Epidemiological characteristics in Shanghai and a prognostic nomogram for 90 day adverse outcomes in acute hepatitis E.

BACKGROUND & AIMS: Hepatitis E virus (HEV) represents a leading cause of acute viral hepatitis in China, yet large-scale studies characterizing its dynamic epidemiology and enabling early prediction of adverse outcomes remain scarce. METHODS: We performed a retrospective cohort study comprising 816 patients hospitalized with acute HEV infection (2019-2024). Predictors were identified through multivariable logistic regression, with Firth's penalized likelihood method applied to address potential small-sample bias, and bootstrap resampling (BCa 95% CI) used to validate estimate robustness. Metabolic factor-based machine learning model for mortality prediction in acute hepatitis E: Development and validation from a dual-center cohort.

BACKGROUND: Hepatitis E virus (HEV) infection remains a major cause of liver failure with high short-term mortality, yet predictive models incorporating systemic metabolic factors are limited. AIMS: We aimed to develop a machine learning model incorporating systemic metabolic parameters for mortality prediction in HEV patients. METHODS: A total of 510 HEV patients from two medical centers were retrospectively enrolled and grouped into training, internal validation and external validation cohorts. Metabolic parameters (total cholesterol, HDL, LDL and diabetes) were integrated using SVM to gen

Prolonged Intrahepatic Cholestasis After Acute Hepatitis E Infection: A Case Series and Genetic Analysis.

Hepatitis E virus (HEV) is a leading cause of acute viral hepatitis worldwide. Whereas HEV infection is typically self-limiting, rare cases of prolonged cholestasis have been reported. The underlying mechanisms remain unclear, though host genetic variation may contribute. This study aimed to investigate the role of genetic predisposition in HEV-induced prolonged cholestasis by analysing variants in genes associated with hepatocanalicular transport. We performed a retrospective review of medical records from three university centres in Switzerland and Germany and identified five immunocompetent

Guillain-Barré syndrome triggered by acute hepatitis E virus infection: The first reported case from Egypt.

A man in his early 50 s presented with a 7-day history of progressive, symmetrical weakness that began in the lower limbs and ascended to involve the upper limbs. His symptoms were accompanied by distal paresthesia and complete areflexia. Three weeks prior to presentation, he experienced a self-limiting febrile illness associated with dark urine and elevated transaminase levels, which resolved spontaneously. On admission, laboratory investigations demonstrated elevated liver enzyme levels and positive anti-hepatitis E virus Immunoglobulin M antibodies, confirming acute hepatitis E virus infect

Acute hepatitis E in older adults: global trends from the global burden of disease study.

BACKGROUND AND AIMS: Hepatitis E virus (HEV) infection poses an increasing threat to the health of older adults, yet global epidemiological data on this susceptible population remain limited. This study aims to evaluate long-term trends in the incidence and mortality of acute hepatitis E (AHE) among individuals aged 60 years and older, with a focus on demographic, regional, and socioeconomic disparities. METHODS: In this study, we used GBD data from 2021 to assess the incidence and mortality rates associated with the HEV in older populations. We quantified incidence rates, determined average a

Global Temporal Trends and Projections of Acute Hepatitis E Epidemiology for Adults 65 Years and Older from 1990 to 2021: Global Burden of Disease 2021 Based Study.

BACKGROUND: Acute hepatitis E (AHE) poses escalating risks to older adults (≥65 years), compounded by immunosenescence and comorbidities. Using Global Burden of Disease (GBD) 2021 data, this study analyzes global AHE burden, trends, and projections in aging populations. METHODS: Age-standardized rates (ASIR, ASMR, ASDR) for AHE in adults ≥ 65 years were extracted from GBD 2021 across 204 countries (1990-2021). Frontier analysis assessed gaps between observed burdens and sociodemographic index (SDI)-based theoretical minima. Age-period-cohort (APC) modeling evaluated age/period/cohort effects.

Unmasking the Threat: Acute Hepatitis E Virus Infection in a Liver Transplant Recipient.

Hepatitis E virus (HEV) infection is an increasingly recognized cause of acute and chronic hepatitis in immunosuppressed individuals, including solid organ transplant recipients. Although typically self-limited in immunocompetent hosts, HEV infection in transplant recipients may lead to persistent viremia, accelerated fibrosis, and graft dysfunction. We report the case of a 47-year-old man who underwent deceased donor liver transplantation for decompensated metabolic-associated steatohepatitis cirrhosis. Five months post-transplant, he was found to have rising liver enzymes during routine moni

Longitudinal Follow-Up of microRNA Expression During Hepatitis E Virus Infection in Immunocompromised Hosts.

Hepatitis E virus (HEV) is a zoonotic cause of acute viral hepatitis that can become chronic in immunocompromised patients. Recently, host microRNAs (miRNAs) miR-122-5p, miR-125b-5p, miR-99a-5p, and miR-192-5p have been associated with acute HEV infection and with chronic HEV infection. Yet, the expression levels of miRNAs during the early phase of HEV infection remained unexplored. In this context, we aimed to identify changes in miRNA profiles occurring between the diagnosis of HEV infection and HEV cure in immunocompromised patients and in a pig model. We retrospectively included 9 immunoco

Intra-host viral population dynamics during acute hepatitis E virus infection.

The hepatitis E virus (HEV) is the most common cause of acute viral hepatitis worldwide. RNA viruses like HEV can establish viral populations with high intra-host variability, enabling them to rapidly adapt to changing immune responses, as observed in chronic infections. This study aimed to investigate how intra-host evolution shapes HEV populations during acute asymptomatic infection. Using a highly sensitive HEV amplicon sequencing approach, we characterized intra-host viral diversity and mutational signatures present in serum samples from a cohort of over 80 asymptomatic blood donors infect

Sequential Immune-Mediated Extrahepatic Complications of Hepatitis E Virus: A Case Report.

Hepatitis E virus (HEV) infection is classically described as causing self-limiting acute hepatitis. However, it is increasingly being recognised for a wide spectrum of immune-mediated extrahepatic complications involving the haematological, neurological, renal and pancreatic systems. These manifestations are more commonly seen as isolated presentations in individual patients. We report a 59-year-old man with serologically confirmed acute hepatitis E, in whom alternative infectious, autoimmune, and malignant etiologies were excluded, who developed three distinct immune-mediated complications i

National trends in acute viral hepatitis in China from 1990-2023: An analysis of the Global Burden of Disease 2023 update.

BACKGROUND: The prerequisite for achieving the goal of the World Health Organization to completely eliminate viral hepatitis by 2030 is China's accurate understanding of the current disease burden, thereby providing a basis for formulating and optimizing intervention measures. METHODS: Based on the Global Burden of Disease (GBD) 2023 dataset, we extracted data on acute hepatitis A, B, C, and E in China, including incidence, mortality, disability-adjusted life years (DALYs), and their corresponding age-standardized rates (ASRs). To account for differences in age and sex distributions, we calcu

Long-Term Neurological Manifestations in HEV-Infected Immigrant Patients: A 11-year Follow-up Study on Meningo-Encephalomyelitis.

This letter reports long-term (6-11 years) follow-up of a series with acute Hepatitis E virus (HEV) infection, highlighting its neurotropic potential in immunocompetent individuals. Among 20 individuals with documented viremia, 15% developed delayed meningo-encephalomyelitis, despite no acute neurological signs. HEV RNA detection in cerebrospinal fluid supports the role of active viral replication in central nervous system invasion. Both genotype 3 and genotype 1 were implicated. Two patients later developed neoplastic diseases in lung and pancreas. Although limited by small sample size, findi

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. Immunogenicity and safety of HepE Hecolin® in chronic hepatitis B patients at clinically stable stage: An open-label study in China.

Acute hepatitis E infection could induce severe outcomes among chronic hepatitis B (CHB) patients. Between 2016 and 2017, an open-label study was conducted to evaluate the immunogenicity and safety of hepatitis E vaccine (HepE) in CHB patients, using healthy adults as parallel controls in China. Eligible participants who were aged ≥30 y were enrolled in the study. The CHB group included participants who had ever developed symptoms of hepatitis because of CHB but was currently at a clinically stable stage, which was defined as ALT ≤ 1.5 times of upper limit of the normal range (ULN) in this stu
