Neurogenic acceleration of degenerative joint lesions.

A severe form of degenerative joint lesion (neuropathic arthropathy) is known to complicate a variety of diseases that are associated with sensory abnormalities. We studied the relationship between sensory deficits and the development of degenerative joint lesions in dogs in two complementary experiments. In Experiment 1, dogs that were subjected to unilateral dorsal-root ganglionectomy (fourth lumbar to first sacral vertebra) failed to show biochemical, gross, or histological evidence of degenerative joint lesions in ipsilateral femoral condylar cartilage after sixteen months. In Experiment 2

Charcot's arthropathy of the spine following spinal cord injury. A report of five cases.

Between 1955 and 1968, we treated five patients who had post-traumatic Charcot's arthropathy of the spine. All of the patients had a neurologically complete spinal-cord lesion. A laminectomy was implicated as the cause in four of the five patients. Spinal arthrodesis was used successfully to correct the deformity, stabilize the spine, restore sitting balance, and prevent complications resulting from neuropathic arthropathy of the vertebrae following complete spinal-cord injury.

[Cauda equina syndrome with plantar perforating ulcer and neurogenic osteoarthropathies in a case of rheumatismal pelvospondylitis].

The authors report a case of ankylosing spondylitis complicated by a cauda equina syndrome presenting with a plantar perforating ulcer and lesions of neuropathic arthropathy of the foot. On the basis of an analysis of 54 cases published in the literature they review the principal aspects of this complication of ankylosing spondylitis. Neurological manifestations occur late after several decades of progression of the disease. Urinary sphincter problems are common whilst trophic disturbances remain rare. Myelography and, above all, lumbar CAT scan reveal characteristic and stereotyped anatomical

Multiple neuropathic arthropathy in a patient with syphilis.

An unusual case of neuropathic arthropathy in a patient with tabes dorsalis is presented. The special feature consisted of multiple joint involvement in the upper limbs and spine. This uncommon location of Charcot joints can probably be attributed to the patient's occupation which involved heavy strains on his upper limbs.

Neuropathic arthropathy (Charcot's joint) in dialysis patients.

To the author's knowledge, uraemic neuropathy has not been previously reported as a cause of Charcot's joint. In this paper they present three cases in which the association between clinical and radiographic patterns suggest the diagnosis of neuropathic arthropathy. The features of uraemic neuropathy are stressed and the role of secondary hyperparathyroidism in the development of this type of arthropathy is discussed. The extremely severe hyperparathyroidism reported here, may cause tendon and ligament disease, especially at the site of their bone insertion. Uraemic tendon and ligament failure

Neuropathic arthropathy (Charcot joints) due to cervical osteolysis: a complication of progressive systemic sclerosis.

A patient with progressive systemic sclerosis who presented with a painless, destructive joint disease consistent with a neuropathic arthropathy is described. Roentgenogram of the cervical spine revealed marked osteolysis of C1 and C2 which allowed the odontoid process to migrate upward, compress and produce atrophy of the cervical spine cord. We suggest that these changes were responsible for the development of the neuropathic joints.

Syphilis and axial arthropathy.

Inadequately treated syphilis predisposes the patient to the long-term complications of his disease. Neuropathic arthropathy (the Charcot joint), especially when axial or spinal, is critical to diagnosis, but is sometimes overlooked. We have reported a patient with syphilis, both neurologic and joint disease, and included the first computed tomographic views of vertebral syphilitic osteitis.

Cauda equina lipoma presenting as acute neuropathic arthropathy of the knee. A case report.

A 36-year-old man with a painless, rapidly progressive, destructive lesion of the right knee proved to have neuropathic arthropathy. Rather than being a tumor, the destructive process was neuropathic in origin. Myelography revealed an unsuspected intradural lesion, which proved to be a benign lipoma, at the third and fourth lumbar levels. Excision of the lipoma and arthrodesis of the knee were successful. True intrathecal lipomas are rare lesions that become symptomatic during a period of many years; when they are removed the prognosis is generally good. As in the present case, in neuropathic

Diagnosis of post-traumatic syringohydromyelia presenting as neuropathic joints. Report of two cases and review of the literature.

Two paraplegic men with post-traumatic syringohydromyelia presented initially with neuropathic arthropathy of the elbow and shoulder, respectively. Both patients had sustained spinal trauma years earlier and had been lost to orthopedic and neurosurgical follow-up study. Characteristic history and physical findings were present in both patients. Conventional myelography failed to demonstrate the lesion in the first patient. The diagnosis in the second patient was confirmed by lumbar injection of low-dose metrizamide followed by immediate and delayed computerized axial tomography in the supine a

Spina bifida and unilateral focal destruction of the distal femoral epiphysis.

Focal destruction of the postero-lateral distal femoral epiphysis was present on radiographs in two children with spina bifida and objective lower limb sensory loss. Each patient presented with painless swelling of the knee. In one patient the epiphysis showed sclerosis and fragmentation associated with a defect. In the second patient the destructive change was the dominant radiographic abnormality and simulated bone tumor. Computed tomography in this patient showed a bone fragment occupying the defect suggesting epiphyseal fracture. The lesion in each patient was believed to be traumatic in o

Neuropathic arthropathy in the diabetic foot.

Neuropathic arthropathy (Charcot's joint) is a relatively painless, progressive and degenerative condition due to underlying neurologic deficits. Although a variety of neurologic disorders may produce this arthropathy, diabetes mellitus has become the most common cause. In diabetes, the foot and ankle are the sites most often involved, particularly the tarsometatarsal and tarsal joints. In addition to neuropathy, trauma is an essential factor in producing the arthropathy.

Radiological changes in serial x-rays of the foot and tarsus in leprosy.

Most bone abnormalieis seen in serial films of the feet in leprosy were static being the end result of old disease. Changes over a period of time were observed in serial fims of 214 feet and these changes were due to a combination of infection, neuropathic changes and avascular necrosis occurring in anaesthetic feet. The bone abnormalities seen were absorption of toes and metatarsals; bone destruction; a neuropathic arthropathy of toes and metatarsals and various patterns of tarsal disintegration.
