Rapid, progressive neuropathic arthropathy of the hip in a patient co-infected with human immunodeficiency virus, hepatitis C virus and tertiary syphilis: case report.

BACKGROUND: Syphilis is a chronic infection that is classified into three stages. In its tertiary stage, syphilis spreads to the brain, heart and other organs; the lesions may involve the skin, mucous membranes and bones. Neuropathic arthropathy associated with tertiary syphilis has rarely been described in Europe and its association with HIV-HCV co-infection has not been reported so far. This article reports the case of a man with tertiary syphilis presenting with rapidly evolving neuropathic arthropathy of the hip and extensive bone destruction. CASE PRESENTATION: On initial presentation, th

Midtarsal arthrodesis in the treatment of Charcot midfoot arthropathy. Surgical technique.

BACKGROUND: Fracture-dislocation of the midfoot with collapse of the longitudinal arch is common in patients with neuropathic arthropathy of the foot. In this study, we describe a technique of midfoot arthrodesis with use of intramedullary axial screw fixation and review the results and complications following use of this technique. METHODS: A retrospective study of twenty-two patients who had undergone surgical reconstruction and arthrodesis to treat Charcot midfoot deformity was performed. Bone resection and/or osteotomy were required to reduce deformity. Axially placed intramedullary screw

Long-term follow-up of the surgical management of neuropathic arthropathy of the spine.

BACKGROUND CONTEXT: No studies have discussed the long-term surgical management and outcomes of Charcot arthropathy of the spine. This case series presents nine patients treated over 30 years. The study hypothesis was that surgery would reduce instability, pain, recurrence, and the need for revision surgery in the long-term, given previous study findings of successful fusion of Charcot spine in the short-term. PURPOSE: To evaluate the long-term outcomes of surgery for Charcot spine. STUDY DESIGN/SETTING: Retrospective case series. Cases took place at Stanford University Medical Center and Sa

Charcot arthropathy of the knee. A case-based review.

The diagnosis of Charcot arthropathy in the knee is rare. However, there is an increasing number of diabetic patients, and they are living longer due to improvements in treatment. Because neuropathic arthropathy is a late effect of peripheral neuropathy, we can expect an increasing incidence of neuropathic arthropathy. Total knee arthroplasty is the preferred choice of treatment by patients, although it may also be associated with a high incidence of serious complications. This case report presents a young female with diabetes mellitus and Charcot arthropathy of the knee managed by total knee

Neuropathic arthropathy of the shoulder associated with syringomyelia: a report of six cases.

Here, we report a series of 5 patients (6 shoulders) diagnosed with neuropathic arthropathy of the shoulder joint in our clinic between 2005 and 2008. Initial diagnosis, previous treatment, and radiological and clinical follow-up findings were reviewed. The mean age at diagnosis was 44.2 years. Four patients had unilateral and 1 patient had bilateral involvement. The presenting symptoms were pain, swelling, and loss in range of motion. Active forward flexion and abduction ranged from 0° to 90°. Hypoesthesia and loss of temperature sense was evident in 3 patients. Radiographs showed massive ost

[Chronic hydarthrosis of the knee in a Tunisian patient: tabetic arthropathy].

Tabetic arthropathy (TA) is a type of neuropathic arthropathy that has become rare. The purpose of this report is to describe a case of tabetic arthropathy involving the knee. A 53-year-old man was hospitalized for painless right knee arthropathy. Clinical examination demonstrated hydarthrosis of the right knee, peripheral neuropathic syndrome of the lower limbs, and paralysis of the IXth and Xth cranial pairs. Plain radiography showed the presence of lytic lesions in the internal femoral condyle and tibial plateau. Syphilitic serology tests were positive in the blood and cerebrospinal fluid.

Arthroplasty of a Charcot knee.

The Charcot knee - or neuropathic arthropathy - presents a considerable challenge to the orthopaedic surgeon. Caused by a combination of sensory, motor and autonomic neuropathy, it was originally described as an arthritic sequelae of neurosyphilis. In today's western orthopaedics it is more often caused by diabetes. A Charcot knee is often symptomatically painful and unstable. Traditional management has usually been conservative or arthrodesis, with limited success. Arthroplasty of a Charcot joint has commonly been avoided at all costs. However, in the right patient, using the right technique,

Case report: Neuropathic arthropathy of the hip as a sequela of undiagnosed tertiary syphilis.

BACKGROUND: Neuropathic arthropathy is characterized by rapidly progressive bone destruction in the setting of impaired nociceptive and proprioceptive innervation to the involved joint. It is seen most commonly in the foot and ankle, secondary to peripheral neuropathy in patients with diabetes mellitus. Other less common sites of involvement may include the knee, hip, shoulder, and spine, depending on the underlying etiology. Neuropathic arthropathy can be associated with tabes dorsalis, a unique manifestation of late, tertiary neurosyphilis that may arise in individuals with untreated syphili

Syringomyelia presenting as shoulder instability.

Three middle-aged women presented with shoulder instability as the first sign of syringomyelia. The patients' symptoms were unilateral in each case. Two patients had posterior shoulder instability, and one patient had anterior instability. Two patients had undergone surgery to stabilize the shoulder before referral to our center. In both cases the surgery had been unsuccessful. Review of two of the patients' plain radiographs after referral demonstrated radiographic changes highly suggestive of neuropathic arthropathy. The diagnosis of syringomyelia was confirmed by myelography, computed tomog

Arthrodesis of neuropathic ankle joint by Ilizarov fixator in diabetic patients.

BACKGROUND: Fusion of the neuropathic ankle joint is extremely difficult and associated with many complications. The use of the Ilizarov fixator in ankle fusion for patients with neuropathic arthropathy is not clear. We aimed to evaluate the results of the Ilizarov method for ankle arthrodesis in diabetic patients with neuropathic arthropathy. METHODS: We report the results of neuropathic ankle joint arthrodesis performed with the Ilizarov apparatus in 11 patients. The mean age of the patients was 51 years (range, 35-67 years), all patients were diabetic, and they all had a history of ankle t

Rapidly progressive neuropathic arthropathy of the knee in possible association with a huge extruded cervical intervertebral disc herniation.

A 58-year-old woman with an insignificant stage of diabetes mellitus presented with rapidly progressive osteoarthritis of her right knee exhibiting painless but significant antero-posterior and lateral instability. In association, she had a huge extruded left-sided cervical intervertebral disc at the C4-5 level. Neurological examination of the upper limbs was normal but, she had diminished pinprick sensation on the right leg. Radiographic findings were consistent with neuropathic arthropathy of the knee. Anterior decompression with fusion of C4-5 and total knee arthroplasty using a semi-constr

Charcot's arthropathy secondary to herpetic encephalitis sequelae: an unusual presentation.

Neuropathic arthropathy (Charcot's arthropathy) is a progressive articular disease associated with a reduced sensorial and protector proprioceptive reflex. Its etiology includes many different conditions such as syringomyelia, traumatic lesion causing medullary deformity, spina bifida, diabetic neuropathy, leprosy neuropathy, neurofibromatosis, amyloid neuropathy, alcohol, and repetitive injection of hydrocortisone into joints, among others. However, the relationship between Charcot's arthropathy and herpetic encephalitis has not yet been described. Herpes encephalitis causes acute and chronic

Unilateral developments of osteoarthritis and Charcot's joint in a patient with neurofibromatosis.

BACKGROUND: Precise mechanism of developing neuropathic arthropathy known as Charcot's joint is not fully understood. CASE REPORT: A 55-year-old Japanese woman with neurofibromatosis-1 complained of right gonalgia in December 2001. Physical examination revealed a huge tumor in the right lower leg without signs of inflammation. Laboratory findings were unremarkable. Radiographic examination disclosed the presence of osteoarthropathy in the right knee joint. In contrast, radiologic findings of the right foot and ankle were compatible with neuropathic arthropathy. Further investigations could no

[Septic arthritis complicating neuropathic shoulder due to cervical syringomyelia].

One of the main causes of neuropathic osteoarthropathy of the shoulder is cervical syringomyelia. Chronic pain and swelling of the shoulder are the most frequent manifestations, but it occasionally can develop rapid osteoarticular destructive lesions (in less than six weeks), which raise the diagnostic possibility of septic arthritis and some tumours We present the report of two men with septic arthritis of the shoulder associated with neuropathic arthropathy secondary to syringomyelia. Both patients presented with sudden shoulder pain exacerbated by either passive or active joint movements, m

Neuropathic arthropathy progressing with multiple joint involvement in the upper extremity due to syringomyelia and type I Arnold-Chiari malformation.

Neuropathic arthropathy (NA), known as Charcot neuroarthropathy, is a chronic, degenerative arthropathy and is associated with decreased sensory innervation. Numerous causes of this arthropathy have been described. Neuropathic joint, although first described by Charcot in tabes dorsalis in 1868, has subsequently been observed in a variety of conditions including syringomyelia, diabetes mellitus and peripheral nerve disorders. Syringomyelia is characterized by slow progression. The shoulders and elbows are the most frequently involved joints in syringomyelia. Involvement of the hand is a quite

[Charcot's neuroarthropathy].

Diabetes is the most common cause of neuropathic arthropathy. The most frequent localization is the tarsus. In clinical practice, the insidious painful onset and discrete inflammatory signs of this disease often lead to a mistaken interpretation by the physicians who usually manage these patients (endocrinologists, rheumatologists, podiatrists, etc.), delaying diagnosis with devastating consequences. We report four cases of neuropathic arthropathy in diabetic patients diagnosed in our service. The clinical features that may suggest a diagnosis in the early stages of this entity are stressed.

Reduced lower extremity vibratory perception in osteoarthritis of the knee.

OBJECTIVE: Vibratory perception threshold (VPT) assesses a distinct yet related sensory pathway that has been associated with neuropathic arthropathy but has not been assessed in knee osteoarthritis (OA). The purpose of this study was to evaluate VPT in subjects with knee OA to determine whether the lower extremity afferent deficits observed in knee OA involve more than just proprioception. METHODS: Twenty-seven individuals with symptomatic and radiographic knee OA were compared with 14 age-matched normal subjects. VPT was assessed using a biothesiometer. Five sites of the lower extremity wer
