroscopy), 194 (12%) received parenteral antibiotics, and 376 (24%) were hospitalized. Clinician suspicion had moderate discriminative abili

Characteristics and Clinical Outcomes of Lyme Arthritis: A Retrospective Study.

BACKGROUND: Lyme arthritis is the most common late manifestation of disseminated Lyme disease in the United States. While most patients respond to antibiotic therapy, a subset develops postantibiotic Lyme arthritis (PALA), characterized by persistent noninfectious inflammatory arthritis. Data describing predictors of PALA and treatment patterns remain limited. METHODS: We conducted a retrospective observational cohort study of patients with polymerase chain reaction (PCR)-confirmed native joint Lyme arthritis treated at our institution from 2004 to 2025. Clinical, laboratory, imaging, treatme

Clinical Features and Outcomes of Pediatric Lyme Arthritis in Europe: A Large Swiss Cohort Study.

BACKGROUND: Lyme arthritis (LA) in children typically manifests as monoarthritis of a large joint. Although the prognosis is generally good, persistent arthritis occurs in 10%-23% of patients despite antibiotic treatment. Most described pediatric LA cohorts are from the United States, with limited European data available. This study aimed to describe clinical features and outcomes of pediatric LA in Europe using a large Swiss cohort. METHODS: Children diagnosed with LA at the University Children's Hospital Zurich, Switzerland between 1 January 2006 and 31 December 2020 were included. Data on

Tabetic Arthropathy as an Uncommon Cause of Destructive Joint Disease: A Case Report.

Tabetic arthropathy is a destructive neurogenic joint disease that has become rare due to the early treatment of syphilis with penicillin G. It results from neurosyphilis caused by Treponema pallidum. The diagnosis is suspected in patients presenting with severe joint destruction disproportionate to the absence or minimal intensity of pain and is confirmed by positive syphilitic serological tests. We report the case of a 62-year-old patient with a history of high-risk sexual behavior and a remote episode of cervical lymphadenopathy treated 40 years ago. He presented with chronic painless monoa

[Borrelia as a differential diagnosis to monoarthritis].

In this case report, a young woman presented with recurring knee monoarthritis. Arthroscopy revealed synovitis, and histology showed chronic plasma cell-dominated inflammation. A positive serum-borrelia IgG titre supported the suspicion of Lyme arthritis despite a negative specific Borrelia PCR result on a paraffin-embedded synovial biopsy. Treatment with phenoxymethylpenicillin, initiated more than two years after symptom onset, resulted in rapid and near-complete remission. The case underscores the diagnostic challenges of Lyme arthritis and the importance of considering the diagnosis in mon
