Urine specimens from 50 patients with various renal diseases and from 10 normal subjects were examined for the presence of low molecular mass (7S) IgM by double diffusion, using a specific anti-7S IgM serum. 7S IgM in urine was also quantitated by radial immunodiffusion in 5% polyacrylamide gel. Multiple myeloma developed in a woman with a 7-year history of seropositive rheumatoid arthritis (RA). An unexpected finding was the presence of massive amyloid deposits in the left femur and traces of amyloid in the synovium. Detailed histopathologic studies, however, conclusively excluded amyloid arthropathy. Although definitive tissue classification was not possible, this patient in all likelihood had 2 types of amyloid: massive bone lesions of myeloma-associated (AL) amyloid, and focal subsynovial deposits of RA-associated secondary (AA) amyloid.

Heberden's nodes lie close to the matrix of the nails and may impinge on them causing nail changes. This occurs particularly when the nodes become inflamed, cystic or grow distally from the joint. We describe three patients with a complex syndrome of apparent arthromyodysplasia, dyscephaly, sacral agenesis, and hypoplastic digitis. A prospective series of 165 displaced subcapital fractures of the femoral neck treated with reduction and percutaneous multiple pinning is presented. The method offers the advantages of a short operating time, no loss of blood, immediate weight-bearing and no infection problems. This is a report of 2 rare, neglected congenital irreducible complete lateral dislocations of the patella. The patients were boys, 10 and 13 years old. The dislocations were designated irreducible because closed manipulative effects were invariably failures and the dislocations are permanent because they persist from birth until openly reduced. A 46-year-old man presented signs of secondary syphilis, including a skin eruption, multiple adenopathies, and alopecia. The diagnosis was confirmed by positive serological and Nelson's tests. Inflammatory arthritic signs developed in a few joints two weeks after the onset of the skin eruption, affecting mainly the left wrist, right sternoclavicular joint, lumbar spine, and right shoulder and hip joints, which were painful and stiff. 45 patients wih degenerative joint diseases (osteoarthrosis of the hip or knee, Heberden's nodes) were treated for 5 weeks in an open controlled trial with Sulindac, a new nonsteroidal antirheumatic drug (300-400 mg daily). The effectiveness of the drug was evaluated by the following criteria: changes of pain intensity with active movements, pain at rest, night pain, duration of morning stiffness and limitation of movement. Intra-articular loose bodies are usually secondary to osteochondral fractures, osteochondritis dissecans, synovial osteochondromatosis, and degenerative arthritis. The authors report four patients with longstanding rheumatoid arthritis and multiple loose bodies in a variety of joints. All patients have remained active despite their disease, suggesting that the loose body production was secondary to trauma of continued activity superimposed on joints containing pannus and eroded cartilage.

Twenty-eight children were treated who had limited flexion of the hips and various degrees of contracture of the abductor and external rotator muscles because of fibrosis of the gluteus maximus muscle. Although the lesions could be classified as those associated with poliomyelitis, infection of the gluteus maximus muscle, and fibrosis of unknown etiology, all forty-five hips had a typical restriction of motion such that an affected hip could not be flexed in the usual sagittal plane, but had to be flexed in abduction. The postoperative results and complications among 41 patients undergoing bilateral total knee arthroplasty were compared with 40 patients who underwent unilateral total knee replacement. Among the bilateral group, the most common diagnosis was rheumatoid arthritis (68%), while degenerative arthritis prevailed in the unilateral group (50%). A 55-year-old black female with ankylosing spondylitis (AS) is described. The patient had a severe flexion attitude secondary to a rotational subluxation at the sacroiliac joints with subsequent bony ankylosis. The sacroiliac abnormality has not been reported in AS patients. Sacroiliac joint laxity during multiple pregnancies might have contributed to the subluxation. The importance of this anatomic site in evaluating the surgical correction of the postural deformities of AS is stressed.

The clinical features of osteoarthritis provide little support for the traditional view of the disease as a wear-and-tear phenomenon of aging. In many cases osteoarthritis consists of a polyarthritis with mildly inflammatory features, such as stiffness, effusions, and hot Heberden's nodes. The finding of biochemical abnormalities of cartilage throws new light on this enormously important disease and suggests the possibility of more effective therapy.
