Fewer poor joint awareness results with cementless TKA compared with the same-design cemented implant.

BACKGROUND: Comparisons of cemented and cementless TKA have focused on mean outcomes and generally reported equivalence. We hypothesized that a 3D-printed full-titanium cementless TKA and the same-design cemented implant would not differ significantly in average joint awareness but would differ in the proportion of patients with the poorest results, and we compared the two both at the mean and across the outcome distribution. METHODS: Prospective two-cohort study of 109 primary unilateral TKAs (54 cementless, 55 cemented) for end-stage knee osteoarthritis, with complete 24-month follow-up. Ex

Real world evidence for glucosamine-chondroitin-calcium supplementation on joint pain.

BACKGROUND: Joint and bone diseases are major causes of morbidity worldwide. While glucosamine sulfate (GS) and chondroitin sulfate (CS) have demonstrated benefits in severe degenerative joint conditions, real-world evidence on the use patterns and reported outcomes of their combination with calcium (GCC) among individuals with mild-to-moderate joint discomfort remains limited. METHODS: This real-world study enrolled 9,090 adults who used GCC for ≥1 month and completed an online questionnaire; pre- and post-use symptom scores were analyzed among participants reporting joint symptoms or back p

Thrombomodulin Shapes Thrombo-Inflammatory Interactions in Lupus Nephritis.

Lupus nephritis (LN) confers a disproportionate thrombotic risk in systemic lupus erythematosus (SLE) that is not fully explained by conventional cardiovascular risk factors. The underlying molecular mechanisms remain largely unknown. We aimed to characterize the thrombo-inflammatory profiles in LN compared with SLE without nephritis (SLE-NonLN), and contextualized these findings to primary antiphospholipid syndrome (APS), primary Sjögren's disease (SjD), and healthy controls. In a cross-sectional design, we included 325 patients with SLE from two Danish cohorts, 10 with primary APS, 45 with p

Risk factors for long-term analgesic prescription after hip or knee arthroplasty for osteoarthritis: results of a multivariable regression analysis based on health insurance claims data from 137,875 patients with postoperative inpatient rehabilitation.

BACKGROUND: Continued analgesic prescription after elective total hip arthroplasty (THA) or total knee arthroplasty (TKA) indicates persistent pain and increases the risk of adverse drug reactions, particularly in older patients or those with chronic conditions. The aim of this study was to identify independent risk factors for long-term analgesic prescription in patients who have undergone arthroplasty followed by inpatient orthopaedic rehabilitation. METHODS: The study is based on nationwide claims data from Germany's largest statutory health insurance provider (AOK) for the period from 202

Is There a Need for a Central Stem within the Keel of Primary Tibial Baseplates in Total Knee Arthroplasty.

Clinical studies suggest a role for the keel of the tibial component in preventing aseptic loosening in total knee arthroplasty (TKA). To analyze the role of a central stem within the keel, we compared two designs of tibial components for the Triathlon TKA (Stryker): standard keel components that have a central stem and fins (Triathlon universal tibial baseplate) and finned components that have fins alone and lack a central stem (Triathlon primary tibial baseplate). Questions: (1) is there a difference in all-cause revision rate or revision rates for loosening? (2) Is there a difference in rev

Effectiveness of early initiation of idursulfase in infants and very young children with mucopolysaccharidosis II: a retrospective chart review in siblings.

INTRODUCTION: Enzyme replacement therapy (ERT) with intravenous idursulfase is the standard of care for patients with mucopolysaccharidosis II (MPS II; Hunter syndrome). Data are limited for children aged ≤12 months. This retrospective study evaluated outcomes with early ERT. METHODS: Male siblings with MPS II treated with idursulfase for >2 years were categorized into five cohorts by age at treatment initiation. The primary objective compared Cohort 1 (ERT initiation aged ≤12 months) with Cohort 2 (ERT initiation at an age ≥ 12 months older). Clinical outcomes were compared using Kaplan-Meie

Additional medial patellotibial ligament reconstruction reduced lateral patellar translation compared with isolated medial patellofemoral ligament reconstruction without increasing patellofemoral contact pressure: a cadaveric study.

INTRODUCTION/OBJECTIVES: The medial patellotibial ligament (MPTL) is a secondary patellar stabilizer whose contribution increases with progressive knee flexion. Combined medial patellofemoral ligament (MPFL) and MPTL reconstruction is increasingly used clinically, but biomechanical data comparing it with isolated MPFL reconstruction for patellar translation and contact pressure are lacking. This cadaveric study compared these two techniques and simulated patellar instability, evaluating patellar translation and mean/peak patellofemoral contact pressure at 0°, 30°, 45°, 60°, and 90° of knee fle

Predictors and rate of satisfactory response to nonoperative management for nonarthritic hip-related pain.

BACKGROUND: Although surgery provides symptomatic relief for many patients with nonarthritic hip-related pain, not all patients are ideal surgical candidates, are interested in pursuing surgery, or require surgery for symptomatic relief. Nevertheless, evidence is limited regarding patients' satisfaction with nonoperative management. OBJECTIVE: To identify the rate of satisfactory response to nonoperative management for nonarthritic hip-related pain at 12-month follow-up. A secondary purpose was to identify possible predictors of response to nonoperative management. DESIGN: Pragmatic, prospec

Transradial Perilunate Fracture-Dislocation With Lunotriquetral Coalition.

Perilunate fracture-dislocations are devastating injuries, and their occurrence in the presence of a congenital carpal coalition is exceedingly rare. This combination fundamentally alters normal wrist biomechanics and significantly complicates surgical decision-making. We present a case of a 52-year-old female with acute hand ischemia following a high-energy motorcycle collision. Radiographic evaluation revealed a transradial perilunate fracture-dislocation complicated by a unilateral Minnaar type 3 congenital lunotriquetral coalition. Emergent closed reduction was performed, which immediately

The ITCC-P4 PDX Platform Enables Preclinical Testing of Pediatric Cancers.

UNLABELLED: Cancer is the leading cause of disease-related deaths among children in high-income countries. Tumor heterogeneity and the lack of mechanism of action-based therapeutic options are key challenges to overcome to improve the survival of pediatric patients with cancer. To address these challenges, we formed the European Union-Innovative Medicines Initiative 2 (EU-IMI2) funded public-private partnership "Innovative Therapies for Children with Cancer Paediatric Preclinical Proof-of-Concept Platform" (ITCC-P4), which built a large repertoire of patient-derived xenograft (PDX) models repr

Joint line obliquity change was associated with femoral and tibial resection patterns and smaller final flexion imbalance after robot-assisted total knee arthroplasty.

PURPOSE: Associations between preoperative-to-post-operative changes in arithmetic hip-knee-ankle angle (aHKA) and joint line obliquity (JLO) and final balance after robot-assisted total knee arthroplasty (TKA) were evaluated. Relationships with resection redistribution and tibial-varus reduction were also examined. METHODS: A retrospective single-surgeon series of 402 primary robot-assisted TKAs was analysed. Preoperative and post-operative aHKA, JLO and coronal plane alignment of the knee (CPAK) type were determined from full-length radiographs. Final extension and flexion imbalance and lax

Establishing Minimal Clinically Important Difference and Substantial Clinical Benefit Thresholds for Objective Gait Metrics After Total Knee Arthroplasty.

BACKGROUND: Assessment of objective gait recovery in patients who are undergoing total knee arthroplasty (TKA) is becoming increasingly prevalent. However, few studies determined minimal clinically important difference (MCID) or substantial clinical benefit (SCB) thresholds in arthroplasty populations for clinical interpretation. This analysis aimed to calculate these values for various gait metrics following TKA. METHODS: A secondary analysis of a longitudinal observational cohort of patients who underwent primary TKA was performed. Objective gait metrics were collected via wearables and sma

Partial triceps tendon transfer for dynamic stabilization in posterolateral rotatory instability of the elbow: a biomechanical study.

PURPOSE: Posterolateral rotatory instability (PLRI) of the elbow results from lateral collateral ligament complex (LCLC) insufficiency. In the setting of failed conventional ligament reconstruction, no established salvage procedure is available. This study biomechanically evaluated partial triceps tendon transfer to the supinator crest as a dynamic stabilization concept for PLRI. METHODS: Nine fresh-frozen human elbow specimens underwent cyclic external rotational loading of 0.5 Nm. Specimens were tested sequentially in the native state, an LCLC/common extensor origin-deficient unstable state

Iron and Other Metal Ions in Human Health and Disease.

Iron is the most abundant trace element, driving both the commonest deficiency and ferroptosis. This review takes iron as the central theme and extends to copper, zinc, and calcium, spanning physiological functions, pathological mechanisms, risk factors, diagnostics, and therapeutics. We classify these ions into redox-active (iron, copper), which engage in catalytic electron transfer, and redox-inert (zinc, calcium), which serve structural and signaling roles without redox cycling. Iron deficiency remains the global leading micronutrient deficit, whereas iron overload triggers ferroptosis-medi

Re-examining the pain-depression vicious cycle in knee osteoarthritis: a random-intercept cross-lagged panel analysis.

BACKGROUND: Pain and depression frequently co-occur in knee osteoarthritis (OA), traditionally viewed as a vicious cycle in which each worsens the other over time. However, conventional cross-lagged models cannot separate stable differences between individuals from real year-to-year changes within a person. We therefore used a random-intercept cross-lagged panel model (RI-CLPM) in the Osteoarthritis Initiative (OAI). METHODS: Of 4,796 enrolled OAI participants with, or at risk of, knee OA, 4,638 contributed data across four annual waves from months 12 to 48. The RI-CLPM was applied to knee pa

Focused Extracorporeal Shock Wave Therapy Versus Therapeutic Ultrasound as Adjuncts to Exercise-Based Rehabilitation for Functional Limitation in Ankylosing Spondylitis: An Assessor-Blinded Randomized Comparative Trial.

BACKGROUND: The comparative effectiveness of adjunctive physical modalities alongside exercise for ankylosing spondylitis (AS) remains inadequately evaluated. OBJECTIVE: To compare the relative association of focused extracorporeal shock wave therapy (ESWT) versus therapeutic ultrasound, both added to identical exercise-based rehabilitation, with long-term functional outcomes in AS. METHODS: This assessor-blinded randomized comparative trial (conducted March 2024-August 2025) evaluated 106 AS patients with chronic lumbar stiffness (90 completed 12-month follow-up). Bilateral Keratouveitis and Secondary Glaucoma in a Young Dog With Immune-Mediated Polyarthritis and Systemic Inflammatory Response Syndrome.

A 7-month-old intact female mixed-breed dog presented emergently for inability to walk, lethargy, and vomiting. The patient was diagnosed with immune-mediated polyarthritis (IMPA) and secondary systemic inflammatory response syndrome (SIRS). Bilateral keratouveitis and secondary glaucoma developed within 24 h of presentation. Thorough diagnostic testing failed to identify an underlying etiology. Despite aggressive medical management, the patient developed rapid vision loss. Due to poor visual prognosis and anticipated need for long-term medical management, both eyes were enucleated.
