A standardized commercially available immunoassay is not available for detection of IgM antibodies against hepatitis C virus antigens (IgM anti-HCV). Therefore, different "in-house" enzyme immunoassays have been assessed. These assays vary greatly in sensitivity, but specificity seems satisfactory in all of them. A typical IgM antibody response to HCV antigens is usually found in nearly all patients with acute hepatitis C. This antibody response rarely precedes the appearance of IgG anti-HCV, and it persists for a few months at high titer. Low titers of IgM anti-HCV are detectable in 50-80% of

In Japanese blood donors, positivity for antibodies to hepatitis C virus (HCV) ranges from 0.2% in subjects under 20 to 3.9% in those over 50 years. It is estimated that at least 2.3 million Japanese have contracted HCV infection through contaminated blood. HCV carrier state was confirmed by polymerase chain reaction for HCV-RNA in subjects positive for antibodies to more than one viral protein (70% of cases). Subjects positive for core antibody alone, however, were found to be HCV-RNA negative with normal liver function, and are considered to have only a past history of HCV infection (30% of

Acute hepatitis B virus (HBV) infection is typically distinguished from chronic disease by a positive IgM anti-hepatitis B core antigen (anti-HBc) test. Patients with chronic hepatitis B remain hepatitis B surface antigen (HBsAg) positive, often with raised serum alanine aminotransferase (ALT) activities, for more than six months. The presence of hepatitis B e antigen (HBeAg) and HBV-DNA correlates with infectivity (although patients infected with the pre-core mutated virus may be HBeAg negative). Immunity after HBV infection is characterised by the presence of anti-HBs and anti-HBc antibodies

Chronic hepatitis C is one of the major causes of liver cirrhosis and seems to play a role in the pathogenesis of primary liver cancer. So far, no effective therapy--with the exception of some antiviral agents--was available for this disease. Interferon-alpha (IFN-alpha) therapy results in about half of patients in a normalisation of liver transaminases within 4 weeks. Placebo-controlled studies confirm that in about 40% of patients IFN therapy achieves complete remissions. Frequently termination of IFN therapy is followed by a relapse. Therefore higher IFN doses and longer and/or repeated the

Acute hepatitis B is a self limiting disease that resolves spontaneously in 95% of patients who acquire the infection in adulthood. Patients with acute hepatitis B, however, usually take between four and 12 weeks to recover; the personal, social, and sanitary costs associated with this are high. Also, 5-10% of patients with acute hepatitis B go on to develop chronic liver disease. Hepatitis C, by comparison, is an asymptomatic disease associated with a high progression to the chronic stage. At least 50% of patients with post-transfusion hepatitis C develop chronic liver disease or cirrhosis. W

Polymerase chain reaction is a highly sensitive technique for the detection of hepatitis B virus-DNA and hepatitis C virus-RNA in serum, liver tissue, and peripheral mononuclear blood cells. In chronic hepatitis B, it is particularly useful for identification of infectious subjects who are hepatitis B surface antigen positive and anti-hepatitis B e antigen antibody-positive, and for follow up of hepatitis B virus infections in liver transplantation programmes. Polymerase chain reaction detection of hepatitis C virus-RNA in serum may be the only means of confirming acute hepatitis C infection a

Post-transfusion hepatitis was studied prospectively in 1,476 patients undergoing open-heart surgery between 1985 and 1988. Thirty-three (2.2%) patients suffered from post-transfusion hepatitis. Acute post-transfusion hepatitis was attributed to hepatitis B in one case and to hepatitis C in ten patients (0.7%). Four additional patients had preexisting serologic markers of hepatitis C. In 22 (1.5%) patients, hepatitis B or C was excluded as a cause of liver disease. Seroconversion for hepatitis C virus occurred from 3 weeks to more than 6 months after infection. Chronic hepatitis C developed in

Aplastic anemia (AA) is a rare complication of viral hepatitis affecting mainly children or young adults. Most reported cases have been associated with community-acquired non-A, non-B hepatitis, but hepatitis A and B have also been implicated in a few patients. We report on a 43-year-old woman with severe AA, in association with acute hepatitis C virus (HCV) infection, diagnosed by detection of HCV RNA by the polymerase chain reaction technique. Her AA was successfully treated with anti-thymocyte globulin and cyclosporin A. The hepatitis C progressed to chronic disease but, despite a follow-up

Patients receiving multiple transfusions are at risk of acquiring hepatitis C (HCV) infection from a donor population which is unscreened for hepatitis C antibodies (anti-HCV). Prior to the introduction of blood donor screening for anti-HCV in the U.K., a group of patients undergoing therapy for haematological malignancies, with repeatedly abnormal liver function tests, were investigated for acute HCV infection. Thirty-two patients had repeatedly raised serum transaminases, and eight of these (25%) had evidence of an acute HCV infection. The diagnosis was made by the detection of HCV-RNA in th

Putative hepatitis C virus core sequence was amplified from a serum sample positive for anti-C-100-3 and expressed in Escherichia coli. Approximately 62 kDa fusion protein with maltose binding protein containing 20 kDa hepatitis C core protein was obtained. The antibody to this protein was detected in 53 of 54 (98%) sera from hepatitis C virus ribonucleic acid-positive patients including 40 sera positive for anti-C-100-3 and 13 sera negative for anti-C-100-3. The antibody was also detected in all of 12 patients with acute hepatitis C showing the earlier detectability of the antibody than anti-

The diagnosis of acute hepatitis C (AHC) often can only be suspected because current serologic tests remain negative for over 3 months. Because histologic features might provide useful clues, we reviewed 85 liver biopsy specimens from 85 patients with acute viral hepatitis, comparing 22 cases of AHC with 23 cases of acute hepatitis A (AHA), 30 cases of acute hepatitis B (AHB), and 10 cases of acute hepatitis non-A, non-B, non-C (AHNC). AHC was characterized by dense portal lymphoid aggregates (7 cases) and Poulsen-Christoffersen-type cholangitis (8 cases); these lesions were not found in any o

The presence of hepatitis C virus RNA was examined in the sera of 32 patients with acute non-A, non-B hepatitis. Hepatitis C virus RNA was extracted from serum, reverse transcribed to cDNA and amplified by two rounds of polymerase chain reaction. Hepatitis C virus RNA was detected in all patients who developed anti-C100-3 as well as 79% who did not. These data suggest that hepatitis C virus accounts for most cases of acute non-A, non-B hepatitis whether anti-C100-3-positive or not. In addition, hepatitis C virus RNA was detected in the earliest serum samples, 9 +/- 11 weeks before anti-C100-3,

The immunoglobulin M antibody against hepatitis C viral capsid (core) antigen has been found to be a possible marker for acute infection in post-transfusion hepatitis C. We extended this observation to a sporadic hepatitis C. In addition, a modified recombinant immunoblot assay (RIBA) that contained three HCV antigens (capsid, NS3 and NS5) in one strip was developed and used to study the antibody response among patients. This allowed a simultaneous comparison of either immunoglobulin M or G specific to individual HCV antigen and yielded more information. The assay was found useful in the diagn

Sixteen patients with acute hepatitis C having a duration of illness more than three months without recovery were randomly assigned to treatment with interferon (IFN). 8 patients received 3 million units a day during the first week and 3 million units every other day for the following 11 weeks. Another 8 patients were given 3 million units every other day for 12 weeks. An additional 16 patients were chosen as controls. The effect of IFN therapy was 93.8% in the treated group and only 12.5% in the control group (P < 0.005). Among 11 patients who completed the course of therapy, only one had a r

Thirty-nine of 61 prospectively followed patients who had had acute non-A, non-B hepatitis in 1978 were clinically reexamined in 1991 and tested for antibodies to hepatitis C virus (anti-HCV) with a second generation ELISA and RIBA and for HCV RNA by PCR. Acute hepatitis C was diagnosed in stored sera from 1978 in 24 patients, who were found still to be anti-HCV positive in 1991, and 16 of them were also HCV RNA positive. The majority of anti-HCV positive patients with or without HCV RNA had elevated serum ALT levels 13 years after onset of their acute hepatitis C. After 13 years follow-up, 1.

IgM antibody against hepatitis C virus (IgM anti-HCV) was measured in serial samples from 15 transfusion recipients in whom posttransfusion chronic non-A, non-B hepatitis (NANBH) developed and three plasmapheresis donors during acute HCV infection using recombinant proteins derived from three immunodominant regions: core, NS-3, and NS-4 (c100). IgM anti-HCV core was detected in 13 of 15 posttransfusion patients. Nine of these patients had transient, acute-phase IgM anti-HCV core detected coincidentally or earlier than active IgG anti-HCV core response. The average duration of IgM anti-HCV core

BACKGROUND: The discovery of the hepatitis C virus (HCV) and the possibility of determining anti-HCV serum antibodies has permitted greater etiological, clinical and epidemiological knowledge of non A non B hepatitis (NANB).
METHODS: The clinical and epidemiological characteristics of 47 patients with acute hepatitis C were studied. Diagnosis was based on compatible clinical and analytical picture as well as positivity of the anti-HCV antibodies.
RESULTS: Thirty-eight percent of the patients were intravenous drug abusers (IVDA). Thirty-four percent had transfusion antecedents and the remaini

The serological response during acute hepatitis C virus (HCV) infection was examined by enzyme-linked immunosorbent assay (ELISA) in sequential serum samples from 13 haemophiliacs following their first exposure to factor VIII concentrates contaminated with HCV. The commercially available C100-3 peptide and a new 22 kDa recombinant protein (p22) encoded by the nucleocapsid region of the viral genome were used for antibody detection, whilst a nested polymerase chain reaction (PCR) method was used for the detection of viraemia. In addition, eight sporadic cases of acute HCV infection were studied
