Evaluation of rheumatic causes underlying childhood-onset arthritis.

BACKGROUND/AIM: Childhood-onset arthritis may result from various rheumatic diseases. This study aimed to systematically evaluate their distribution and characteristics in a large pediatric cohort. MATERIALS AND METHODS: We retrospectively analyzed all pediatric patients who presented with arthritis to the pediatric rheumatology clinic between January 2000 and January 2023. RESULTS: A total of 1713 pediatric patients who presented with arthritis were included in this study (median age at diagnosis: 6.8 years; F/M = 1.2). Most of the patients (n = 859, 50.1%) had juvenile idiopathic arthritis

Clinical and Imaging Features of Chronic Occult Infectious Arthritis and Undifferentiated Oligoarthritis: A Comparative Analysis.

Background: Undifferentiated arthritis is characterized by synovitis that does not meet the criteria for any specific rheumatic disease. However, a subset of chronic occult infectious arthritis, owing to atypical or overlapping clinical features, is often misclassified as undifferentiated oligoarthritis, potentially leading to diagnostic delays and suboptimal management. This study aimed to compare the clinical, laboratory, and imaging characteristics of these two types of oligoarthritis and to evaluate potential discriminatory markers. Methods: Patients older than 16 years with synovitis invo

Diagnosing Oligoarticular Juvenile Idiopathic Arthritis in a Two-Year-Old: A Case Report.

Juvenile idiopathic arthritis (JIA) is a rare heterogeneous group of autoimmune disorders presenting in children and is characterized by persistent joint inflammation without a known cause. Here, we present the clinical course of a two-year-old girl who developed acute right knee swelling without preceding trauma. Laboratory findings, joint aspiration, imaging, and clinical diagnosis initially suggested reactive arthritis; however, the persistence of symptoms and the emergence of a second affected joint led to the diagnosis of oligoarticular JIA. Laboratory analysis revealed a markedly elevate

Specific dietary micronutrients are causally associated with arthritis subtypes - Results from the NHANES 2015-2018 and Mendelian randomization study.

Background: micronutrients (MNs) in the diet are linked to arthritis, but the causal relationships and dose-dependent effects remain unclear. Methods: using the National Health and Nutrition Examination Survey (NHANES) and genome-wide association studies (GWAS) databases, multivariate logistic regression, restricted cubic splines (RCS), and Mendelian randomization (MR) were applied to analyze MNs' impact on arthritis. Results: multivariate logistic regression identified six MNs significantly associated with arthritis: calcium (OR = 0.74 [95 % CI, 0.60, 0.92] p = 0.013), magnesium (OR = 0.74 [9

Prevalence and characteristics of concomitant septic and crystal-induced arthritis: A hospital database and literature review.

BACKGROUND AND OBJECTIVES: Concomitant septic and crystal-induced arthritis is a rare condition. Failure to diagnose this condition can result in significant harm to the patient. This study aims to investigate the prevalence and characteristics of concomitant septic and crystal-induced arthritis. METHODS: A retrospective study included patients diagnosed with concomitant septic and crystal-induced arthritis confirmed by positive bacterial culture and intracellular crystals in synovial fluid of the same joint, from January 1, 2015, to July 31 ,2024. RESULTS: A total of 45 cases were defined a

Predictive value of preceding fever or rigor for septic arthritis in acute monoarthritis: A retrospective observational study.

BACKGROUND: Previous studies of septic arthritis (SA) have assessed the predictive value of fever without regard to its relationship with the onset of systemic and articular symptoms. The present study aimed to assess the diagnostic value to fever and rigor with respect to the onset of articular symptoms. METHODS: The present study retrospectively reviewed 234 adult patients with acute monoarthritis who underwent synovial fluid analysis at the emergency department of a tertiary hospital. SA was defined by modified Newman's criteria. Diagnostic accuracy was compared between the index history,

Delayed Diagnosis of Tuberculous Coxitis in a Child Initially Misdiagnosed as Septic Arthritis: A Case Report.

Tuberculous coxitis is a rare form of extrapulmonary tuberculosis in children, often misdiagnosed due to its subtle presentation and resemblance to other joint disorders. We report the case of a 12-year-old boy with a seven-month history of progressive right hip pain, joint deformity, and a draining sinus, initially mismanaged as a nonspecific infection. Imaging revealed destructive changes in the right hip, and molecular testing confirmed Mycobacterium tuberculosis from both gastric aspirate and intra-articular pus. The patient underwent anti-tuberculosis therapy and surgical debridement with

Refining the adjuvant-induced rat model of monoarthritis by optimizing the induction volume and injection site.

Arthritis is a highly prevalent and disabling condition characterized by pathological joint-damage, clinical symptoms of pain and loss of normal joint function. Monoarthritis can be modelled in rodents via intraarticular injection of Complete Freund's Adjuvant (CFA), inducing both joint inflammation and pain-like behaviours. This study aimed to compare the outcome of different injection-volumes and joint-locations, to refine the model's use and to improve its validity. Male and female Sprague Dawley rats were injected with CFA into the ankle (10, 20 or 50 µl) or knee joint (10, 50 or 100 µl),

Canakinumab treatment in patients with colchicine-resistant familial mediterranean fever: a multicenter observational study.

BACKGROUND/AIM: Anti-interleukin-1 agents have known beneficial effects in the treatment of colchicine-resistant familial Mediterranean fever (cr-FMF); however, studies to date have tended to have small sample sizes and to be based on pooled data. The present study investigates the efficacy of Canakinumab (CAN) in a homogeneous cohort of cr-FMF cases. MATERIALS AND METHODS: The study included patients who underwent treatment in three tertiary rheumatology departments, whose electronic medical records were reviewed retrospectively. The inclusion criteria were presence of colchicine resistant d

Musculoskeletal manifestations in children with inflammatory bowel disease: a multicenter cohort study (GASTROREUM study).

BACKGROUND: Musculoskeletal (MSK) symptoms are the most common extra-articular manifestations of pediatric-onset inflammatory bowel diseases (pIBD), and are associated with a more aggressive disease course. This study aims to characterize MSK manifestations in patients with pIBD, and to seek for predictors of persistently active arthritis one year after pIBD diagnosis. METHODS: A multicenter, retrospective cohort study was conducted at 25 Italian pediatric rheumatology centers. Patients aged < 18 years with pIBD and MSK manifestations, followed for at least one year, were included. Data at ons

When Fever Strikes Twice: A Case Report of Streptococcus pneumoniae Myelitis with Delayed-Onset Reactive Arthritis.

Background:Streptococcus pneumoniae is a well-known pathogen responsible for respiratory and invasive diseases; however, central nervous system (CNS) involvement in the form of bacterial myelitis is exceedingly rare, particularly in immunocompetent adults. Moreover, the association between pneumococcal infections and reactive arthritis is scarcely documented. We report an unusual case of pneumococcal myelitis complicated by reactive arthritis in an elderly patient with no evident immunosuppression. Case Presentation: A 68-year-old man with a medical history of hypertension, benign prostatic hy

Acute Monoarthritis: Diagnosis in Adults.

Acute monoarthritis, characterized by pain or swelling in a single joint, is a diagnostic challenge in the primary care setting. Intra-articular conditions typically manifest with reduced active and passive range of motion, whereas patients with periarticular conditions such as tendinitis or bursitis often maintain passive range of motion. When evaluating a patient with acute monoarthritis, it is essential to remember that many polyarthritic processes can initially present in a single joint. A broad differential diagnosis for monoarthritis should include septic arthritis, osteoarthritis, gout,

Synovial Lipomatosis of the Knee: A Case Report and Review of the Literature.

Synovial lipomatosis (lipoma arborescens) is a rare, benign, intra-articular lesion characterized by villous proliferation of mature adipocytes, most commonly affecting the knee. Early recognition is essential to prevent misdiagnosis and long-term joint dysfunction. MRI is highly suggestive, while histopathology confirms the diagnosis. We describe the case of a 34-year-old male with persistent left knee monoarthritis. Laboratory investigations, including complete blood count, erythrocyte sedimentation rate, and C-reactive protein, were within normal limits. MRI showed frond-like synovial proje

Diagnostic accuracy and trajectories of referrals for gout to rheumatology.

OBJECTIVES: To evaluate diagnostic accuracy and trajectories of gout referrals to rheumatology including factors associated with an accurate diagnosis. METHODS: We performed a retrospective cohort study of referrals at 4 rheumatology clinics in Brampton, Canada from December 2019 to January 2023. We assessed gout diagnostic accuracy referenced to the rheumatologist's "gold standard" diagnosis, describing alternative final diagnoses. Using multivariable logistic regression, we identified factors associated with an accurate gout diagnosis. RESULTS: Among 4315 patients, 216 were diagnosed with

Gout on the acute medical take.

Gout is the commonest form of inflammatory arthritis. Flares are the commonest presentation of gout. Typically, gout flares present as acute monoarthritis, and most often affect the first metatarsophalangeal joint ('podagra'). Other joints in the lower limbs are affected more often than those in the upper limbs. Joint aspiration followed by examination of the synovial fluid using a polarised light microscope is the gold standard for a definite diagnosis of gout. Gout may be diagnosed without recourse to joint aspiration if there is podagra, elevated serum urate and no suspicion of infection. U

Non-radiographic axial spondyloarthritis developing after Lyme arthritis: a case report and review.

BACKGROUND: Lyme disease is a tick-borne illness that can result in a spectrum of musculoskeletal complications, ranging from infection-related arthritis to post-infectious, immune-mediated conditions. Although prior reports have linked Lyme disease to systemic autoimmune diseases, such as rheumatoid arthritis and psoriatic arthritis, non-radiographic axial spondyloarthritis (nr-axSpA) has not previously been described following Lyme disease. CASE PRESENTATION: A previously healthy patient developed monoarthritis of the ankle, with inflammatory synovial fluid and a positive Lyme Western blot,
