Lipoid Pneumonia: HRCT and MRI Spectrum, Diagnostic Pitfalls, and Imaging-Based Diagnostic Workflow.

Background/Objectives: Lipoid pneumonia (LP) is a rare and frequently underdiagnosed pulmonary condition with a broad spectrum of radiological manifestations that can closely mimic infectious, inflammatory, and neoplastic lung diseases. Despite its clinical relevance, no standardized imaging-based diagnostic pathway exists. For this reason, this pictorial narrative review aims to provide a structured, imaging-centred synthesis of LP, to characterise the full spectrum of high-resolution CT (HRCT) and magnetic resonance imaging (MRI) findings, and to propose a pragmatic diagnostic workflow. Gonadotropin-Releasing Hormone Analog Followed by Sex Hormone Therapy as a Preventive Strategy against Ovarian Complications in Congenital Lipoid Adrenal Hyperplasia: A Case Report.

BACKGROUND: Congenital lipoid adrenal hyperplasia (lipoid CAH) is caused by pathogenic variants in STAR. In 46,XX patients, gonadotropin-driven cholesterol ester accumulation leads to premature ovarian insufficiency, with approximately one-third developing large ovarian cysts or torsion often requiring surgical intervention. No preventive strategy is established. We report a 46,XX patient treated with a gonadotropin-releasing hormone (GnRH) analog followed by estrogen and progestin, with longitudinal ovarian monitoring. CASE PRESENTATION: An 18-year-old Japanese female was diagnosed with lipo

Repeated Self-induced Vomiting-associated Exogenous Lipoid Pneumonia Following Occult Olive Oil Aspiration.

We report a case of exogenous lipoid pneumonia, which was possibly caused by the chronic aspiration of olive oil during recurrent self-induced vomiting episodes. A 28-year-old woman with a history of depression, self-induced vomiting, and type 1 diabetes was referred because of abnormal chest radiography findings during screening. Chest CT revealed ground-glass opacities, predominantly in the right middle and lower lobes. A bronchoalveolar lavage fluid examination revealed a bi-layer separation of lipid-laden macrophages from the cell suspension. Diagnostic video-assisted thoracoscopy confirme

The successful use of rituximab in children with IgA nephropathy and minimal change-like lesions: a case series.

IgA nephropathy with minimal change disease (IgAN-MCD) is a rare glomerulopathy combining features of both entities, typically presenting as steroid-sensitive nephrotic syndrome (NS) with frequent relapses. While rituximab (RTX) is commonly used in pediatric glomerular diseases, its efficacy in IgAN-MCD remains unclear. Our retrospective study evaluated the efficacy and safety of RTX in eight children with IgAN-MCD who remained steroid-dependent or frequently relapsed despite treatment with glucocorticoids and second-line immunosuppressants before RTX. Over a follow-up period of 12-41 months,

Clinical characteristics and outcomes of adults with minimal change disease: A retrospective, cross-sectional study.

Minimal change disease (MCD) is a common cause of nephrotic syndrome in adults, with limited evidence available on its treatment and prognosis. In this study, we retrospectively included the clinical characteristics and treatment results of adult MCD patients in our center and explored and analyzed potential risk factors for relapse in MCD patients. We included 51 adult MCD patients with a median age of 29 years, and 30 were men. Among them, 16 patients (31.37%) had acute kidney injury (AKI) at presentation. The average urinary protein excretion was 6.39 ± 5.54 g. Notably, 29 (59%) patients ha

Immunoglobulin A nephropathy with superimposed minimal change disease complicated by acute kidney injury requiring temporary hemodialysis.

Immunoglobulin A (IgA) nephropathy (IgAN) with superimposed minimal change disease (MCD) is a rare but increasingly recognized clinicopathological phenotype that is generally associated with a favorable renal prognosis. We present a case of IgAN with MCD complicated by severe acute kidney injury (AKI) during the clinical course and requiring temporary renal replacement therapy. A 38-year-old man presented with acute nephrotic syndrome. Renal biopsy showed mesangial IgA deposition, and anti-galactose-deficient IgA1 antibody (KM55) staining demonstrated co-localization with IgA deposits, support

Polymeric mixed micellar nanogel enhances dermal delivery and therapeutic efficacy of tofacitinib citrate.

This study aimed to develop and evaluate a polymeric-surfactant stabilized mixed micellar (MM) gel of tofacitinib citrate (TFC) for localized management of rheumatoid arthritis (RA). TFC-MM gel was formulated via a simple and scalable cold dispersion self-assembly approach using Lipoid S75, Tween 80, and Poloxamer 407, incorporated into a Carbopol hydrogel matrix. The optimized formulation exhibited nanosized particles (i.e., 67.2 nm), narrow size distribution (PDI ∼ 0.11), and high drug entrapment (i.e., 80.7%). The developed micellar gel showed sustained kind of drug release (i.e., 98.6% in

Cerebral venous sinus thrombosis secondary to relapsed minimal change disease: a case managed with endovascular thrombolysis.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare type of stroke characterized by the formation of blood clots within the dural venous sinuses which has a high mortality rate. Despite its potential severity, cerebral venous sinus thrombosis secondary to nephrotic syndrome remains rarely reported and under-recognized in clinical practice. CASE PRESENTATION: We described a 20-year-old male who presented with a relapse of minimal change disease, complicated by extensive CVST. On the basis of anticoagulant therapy, his neurological condition deteriorated rapidly, manifesting as altere

Lipoid Pneumonia.

Lipoid pneumonia is an uncommon disorder characterized by intra-alveolar lipid accumulation and classified as exogenous or endogenous. Exogenous forms most commonly result from aspiration of oil-based substances, whereas endogenous forms are associated with airway obstruction, malignancy, inflammatory disorders, and impaired macrophage function. Imaging manifestations are varied and frequently misleading, ranging from ground-glass opacities and consolidation to crazy-paving, nodules, and mass-like lesions that may closely mimic malignancy, infection, or other alveolar filling disorders. Althou

Solid nano-lipoidal intelligent premix (SNIP): designing, optimization, and characterization.

BACKGROUND: Bicalutamide possesses very low oral bioavailability due to its low water solubility and p-gp efflux. The present invention aimed to improve the solubility and availability of Bicalutamide at the absorption site using Epigallocatechin-3-gallate (EGCG) and to design a patient- and industry-friendly solid nanolipoidal intelligent premix (SNIP). Exploring natural excipients is another aim. METHODS: Various oils, surfactants, and cosurfactants were employed to investigate the Bicalutamide solubility. EGCG was used as a p-gp substrate to improve the absorption of Bicalutamide. An optim

Tip Lesion Variant of Focal Segmental Glomerulosclerosis in Familial Relapsing Polychondritis.

Relapsing polychondritis (RP) is a rare autoimmune disorder with minimal reported renal involvement. We describe the first case of tip lesion variant of focal segmental glomerulosclerosis (FSGS) in a 60-year-old male with familial RP. The patient initially presented with nephrotic syndrome concomitant with RP, which was diagnosed 17 years ago; renal biopsy revealed minimal change disease at that time. Over the subsequent 17 years, he experienced four RP flares without nephrotic syndrome recurrence until his recent presentation with severe nephrotic syndrome. Repeated renal biopsy confirmed tip

Ethanol-free nanostructured lipid carrier hydrogels as alternatives to hydroalcoholic gels and compounded creams for dermal testosterone delivery: physicochemical stability, ex vivo human skin penetration and cellular compatibility.

Topical testosterone therapy is widely used for testosterone replacement therapy and gender-affirming hormone therapy, but available hydroalcoholic gels contain high ethanol concentrations that may compromise skin barrier integrity during repeated use. In parallel, testosterone creams are prepared through pharmaceutical compounding. However, they lack data on physicochemical stability, skin penetration potential, and cellular compatibility. This study aimed to develop ethanol-free nanostructured lipid carrier (NLC)-based hydrogels as alternatives for dermal testosterone delivery. Testosterone-

Protein endocytosis dysfunction in kidney biopsies of patients with diabetic nephropathy and minimal change disease: a pilot study on ClC-5 and the megalin receptor.

BACKGROUND: Diabetic nephropathy (DN) is a major cause of chronic kidney disease. Proteinuria is a hallmark of DN. An active receptor-mediated pathway involving megalin and cubilin receptors and the Cl-/H+ antiporter ClC-5 is responsible for filtered albumin uptake in the proximal tubular epithelium. The aim of our pilot study was to analyze the patterns of megalin and ClC-5 expression in the kidney biopsies of patients with DN, minimal change disease (MCD), and controls, and their correlations with kidney dysfunction. METHODS: Kidney biopsies of 10 DN and 8 MCD patients, and 9 controls (CTRs

Mortality in Salmon (Salmo salar) Fry Related to Lipoid Liver Disease Induced by Unbalanced Fatty Acid Composition of the Diet.

In 2023, all but one Swedish restocking farm rearing Baltic salmon and sea trout fry experienced massive mortality caused by lipoid liver disease (LLD). The struck farms used the same feed, and this was suspected to be the cause of the outbreak. We performed two parallel feed trials in three salmon populations as well as fatty acid and antioxidant analyses of feed and fish to investigate the causal relationship between the feed and the LLD mortality. Significant mortality was induced in one population and LLD histopathology was induced in all three populations in groups fed the suspected feed

Lipoid Proteinosis with Homozygous ECM1 c.507del Variant and Functionally Significant Eyelid Involvement: A Case Report.

BACKGROUND: Lipoid proteinosis (LP), or Urbach-Wiethe disease, is a rare autosomal recessive disorder caused by pathogenic variants in ECM1. Early-onset hoarseness and moniliform blepharosis are characteristic, but diagnosis may be delayed when early airway findings are nonspecific or the mucocutaneous features are not considered together. CASE SUMMARY: A 13-year-old male presented with hoarseness since infancy and a 2-year history of progressive bilateral upper-eyelid margin papules. Flexible nasopharyngolaryngoscopy at ages 1 and 2 years had been interpreted as congenital laryngomalacia. Re
