Joint associations of leisure-time physical activity and daily sitting time with mortality among US adults classified as CKM stages 0-3: a prospective cohort study of NHANES 2007-2018.

BACKGROUND: Prior studies have linked physical activity to cardiovascular-kidney-metabolic (CKM) prevalence or mortality and sedentary behavior to CKM severity, and both behaviors are established mortality risk factors in the general population; however, prospective evidence on their joint associations with cause-specific mortality across CKM stages 0-3 remains limited. METHODS: We analyzed 8,556 NHANES 2007-2018 participants aged ≥20 years with CKM stages 0-3, complete prespecified covariates, and a positive fasting-subsample weight. Leisure-time physical activity (LTPA) was categorized as i

A corticohypothalamic circuit that mediates maternal awakening to pup cries.

Maternal caregiving requires sensory vigilance to ensure offspring survival, particularly during sleep. Infant cries are salient sensory cues that reliably trigger maternal arousal and caregiving responses in both humans and dams, but the underlying neural circuitry remains poorly defined. Here we identify a corticohypothalamic circuit in mice that mediates maternal awakening in response to pup calls. We show that layer 6 (L6) neurons in the temporal association cortex and ectorhinal cortex (TeA/Ect) projecting to the preoptic area (POA) exhibit enhanced responsiveness to pup calls after the t

Deep dynamical models of single-cell multiomic velocities predict loss-of-function and rescue perturbations in B cells.

Here we present DynaVelo, a generative neural ordinary differential equation model that learns the joint dynamics of gene expression and transcription factor (TF) motif activity from single-cell multiome data. DynaVelo learns a functional form of the vector field, such that the model can be perturbed in silico and not only used to draw trajectories. We show that DynaVelo recovers the bifurcating in vivo dynamics of wild-type murine germinal center (GC) B cells and reveals how these dynamics change under heterozygous loss of epigenetic regulators Arid1a and Ctcf. In silico perturbation yields d

A latent genetic factor underlies multiple component phenotypes of cerebral small vessel disease.

BACKGROUND: Cerebral small vessel disease (cSVD) encompasses white matter hyperintensities (WMH), enlarged perivascular spaces (PVS), intracerebral hemorrhage (ICH), small vessel stroke (SVS), and cognitive impairment-phenotypes that frequently co-occur yet have been studied as independent genetic endpoints. Whether a shared genetic architecture underlies these co-occurring cSVD-related phenotypes remains unclear. METHODS: We applied Genomic SEM to GWAS summary statistics from six phenotypes (WMH, PVS, ICH, SVS, cognitive performance, systolic blood pressure), followed by multivariate associa

Associations of pre-pregnancy BMI and gestational weight gain with large-for-gestational-age in women with gestational diabetes mellitus in Southwestern China.

BACKGROUND: Large-for-gestational-age (LGA) infants are a common complication of gestational diabetes mellitus (GDM), increasing the risk of obstetric trauma and long-term metabolic disorders. Pre-pregnancy body mass index (ppBMI) and gestational weight gain (GWG) are modifiable maternal factors, but their joint impact on LGA in sub-high-altitude, multiethnic populations remain unclear. OBJECTIVE: To examine the independent and combined associations of ppBMI and GWG with LGA risk among women with GDM in a sub-high-altitude, multiethnic region of southwestern China. METHODS: In this retrospec

Plant-derived extracellular vesicles deliver spermidine to reprogram macrophage metabolism and ameliorate rheumatoid arthritis.

Rheumatoid arthritis (RA) is driven by macrophage-mediated synovial inflammation closely linked to metabolic dysfunction, highlighting immunometabolism as a therapeutic target. Here, we engineered spermidine-loaded red cabbage-derived extracellular vesicles (RCEVs@SPD) as a plant-based nanoplatform to reprogram macrophage bioenergetics. In LPS-stimulated macrophages, RCEVs@SPD suppressed pro-inflammatory polarization and enhanced mitochondrial function, as evidenced by increased ATP production and mitochondrial membrane potential. Assay-based measurements further showed reduced extracellular a

A circRNA platform for safe partial reprogramming and rejuvenation of mesenchymal stem cells without pluripotent conversion.

BACKGROUND: Mesenchymal stem cells (MSCs) are widely used in cell therapies; however, replicative senescence during in vitro expansion limits their potency and consistency. Partial reprogramming has emerged as a promising strategy to rejuvenate aged cells, yet concerns regarding unintended pluripotent conversion have hindered its translational application. METHODS: Here we developed a circular RNA (circRNA)-based platform enabling safe partial reprogramming and rejuvenation of MSCs. We generated a lipid nanoparticle (LNP)-delivered circRNA co-expressing OCT4, SOX2, and KLF4 (circ-OSK) to achi

Health-related quality of life in childhood arthritis: the impact of generic versus disease-specific instruments.

BACKGROUND: Health-related quality of life (HRQoL) data provided by children with arthritis and their parents is critically important to enable co-design of care and shared decision making. However, there is significant heterogeneity in how HRQoL is captured, as both generic (EQ-5D-Y-5L) and disease-specific instruments (JAMAR) are available. This study aimed to examine the concordance of the JAMAR and EQ-5D-Y-5L questionnaires in childhood arthritis. METHODS: The international UCAN CAN-DU study prospectively enrolled consecutive children with arthritis across Canada and the Netherlands. This

Ferro-aging: a potential driver of degenerative bone and joint diseases.

Degenerative bone and joint diseases (DBJDs), including osteoporosis (OP), osteoarthritis (OA), and intervertebral disc degeneration (IVDD), are chronic age-related disorders characterized by progressive destruction of bone and joint tissues and loss of function, posing a heavy global health burden. Emerging evidence indicates that iron dyshomeostasis, chronic lipid peroxidation, and cellular senescence are common pathological features shared by these diseases. The newly discovered concept of ferro-aging-a non-lethal, persistent cellular state driven by sub-lethal lipid peroxidation that leads

Cadmium exposure triggers a functional switch of APP to drive joint degeneration.

BACKGROUND: Cadmium (Cd) is a pervasive environmental heavy metal linked to various chronic pathologies; however, its specific role and underlying molecular mechanisms in osteoarthritis (OA) pathogenesis remain poorly characterized. METHODS: We performed a cross-sectional analysis using the NHANES database (2001-2018) to evaluate the association between blood Cd levels and OA risk. Hub targets were identified by integrating the CTD and GEO databases through network toxicology and machine learning algorithms. Single-cell RNA sequencing (scRNA-seq) was employed to resolve the cellular localizat

Nuclear Receptor Subfamily 1 Group D Member 1 Promotes Wound Healing in Diabetic Mice through Transcriptional Repression of Stimulator of Interferon Genes.

Diabetic foot ulcer is one of the most debilitating complications of diabetes mellitus, primarily driven by persistent inflammation and impaired angiogenesis. This study investigates the role of the circadian clock protein, nuclear receptor subfamily 1 group D member 1 (NR1D1), in diabetic wound healing and elucidates its underlying mechanism. Clinical and experimental analyses revealed that NR1D1 levels were significantly reduced in both the serum and cutaneous wounds of diabetic patients and mice, with down-regulated NR1D1 correlating with delayed wound closure. Functionally, NR1D1 knockout

An anterior-first two-stage surgical strategy for neglected traumatic facet dislocation of subaxial cervical spine: a technical note.

BACKGROUND: Neglected subaxial cervical traumatic facet dislocation (TFD) presents unique surgical challenges due to fibro-osseous ankylosis and severe segmental instability. Traditional three-stage procedures are effective in complex cases but may increase operative complexity because of repeated patient repositioning and multiple surgical corridors. This study aimed to describe an anterior-first two-stage algorithm for neglected TFD and to report preliminary feasibility observations, including the selective use of a single-screw zero-profile cage as an anterior spacer. METHODS: Five patient

[The complicated ankle sprain].

Inadequate healing and rehabilitation after an ankle sprain may lead to chronic ankle instability, predisposing patients to impingement symptoms. The initial injury may also cause osteochondral lesions of the talus or peroneal tendon injuries, which are frequently underdiagnosed because symptoms overlap early with those of a simple ankle sprain. This review finds that rehabilitation remains the cornerstone of treatment for instability and impingement, while peroneal injuries are typically managed surgically. Management of osteochondral lesions depends on the type and severity.

Engineering composites inspired cortical-trabecular interlocking effect underlies bone strength in mouse vertebrae.

Bone strength is a major determinant of osteoporotic fracture risk. Current clinical assessments, however, rely primarily on imaging-based surrogates such as bone mineral density, offering limited insight into the integrated mechanical behavior of vertebral bone. Inspired by engineered composite structures, in which interfacial interactions substantially enhance mechanical properties, this study introduces the concept of the cortical-trabecular interlocking effect to characterize the mechanical integration between these two compartments and investigate its contribution to vertebral bone streng
