Tuberculous monoarthritis in a rituximab-treated patient with rheumatoid arthritis: diagnostic and therapeutic challenges in a tuberculosis-endemic region.

A woman in her 60s with seropositive rheumatoid arthritis developed tuberculous arthritis of the wrist following rituximab therapy, despite negative latent tuberculosis (TB) screening. Although rituximab is considered safer than anti-TNF agents in terms of TB risk, this case highlights a rare yet serious complication. Diagnosis was confirmed via synovial fluid analysis after persistent monarthritis unresponsive to standard RA therapy. Management was complicated by drug-induced liver injury and ototoxicity, leading to multiple modifications of anti-tubercular therapy. The therapeutic dilemma ce

Latent Reactivation of Osteoarticular Tuberculosis of the Knee.

Osteoarticular tuberculosis (TB) is an uncommon manifestation of extrapulmonary TB (EPTB), accounting for approximately 10-15% of such cases. Amongst these, knee involvement is particularly rare, representing only 0.1-0.3% of all TB infections. Due to its rarity and indolent presentation, tuberculous arthritis of the knee can be misdiagnosed as pyogenic septic arthritis or degenerative joint disease. We report the case of a 43-year-old Indian woman, who has been a resident in the United Kingdom for eight years, who presented with a two-week history of progressive swelling of the left knee that

Acute Pyogenic Ankle Monoarthritis Due to Tuberculosis: A Real-Life Clinical Conundrum.

Musculoskeletal tuberculosis (TB) usually involves the spine, hip, or knee. Ankle disease is rare and can present insidiously, which contributes to delayed diagnosis and poor outcomes. We report a case of an 86-year-old Chinese woman who presented with acute pain and swelling of her left ankle for two days, fevers and inability to bear weight. Past medical history included ischemic heart disease, congestive cardiac failure and recurrent urinary tract infections. Examination revealed a fever of 39.5°C and a tender, swollen ankle with restricted movement. Laboratory tests showed anaemia, lymphop

Ultrasound-Guided Synovial Biopsy in Undifferentiated Monoarthritis: A Case Series From Bangladesh.

Ultrasound-guided synovial biopsy (USG-SB) is a minimally invasive technique useful in diagnosing joint tuberculosis, particularly when conventional methods fail. However, its role in diagnosing tubercular arthritis in Bangladesh remains underexplored. This study presents a case series of five patients with monoarthritis - three with ankle involvement and two with knee joint tuberculosis - evaluated using USG-SB or aspiration. The procedures were performed using a high-resolution musculoskeletal ultrasound (14-6 MHz probe, Mindray, Shenzhen, China). Synovial tissue samples were analyzed via hi

The Need for Persistence in the Diagnosis of Mycobacterium Tuberculosis Mono-arthritis: A Unique Case Presentation.

Although tuberculosis (TB) is considered to be a common disease confined to undeveloped or developing countries, it may also appear in countries that accept great migrant influx from endemic areas. In terms of the skeletal system, it can be involved in 1-6% of tuberculosis patients while the knee joint is the third most frequently affected site after spine and hip. Given that systemic symptoms are present in only one-third of patients with skeletal tuberculosis and secondary septic arthritis, TB is often indolent and diagnosis can hence be missed or delayed. As a result, a high index of suspic

Successful treatment of a rheumatoid arthritis patient with severe synovial hypertrophy and impaired shoulder function with microwave ablation technique.

Rheumatoid arthritis (RA) is a chronic, inflammatory systemic disorder of synovial joints and results in polyarthritis, chronical degeneration, and finally deformities and ankylosis in severe cases. Synovitis and pannus formation are results of inflammatory changes and lead into restriction in joint movement. Shoulders are among the later affected and larger joints and formation of synovitis in early active stages and pannus in later stages might be concluded with frozen shoulder and severe impairment in functionality. These late-term changes cannot be controlled with systemic or local anti-in

Covid-19-related right hip monoarthritis: A case report and literature review.

This article presents the case of a young patient with severe acute left hip pain who tested positive for COVID-19. The diagnostic approach and management are presented, followed by a review of the literature. This 11-year-old student was admitted for acute left hip pain of abrupt onset, inflammatory, evolving for 24 h in a febrile context with no change in general condition. The initial workup revealed a hyperleukocytosis in the blood count, CRP 198.52 mg/L, and a 95 mm SV. The pelvic X-ray was unremarkable, and the hip ultrasound showed synovial hypertrophy, with no effusion on Doppler exami

Electroacupuncture Alleviates Pain by Suppressing P2Y12R-Dependent Microglial Activation in Monoarthritic Rats.

Electroacupuncture (EA) effectively improves arthritis-induced hyperalgesia and allodynia by repressing spinal microglial activation, which plays a crucial role in pain hypersensitivity following tissue inflammation. However, the mechanism by which EA suppresses spinal microglial activation in monoarthritis (MA) remains unclear. In the present study, a rat model of MA was established through unilateral ankle intra-articular injection of complete Freund's adjuvant (CFA). The relationship among P2Y12 receptor (P2Y12R) expression, spinal microglial activation, and EA analgesia was investigated us

μ-Opioid Receptor Activation at the Dorsal Reticular Nucleus Shifts Diffuse Noxious Inhibitory Controls to Hyperalgesia in Chronic Joint Pain in Male Rats.

BACKGROUND: The dorsal reticular nucleus is a pain facilitatory area involved in diffuse noxious inhibitory control (DNIC) through opioidergic mechanisms that are poorly understood. The hypothesis was that signaling of μ-opioid receptors is altered in this area with prolonged chronic inflammatory pain and that this accounts for the loss of DNICs occurring in this condition. METHODS: Monoarthritis was induced in male Wistar rats (n = 5 to 9/group) by tibiotarsal injection of complete Freund's adjuvant. The immunolabeling of µ-opioid receptors and the phosphorylated forms of µ-opioid receptors

Fatal Myocarditis following COVID-19 mRNA Immunization: A Case Report and Differential Diagnosis Review.

Carditis in childhood is a rare disease with several etiologies. We report a case of infant death due to pericarditis and myocarditis after the mRNA vaccine against COVID-19 (COVIDmRNAV). A 7-year-old male child received the first dose of the COVIDmRNAV and presented with monoarthritis and a fever non-responsive to oral antibiotics. The laboratory investigation showed signs of infection (leukocytosis, high levels of c-reactive protein). His condition rapidly deteriorated, and the patient died. The autopsy identified pericardial fibrin deposits, hemorrhagic areas in the myocardium, and normal v

Lipoma arborescens with patellofemoral osteoarthritis and genu valgum.

Lipoma arborescens is a rare benign intra articular lesion characterized by lipomatous proliferation of the synovium. The condition typically affects the knee presenting with atraumatic pain and swelling. It should be considered in the differential diagnosis of monoarthritis and joint effusion. Diagnosis is made on MRI with characteristic features. We present a rare case of lipoma arborescens with patellofemoral osteoarthritis and genu valgum. We discuss the clinical presentation, radiological findings, and management.

Brucellosis an Unusual presentation as isolated septic mono-arthritis of the knee joint: A case report.

Brucellosis is a zoonotic infection that is widely spread across the world. It is becoming more common in Middle Eastern countries such as Qatar, Saudi Arabia, and the Mediterranean region. Despite this, we need to remain vigilant as it is still prevalent in many parts of the world. The most common presentation is musculoskeletal, but it can also present as septic arthritis in the sacroiliac, hip, or knee joints. Brucella melitensis was only found in one extended culture of synovial fluid. Treatment involved a combination of antimicrobial therapy using gentamycin, doxycycline, and rifampin. A

Complex Interplay: A Gigantic Recurrent Popliteal Cyst Following Total Knee Arthroplasty in a Patient with Refractory Rheumatoid Arthritis Case Report.

INTRODUCTION: Juvenile idiopathic arthritis ( JIA) is a persistent autoimmune-inflammatory disease that affects children younger than 16. Aggressive synovitis of the hip may cause joint destruction, hip protrusion, erosion, pseudosubluxation, dysplasia, and osteoarthritis. Subluxation of the hip had been reported previously. However, dislocation of the hip in JIA is an extremely rare situation, and only two cases have been reported up to date. Reduction of the dislocated hip has to be performed in any way. However, there is no algorithm to be followed for the treatment of hip dislocations caus

Repeated social defeat stress differently affects arthritis-associated hypersensitivity in male and female mice.

Chronic stress enhances the risk of neuropsychiatric disorders and contributes to the aggravation and chronicity of pain. The development of stress-associated diseases, including pain, is affected by individual vulnerability or resilience to stress, although the mechanisms remain elusive. We used the repeated social defeat stress model promoting susceptible and resilient phenotypes in male and female mice and induced knee mono-arthritis to investigate the impact of stress vulnerability on pain and immune system regulation. We analyzed different pain-related behaviors, measured blood cytokine a

Monoarthritis as the initial presentation of acute rheumatic fever in Iran: A single-center retrospective cross-sectional study.

BACKGROUND: In Iran, there is a lack of information and studies on acute rheumatic fever (ARF), a global health issue. The limited understanding of ARF's prevalence and primary clinical symptoms has led to confusion. This research investigates the characteristics of children aged 3-17 years who experience ARF with monoarthritis as their initial symptom. METHODS: A retrospective evaluation of medical records of children diagnosed with ARF was conducted. The study aimed to determine the prevalence of monoarthritis as the first manifestation of ARF and its association with age, gender, family hi
