Acid-responsive nanoparticles with microneedles for anti‑inflammatory sonodynamic therapy of gouty arthritis.

The pathological microenvironment of acute gouty arthritis (AGA) features local tissue acidification and the release of a storm of pro-inflammatory cytokines. To address this dual challenge, we engineered an acid-responsive curcumin-loaded nanoparticles microneedle system (Cur-PNPs@MN) that delivers curcumin with pH-triggered release and anti-inflammatory activity. This integrated platform combines a hyaluronic acid (HA)-based dissolving microneedle for localized transdermal delivery of acid-degradable PCADK (poly (cyclohexane-1,4-diyl acetone dimethylene ketal)) curcumin nanoparticles (Cur-PN

Reclassification of the type B CAT family and identification of the novel chloramphenicol acetyltransferase CatB14 in Riemerella anatipestifer.

Chloramphenicol resistance in Riemerella anatipestifer is primarily mediated by chloramphenicol acetyltransferases (CATs). Inconsistent nomenclature of the CatB superfamily in public databases currently complicates the genomic surveillance of these resistance determinants. In this study, we tested the subtype scheme recorded in the NCBI Reference Gene Catalog against a complete pairwise amino acid identity matrix, applying a ≥ 80% identity criterion. The analysis supports the existing type B-1 to type B-5 subtypes, assigns two previously unassigned sequences to Type B-2, and recommends retirem

Single Prone Position, Dual Surgeon Simultaneous Navigated Prone Lateral Interbody Placement with Endoscopic Laminectomy for Symptomatic Immobile Spondylolisthesis.

Symptomatic immobile spondylolisthesis with severe circumferential stenosis consisting of foraminal, lateral recess, and central canal narrowing secondary to disc height loss and facet arthropathy typically may require direct decompression followed by fixation given the need for complete facetectomies to adequately release exiting nerve roots. Recent evidence suggests that standalone grafts may offer a favorable risk profile for indirect decompression in cases with appropriate spinal alignment and normal bone density. A 54-year-old female presented with a one-year history of neurogenic claudic

Gait and Plantar Pressure Analysis in Charcot Neuroarthropathy: A Clinic-Based Biomechanical Study.

INTRODUCTION: Charcot Neuroarthropathy (CN) is a progressive joint disorder resulting from peripheral neuropathy and repetitive microtrauma. Management prioritizes offloading to prevent deformity, ulceration, infection, and amputation. While three-dimensional gait analysis can be useful for disease staging and guiding management, its cost and limited accessibility limit routine clinical use. This study evaluated plantar pressure and two-dimensional gait kinematics in CN patients using a relatively low-cost clinic-based approach. METHODS: Following institutional review board approval, this cro

Epidemiology, risk factors and clinical outcome of community-acquired pneumonia in severely immunosuppressed patients: a comprehensive analysis from the multinational, prospective CAPNETZ Cohort.

PURPOSE: Despite growing evidence that immunosuppression (IS) influences aetiology and prognosis of community-acquired pneumonia (CAP), there remains a lack of data from representative multicentre studies. METHODS: The multinational prospective CAPNETZ study includes patients with CAP and HIV since 2017, and with other causes of IS since 2020. We present the first comprehensive analysis of patients with CAP and IS (HIV with low CD4 cells, systemic steroids, immunosuppressive drugs, antineoplastic treatment, neutropenia, primary immunodeficiency, transplantation, haematological neoplasia) incl

Multifocal ground-glass opacities in lung cancer: a narrative review of genomic evolution, immune ecology, and biology-informed clinical management.

BACKGROUND AND OBJECTIVE: Ground-glass opacity (GGO) is a computed tomography descriptor rather than a histologic diagnosis. Multifocal GGOs (mGGOs) are increasingly detected during lung cancer screening, yet their clonal origins, immune ecology, progression risk, and optimal management remain difficult to reconcile within conventional lung cancer frameworks. This narrative review evaluates genomic, immunologic, and clinical evidence relevant to mGGOs and examines how biological findings may inform surveillance and local treatment without encouraging overtreatment. METHODS: PubMed/MEDLINE, We

Evaluation of DeepSeek with a RAG-based knowledge base in preoperative assessment for arthroplasty to improve the decision-making accuracy of junior physicians.

BACKGROUND: Patients undergoing elective joint replacement are mostly elderly individuals with multiple chronic diseases. These patients have a higher risk, so preoperative assessment for joint replacement is vital for surgical safety but remains challenging for junior physicians. This study aimed to evaluate the decision-making performance of different versions of DeepSeek (Chat and R1) and their knowledge base (KB)-enhanced schemes in perioperative risk assessment and their impact on junior physicians' decision-making quality. METHODS: A total of 292 anonymized medical records from joint re

Ultrasound-Responsive Hydrogels Empowered by Two-Dimensional Sonosensitizers Enable Spatiotemporally Controlled Fracture Fixation and Bone Regeneration.

Precise adhesion of bone fragments and robust bone regeneration are both essential for successful fracture healing, yet existing hydrogel adhesives rarely achieve controllable fixation and sustained osteogenesis simultaneously. Herein, we report a polyphenol-engineered ultrasound-responsive injectable hydrogel for fracture fixation and bone regeneration. The hydrogel is composed of dihydrocaffeic acid-modified chitosan, gelatin methacryloyl, a 2D sonosensitizer (PGO-Fe) constructed from polydopamine-mediated ferroferric oxide-decorated reduced graphene oxide, and polydopamine-modified hydroxya

Metaproteomic analysis of human sonicate fluid highlights neutrophil extracellular trap formation associated with periprosthetic joint infection.

UNLABELLED: Periprosthetic joint infection (PJI) is a devastating complication of total joint arthroplasty that can be challenging to diagnose and treat. Neutrophils are a prominent feature in PJI, with neutrophil extracellular traps (NETs) known to be present; little is known, however, about the physiology of PJI-associated NETs. Here, the proteomes of PJI and non-infectious arthroplasty failure (NIAF), analyzed using liquid chromatography tandem mass-spectrometry (LC-MS/MS) and proximity extension assay (PEA)-based profiling of fluids derived from sonication of arthroplasty components remove

Evaluation and management of neuropathic arthropathy of the shoulder: narrative review.

Neuropathic arthropathy of the shoulder, or Charcot shoulder, is a rare and destructive neuroarthropathy resulting from impaired protective sensation and altered joint mechanics that is most commonly associated with syringomyelia. The pathogenesis is believed to involve both neurotraumatic and neurovascular mechanisms, resulting in progressive osseous destruction, joint instability, and soft tissue compromise. Clinical presentation varies, with painless swelling, joint effusion, crepitus, and progressive limitation in shoulder function commonly reported. Radiographic findings frequently demons

Calcium Pyrophosphate Deposition Disease of the Temporomandibular Joint Extending into the External Auditory Canal and the Middle Ear, Coexisting with Otosclerosis: A Case Report.

Calcium pyrophosphate deposition disease (CPPD) is caused by CPP crystals and rarely forms tumors in joints. The authors describe a case of tumoral CPPD in the temporomandibular joint (TMJ), extending into the external auditory canal (EAC) and the middle ear. A 71-year-old man was referred to the department with a tentative diagnosis of CPPD in the right TMJ. Computed tomography showed the mass extending into the EAC, tympanic cavity, and bony Eustachian tube, with opacification of the mastoid cells. The audiogram revealed bilateral mixed hearing loss, suggesting the coexistence of bilateral o

Copper homeostasis dysregulation promotes malignant transformation of renal tubular epithelial cells via the ROS/NF-κB/AID signaling axis.

BACKGROUND: Clear cell renal cell carcinoma (ccRCC) is the most common subtype of renal cell carcinoma and is characterized by aggressive biological behavior and poor clinical outcomes. However, the molecular mechanisms underlying the malignant transformation of renal tubular epithelial cells are not yet fully understood. This study aimed to investigate the role of copper homeostasis dysregulation in regulating the reactive oxygen species/nuclear factor-kappa B/activation-induced cytidine deaminase (ROS/NF-κB/AID) signaling axis during ccRCC progression. METHODS: Copper ion levels, ROS accumu

Exploring the Role of Hyperfocus in the Psychophysiology of Probable Bruxism and Painful Temporomandibular Disorders.

BACKGROUND: Bruxism and temporomandibular disorders (TMD) are multifactorial conditions in which cognitive and behavioural traits may play an underrecognised role. Hyperfocus, defined as intense and sustained attentional engagement, has been proposed as a potential driver of heightened sensorimotor activity and pain vulnerability, yet its relevance to bruxism and painful TMD remains poorly understood. OBJECTIVES: To examine whether hyperfocus is associated with probable bruxism and painful TMD (pTMD), and to determine whether probable bruxism mediates the relationship between hyperfocus and p

Systemic Sclerosis Presenting With Long-Standing Peripheral Neuropathy and Delayed Multisystem Manifestations: A Case Report.

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder characterized by immune dysregulation, microvascular injury, and progressive fibrosis involving the skin and internal organs. Although Raynaud phenomenon and cutaneous sclerosis are the hallmark manifestations, atypical presentations may significantly delay diagnosis. Peripheral neuropathy is an uncommon neurological manifestation and rarely precedes the diagnosis of SSc by several years. We report the case of a 62-year-old female who presented with a 20-year history of progressive distal paresthesia involving both upper

A Living Multi-Omics PDX Biobank of High-Risk Sarcomas Reveals ecDNA-Driven Oncogene Amplification in Osteosarcoma and Enables Preclinical Precision Oncology.

Sarcomas are rare, genomically heterogeneous malignancies for which prognosis has remained poor and largely unchanged over recent decades. Patient-derived xenograft (PDX) models are an important tool for cancer research, but their availability and utility remain limited in sarcoma. Here, we establish the SGH sarcoma PDX biobank, a living multi-omics resource comprising 155 xenografts from 132 patients across 11 sarcoma histotypes, with osteosarcoma as the predominant subtype. This cohort is enriched for high-risk sarcomas characterized by poor prognosis and widespread copy-number alterations.

Evaluation of PDA‑PLL Nanoparticles for siRNA Delivery and ROS Scavenging in Rat Osteoarthritis.

INTRODUCTION: Osteoarthritis (OA) is a prevalent chronic inflammatory disease in which elevated TNF-α and excessive reactive oxygen species (ROS) play central roles. These two pathological factors mutually reinforce each other-TNF-α induces ROS production while ROS upregulates TNF-α expression-forming a vicious cycle that propels disease progression. However, current therapies offer only symptomatic relief without halting this process. To address this, we developed polydopamine-poly(L-lysine) (PDA-PLL) nanoparticles as a dual-functional platform that combines PDA-mediated ROS scavenging with P

Integrated Assessment of Abdominal Adiposity and Muscle Strength for Atherosclerotic Cardiovascular Disease Risk in UK Biobank: A Comprehensive Analysis From Population Cohort to Multi-Omics Molecular Exploration.

BACKGROUND: Waist circumference (WC) and grip strength (GS) are independently associated with atherosclerotic cardiovascular disease (ASCVD) risk. However, their joint impact remains insufficiently explored. This study aimed to investigate the association between WC, GS and ASCVD risk and to explore the underlying mediating factors. METHODS: This prospective cohort study included 271 061 UK Biobank participants. WC was categorised using World Health Organization sex-specific thresholds and GS was categorised into age- and sex-specific tertiles. Cox models estimated hazard ratios (HRs) and 95%
