Increased occurrence of Heberden nodes has been observed in judo practitioners. Nine out of 30 members of the Swiss national team showed such nodes clinically on several fingers, whereas sportsmen in other disciplines did not. X-ray showed that all judokas examined have more or less severe osteoarthrosis of the distal interphalangeal joints (DIP), whether Heberden nodes were present or not. X-rays of the shoulder, hand, and knee joints from 30 patients with pyrophosphate arthropathy (PA) and 30 age and sex matched control subjects were examined for periarticular, dense, homogeneous calcifications considered to be apatite deposits. They were found in 30% of the patients with PA compared with 3.3% of the controls. In addition in the PA group the incidence of Heberden's nodes was significantly increased in cases with periarticular calcifications, implying that mixed (calcium pyrophosphate dihydrate + apatite) crystal deposition disease could be related to nodal osteoarthrosis.

A fixed post-traumatic flexion contracture of a finger is usually secondary to multiple previous operations. We have observed that a former flexor tendon laceration is not constant and is missing in 18% of our cases. The flexor tendons are, nevertheless, always involved in the contracture. A volar skin contracture was present in all cases, but only in half of them was noted a retraction of the volar components of the PIP joint. A unique case of pachydermoperiostosis, multiple epiphyseal dysplasia, and secondary osteoarthritis is described. An additional interesting feature was the presence of acroosteolysis of the distal phalanges of fingers and toes. Scintigraphic, thermographic, plethysmographic, and capillaroscopic studies suggested increased blood flow through clubbed fingers, raising the possibility that this may play a role in the pathogenesis of pachydermoperiostosis.

Natural killing (NK) by peripheral blood mononuclear cells (PBMC) against K562 cells was examined in 27 patients with Sjögren's syndrome and 17 normal controls. Fourteen patients with cauda equina syndrome secondary to long-standing ankylosing spondylitis are described. Three children with idiopathic hypertrophic osteoarthropathy and cranial suture defects are reported. The syndrome was recognized after birth and in the two oldest siblings, the cranial defects and subperiosteal bone formation resolved almost completely by age 4 and 6 years. The joint swelling and clubbing persisted and mild bone reabsorption of the distal phalanges became apparent at an older age. Two siblings and both parents had normal bone X-rays and no clubbing. This study confirms the association of cranial sutural defects and familial idiopathic hypertrophic osteoarthropathy.

Pressure sore are caused by multiple mechanisms. Preventive management requires relief of pressure, skin care, control of spasms, release of contractures and treatment of infection. Conservative treatment succeeds only when pressure on the ulcer site is relieved. Surgical treatment involves primary, secondary and tertiary procedures. Benefits of surgical closure include avoidance of protein loss, prevention of osteomyelitis and other infections, improved appearance and prevention of carcinoma and amyloidosis. A coordinated team approach, incorporating patient education, is effective.

A series of 18 patients with odontoid fractures due to metastatic cancer were treated at Memorial Sloan-Kettering Cancer Center between 1974--1980. The primary source of cancer was breast (12 cases), lung (two cases), nasopharynx (one case), multiple myeloma (one case), colon (one case), and rhabdomyosarcoma (one case).
