A case of autonomic neuropathy and hypertrophic osteoarthropathy in association with metastatic giant cell carcinoma of the lung and leiomyosarcoma of the stomach is reported. The link between these conditions is discussed and the role of pancreatic polypeptide as a possible marker for hypertrophic osteoarthropathy reported.

We present clinical findings in infants from three kindreds (two Hutterite and one Mennonite) with an apparently unique, fatal disorder. The major manifestations consist of severe intrauterine growth retardation, congenital contractures, and tense skin which is easily eroded. The skin is tightly drawn over the face, giving an abnormal appearance consisting of a narrow, pinched nose, small mouth, limited jaw mobility, and ectropion (in one). One infant had first-degree hypospadias. Apart from this, there were no organ malformations and the infants did not have hydrops. Histologically, the skin

Gastrointestinal amyloid deposition was investigated in 21 autopsy cases of nonhereditary systemic amyloidosis, 18 of the AL (primary or myeloma-associated) type and three of the AA (secondary) type. Vascular deposition of amyloid, most apparent in the submucosa, was found in all cases. Parenchymal deposition was observed mainly in the muscularis mucosae and muscularis externa in the AL type, and in the lamina propria mucosae in the AA type. Comparison of amyloid deposition in the stomach and rectum revealed no differences for the AA type. In the AL type, however, deposition in the lamina prop

Limb abnormalities in 30 calves with an inherited congenital neuromuscular disorder known as syndrome of arthrogryposis and palatoschisis were classified according to the range of severity of joint deformity in either flexion or extension, and restricted joint movement. Joint movement was variably affected; it was either normal, lax, restricted in the range of mobility, or occasionally, fixed. The characteristic findings were: bilateral hyperextension of the hind fetlock, flexion deformity of the forelimb that particularly involved the fetlock and the carpus, with restricted articular movement

The efficacy of magnetic resonance imaging (MRI) in the assessment of pediatric hip disease was tested by scanning the hips of 24 children (30 scans). Twelve patients with Legg-Calvé-Perthes disease (17 hips) showed characteristic areas of low-intensity signal representative of necrotic areas of the capital epiphysis. Abnormal scans were also obtained on patients with transient synovitis, avascular necrosis secondary to steroids, epiphyseal dysplasia, and multiple osteochondromatosis. MRI accurately shows articular cartilage, femoral head shape, quality of containment, and areas of necrosis in

This communication has attempted to review causal processes in polyarticular osteoarthritis (poly-OA). Since mechanical features are an almost constant feature of OA joint deterioration, their interplay, both in static and dynamic form, has been stressed. Age-related polyarticular cartilage softening is expounded as an example of degeneration and atrophy--a process not to be confused with OA which is considered as an age-accelerated disorder. I have stressed the influence of genetic factors, best exemplified as a single gene aberration in the occurrence of Heberden's nodes, while a polygenetic

Osteoarthrosis occurs in all population groups throughout the world. However, the pattern of the condition may differ considerably amongst various developing, traditionally living people, perhaps because of their different living habits, occupational factors and genetics. The distribution and prevalence of osteoarthrosis in a developing African population are reported in this paper. A radiological survey of the hands and feet of 543 adult respondents was undertaken. The prevalence of multiple osteoarthrosis and Heberden's nodes was very low. There was a sharp rise in the prevalence of osteoart

Patients with Sjögren's syndrome (SS) in association with either Rheumatoid Arthritis (RA), Systemic Lupus Erythematosus (SLE) or Sclerodactyly were tested for HLA-A, B and Dr antigens and the phenotypes of the 4th component of complement (CA). Significant associations were identified between HLA-Bw62, Dr4, C4B2.9, C4A8 and females with SS-RA. The MHC antigen frequencies noted for secondary Sjögren's syndrome differed from those reported for their respective primary diseases and the implications of these observations are discussed.

Although some cases of spoon nails and clubbing are manifestations of internal disease, a significant number are completely idiopathic and of no consequence to the patient. Many patients have been forced to pay high insurance premiums or have been denied employment because of insignificant nail changes. When a patient has spoon nails or clubbing, a thorough but reasonable search for a primary etiology should be made. It should be recalled that when the complete syndrome of hypertrophic osteoarthropathy is present and joint pain is prominent, the possibility of a bronchogenic carcinoma is great

A 56-year-old diabetic woman developed multiple cutaneous lesions and septic arthritis secondary to disseminated Nocardia asteroides pulmonary infection. The diagnosis was made rapidly by Gram's and acid-fast stains of pus, sputum, and synovial fluid and later confirmed by cultures. Sulfonamides are the drugs of choice for nocardiosis, and this patient had a complete response to two weeks of intravenous trimethoprim and sulfamethoxasole followed by three months of oral therapy.

Five patients with radial aplasia studied during a period of ten years at the University College Hospital, Ibadan, Nigeria, had the amegakaryocytic thrombocytopenia syndrome. Together, they are termed the TAR syndrome. Two of the five patients were siblings from a polygamous family; three patients had relative hypogammaglobulinemia, and one of these patients had late onset of congenital varicella. The findings in these five cases led to further understanding of the genetic mechanisms of the TAR syndrome. Furthermore, the high incidence of infection associated with the syndrome could be partial

Thirty-three Girdlestone's arthroplasties used to salvage infected total hip replacements were followed for a mean of 6.2 years to assess the efficacy of this procedure. Subjective and objective assessment was obtained at the time of review. Gram-negative organisms, retained cement, bony sequestra and secondary healing were common features of the five patients (15%) whose wounds discharged for more than four weeks after operation. Girdlestone's arthroplasty provided satisfactory pain relief in 91% of patients, and control of infection in 97%. Although leg-length discrepancy (mean 4 cm) and a T

Using GLC, multiple adrenal corticosteroid urinary metabolites, including androgenic-anabolic, glucocorticoid, pregnanediol, and pregnanetriol, were measured in eight ambulatory female RA patients and eight matched normal control subjects on baseline, ACTH-, and metyrapone-stimulation days under carefully monitored clinical research center protocol. Neither group had been treated previously with any steroid hormones. The 11-deoxy-17-KS metabolites, derived from adrenal androgenic-anabolic steroids, and comprising androsterone, etiocholanolone, and DHA, were significantly lower in RA patients o

We report two cases of osteomyelitis secondary to staphylococcal septicaemia, presenting with large sterile effusions in adjacent joints. In both cases the clinical features were suggestive of septic arthritis. Multiple joints were involved in each patient. Initial radiographs showed no abnormalities.

The relationships of HLA-DR and the newer DS (second D locus) B cell alloantigens (MB and MT) to the clinical and serologic expression of primary and secondary forms of Sjögren's syndrome (SS) were examined in 102 patients (86 whites and 16 blacks). Although HLA-DR3 was significantly increased in whites (25 of 50, 50%) and blacks (4 of 5, 80%) with primary SS compared with race-matched normal controls, it was not appreciably elevated in those with systemic lupus erythematosus (SLE)-SS, rheumatoid arthritis (RA)-SS, or connective tissue disease-SS. The MT2 specificity, however, was more strongl

Synovial osteochondromatosis is a benign lesion characterized by multiple cartilaginous and osteocartilaginous nodules of the synovium, as well as loose bodies within the joint. Present theories of origin include spontaneous metaplastic changes of the synovial cells or bursal tissues, which form foci of cartilage. The cartilage detaches, enters the joint cavity, and ultimately becomes calcified and ossified. Several authors have suggested trauma to the synovial tissues as being causative, but this has never been statistically confirmed. Others have even mentioned the spontaneous metablastic ch

We reviewed the cases of eighty-one patients with pigmented villonodular synovitis and giant-cell tumor of the tendon sheath. The average duration of symptoms prior to consulting a physician was twenty-seven months. The lesion was slow-growing, was located in one joint or tendon sheath, and usually was asymptomatic in a finger or thumb but painful in a knee or toe. Twenty-nine per cent of the finger or thumb lesions and 21 per cent of the knee lesions that were followed recurred. Approximately 70 per cent of the radiographs showed abnormalities that ranged from soft-tissue swelling to bone ero

Conventional chest roentgenograms provide an opportunity to study changes in the thoracic cage produced by systemic or localized disorders. Careful evaluation of abnormalities often gives a clue to the diagnosis of clinically unsuspected disorders. In this article, the authors present a review of developmental, granulomatous, collagen vascular, hematopoietic, metabolic, endocrine, and neoplastic disorders affecting the thoracic cage.

Erythema chronicum migrans (ECM) is the distinctive cutaneous lesion of the multisystem tick-borne spirochetosis, Lyme disease. Its clinical and histologic pictures are variable. Of the 51 patients with ECM of Lyme disease, 38 patients (75%) had single lesions, 13 patients (25%) had multiple lesions, and 32 patients (62%) had extracutaneous signs and symptoms. Only 15 patients (29%) had symptoms referable to ECM. Extracutaneous signs and symptoms were absent in 12 (80%) of the 15 patients. These patients had uneventful recoveries following treatment with systemic antibiotics. Thirty-four skin
