A systematic literature review to inform the 2025 EULAR recommendations for the management of polymyalgia rheumatica and primary large vessel vasculitis: management of disease including relapse and complications.

OBJECTIVES: EULAR recommendations for polymyalgia rheumatica (PMR) and large vessel vasculitis (LVV) were published in 2015 and 2020, respectively. New evidence necessitated an update. We conducted a systematic literature review (SLR) to inform the 2025 EULAR recommendations for PMR and LVV. METHODS: An SLR was conducted on MEDLINE, EMBASE, and CENTRAL on adults with PMR, giant cell arteritis (GCA), or Takayasu arteritis (TAK), using Medical Subject Heading terms related to management strategies on 31 December 2024 (PROSPERO: CRD42024560403). Eligible studies were randomised controlled trials

Thrombomodulin Shapes Thrombo-Inflammatory Interactions in Lupus Nephritis.

Lupus nephritis (LN) confers a disproportionate thrombotic risk in systemic lupus erythematosus (SLE) that is not fully explained by conventional cardiovascular risk factors. The underlying molecular mechanisms remain largely unknown. We aimed to characterize the thrombo-inflammatory profiles in LN compared with SLE without nephritis (SLE-NonLN), and contextualized these findings to primary antiphospholipid syndrome (APS), primary Sjögren's disease (SjD), and healthy controls. In a cross-sectional design, we included 325 patients with SLE from two Danish cohorts, 10 with primary APS, 45 with p

Establishing Minimal Clinically Important Difference and Substantial Clinical Benefit Thresholds for Objective Gait Metrics After Total Knee Arthroplasty.

BACKGROUND: Assessment of objective gait recovery in patients who are undergoing total knee arthroplasty (TKA) is becoming increasingly prevalent. However, few studies determined minimal clinically important difference (MCID) or substantial clinical benefit (SCB) thresholds in arthroplasty populations for clinical interpretation. This analysis aimed to calculate these values for various gait metrics following TKA. METHODS: A secondary analysis of a longitudinal observational cohort of patients who underwent primary TKA was performed. Objective gait metrics were collected via wearables and sma

Re-examining the pain-depression vicious cycle in knee osteoarthritis: a random-intercept cross-lagged panel analysis.

BACKGROUND: Pain and depression frequently co-occur in knee osteoarthritis (OA), traditionally viewed as a vicious cycle in which each worsens the other over time. However, conventional cross-lagged models cannot separate stable differences between individuals from real year-to-year changes within a person. We therefore used a random-intercept cross-lagged panel model (RI-CLPM) in the Osteoarthritis Initiative (OAI). METHODS: Of 4,796 enrolled OAI participants with, or at risk of, knee OA, 4,638 contributed data across four annual waves from months 12 to 48. The RI-CLPM was applied to knee pa

Focused Extracorporeal Shock Wave Therapy Versus Therapeutic Ultrasound as Adjuncts to Exercise-Based Rehabilitation for Functional Limitation in Ankylosing Spondylitis: An Assessor-Blinded Randomized Comparative Trial.

BACKGROUND: The comparative effectiveness of adjunctive physical modalities alongside exercise for ankylosing spondylitis (AS) remains inadequately evaluated. OBJECTIVE: To compare the relative association of focused extracorporeal shock wave therapy (ESWT) versus therapeutic ultrasound, both added to identical exercise-based rehabilitation, with long-term functional outcomes in AS. METHODS: This assessor-blinded randomized comparative trial (conducted March 2024-August 2025) evaluated 106 AS patients with chronic lumbar stiffness (90 completed 12-month follow-up). Participants received eith

Deep-ZOMA: A Deep Learning-Based Approach for Automated Morphometric Analysis of Zebrafish Larvae Ocular Structures.

IMPORTANCE: Urinary tract infection (UTI) is the second most common infection leading to hospitalization and is often associated with gram-negative multidrug-resistant organisms (MDROs). Clinicians overuse extended-spectrum antibiotics although most patients are at low risk for MDRO infection. Safe strategies to limit overuse of empiric antibiotics are needed. OBJECTIVE: To evaluate whether computerized provider order entry (CPOE) prompts providing patient- and pathogen-specific MDRO risk estimates could reduce use of empiric extended-spectrum antibiotics for treatment of UTI. DESIGN, SETTIN

Management and prevention strategies for osteoarthritis in tactical athletes.

Osteoarthritis (OA) affects over 600 million worldwide, is one of the leading causes of disability and has a significant burden of morbidity. There are multiple modifiable and non-modifiable risk factors, with professional and tactical athletes at higher risk than other occupational groups. Without specific anti-OA pharmacological agents, clinicians may feel helpless. However, primary, secondary and tertiary preventative strategies can slow or prevent OA development or progression. There are many modifiable risk factors which, if targeted, can contribute to an improvement in the experience of

Synovial Fluid Leukocyte Count and Differential Are Poor Standalone Rule-in Tests for Periprosthetic Joint Infection: Results of a Methodological Audit and Reanalysis of the 2025 Meta-analysis Underpinning the Unified Periprosthetic Joint Infection Criteria.

BACKGROUND: The Unified Periprosthetic Joint Infection (PJI) Criteria were developed and endorsed by the European Society of Clinical Microbiology and Infectious Diseases, the European Bone and Joint Infection Society, the Infectious Diseases Society of America, and the Musculoskeletal Infection Society and formally presented at the 2025 International Consensus Meeting. These criteria recommended that synovial fluid white blood cells (WBC) > 3000 cells/µL or polymorphonuclear cells (PMN) > 75% each should be considered individually sufficient to confirm a diagnosis of PJI as standalone rule-in

High MDR1 expression in RA is associated with increased MMP-3 levels and use of bDMARDs: potential independence of JAK inhibitors from MDR1.

OBJECTIVES: To identify patient-related factors linked to high multidrug resistance gene 1 (MDR1) expression in RA and assess whether MDR1 expression affects Janus kinase (JAK) inhibitor effectiveness. METHODS: Synovial tissues were obtained from 53 RA patients undergoing total knee arthroplasty between October 2020 and June 2024, with 41 included in the primary analysis after exclusion. MDR1 expression was measured by reverse transcription-PCR (RT-PCR). Correlations with clinical characteristics were evaluated using Pearson's correlation and multiple regression analyses. Fusion of Tumor Cells with Lipid-Associated Macrophages Drives Metastatic Progression of Breast Cancer.

UNLABELLED: Intratumoral heterogeneity supports cancer progression and metastasis while limiting therapeutic efficacy. Comprehensive biological characterization of heterogeneous subclones in breast cancer may hold promise in battling against this deadly disease. Using single-cell transcriptomic analysis and in situ profiling of primary tumors, metastases, and circulating tumor cells from multiple patients with breast cancer, we identified a unique EPCAM+ CD68+ TREM2+ tumor subpopulation, likely resulting from the fusion of tumor cells and lipid-associated macrophages (LAM). The presence of the

Comparison of cancer risks associated with JAK inhibitors and TNF inhibitors treatment in patients with rheumatoid arthritis: a systematic review and meta-analysis of real-world cohort studies.

INTRODUCTION: Potential increased cancer risk associated with janus kinase inhibitors (JAKi) compared with anti-tumor necrosis factor inhibitors (TNFi) in patients with rheumatoid arthritis (RA) remains a concern. Published cohort studies have reported conflicting results, and the discrepancies between randomized trials and real-world data remain unclear. We conducted this systematic review and meta-analysis to assess this association. MATERIAL/METHODS: We systematically searched PubMed, Embase and Cochrane Library for cohort studies up to January 31, 2025, comparing JAKi with TNFi and report

How Effective Are Constrained Liners at Preventing Dislocation After Revision Total Hip Arthroplasty?

BACKGROUND: Recurrent instability continues to challenge arthroplasty surgeons, and constrained liners may be utilized for cases of recurrent or multidirectional instability following revision total hip arthroplasty (rTHA). The purpose of this study was to evaluate dislocation rates and survivorship after rTHA using constrained liner (CL) prostheses. METHODS: Patients who underwent rTHA using a CL prosthesis were retrospectively reviewed from January 1, 2013, to December 31, 2024. Patients who had antibiotic spacers or megaprostheses, or those who had less than one year of follow-up, were exc

Full spatio-temporal analyses of migration and colonization in evolution-dense 3D mapping of cancer metastases provides new insights.

The process of migration and colonization is important in evolution; for example, modern humans experienced multiple waves of migrations out of Africa. However, no data cover the spatio-temporal patterns sufficiently to be truly informative. Metastatic cancer provides a unique in vivo model to study these processes through rapid somatic evolution. Here, we apply the high-resolution sampling technique (Dense 3D Crypt-scale Sampling) to analyze hundreds of spatially mapped micro-samples from the primary colorectal cancer and liver metastases in two representative cases. This would be analogous t

Patellar management in primary total knee arthroplasty: A narrative review and evidence-based framework for clinical decision-making.

Patellar management in primary total knee arthroplasty (TKA) remains one of the most debated and variable aspects of contemporary arthroplasty practice. Patellar resurfacing, non-resurfacing, and selective resurfacing each offer distinct advantages, but no universally superior strategy has emerged. Although global functional outcomes are generally comparable across techniques, differences persist in anterior knee pain, survivorship, patellofemoral degeneration, and the likelihood of secondary patellar procedures. This narrative review synthesizes current clinical, radiographic, biomechanical,

Age-associated differences in XBB.1.5 trivalent booster vaccine-induced adaptive responses revealed by single-cell RNA sequencing.

Older adults remain highly vulnerable to severe SARS-CoV-2 outcomes despite multiple vaccinations, yet age-associated differences in immune responses to updated COVID-19 booster vaccines remain incompletely characterized. Here, we administered an XBB.1.5 trivalent recombinant protein booster (WSK-V102C) to 22 individuals (<38 years) and 20 individuals (≥73 years), all of whom had previously received 2-3 doses of inactivated COVID-19 vaccines. Neutralizing antibody responses against multiple SARS-CoV-2 variants were quantified and compared between age groups. Meanwhile, single-cell RNA sequenci

Medial pivot designs result in improved patient reported outcome measures and range of motion when compared to cruciate retaining total knee replacements: A systematic review and meta-analysis.

BACKGROUND: Knee arthroplasty remains one of the most important treatment options in improving quality of life for patients with end-stage knee osteoarthritis. However, roughly 20% of patients remain dissatisfied with their outcome. Perceived implant instability and range of motion are factors that may contribute to dissatisfaction. The medial pivot (MP) total knee replacement (TKR) is postulated to provide increased stability due to greater implant conformity and replication of anatomical function compared to cruciate retaining (CR) implants. This systematic review and meta-analysis evaluates

Ixekizumab for the treatment of psoriatic arthritis: an Italian multicentric retrospective observational study.

Ixekizumab (IXE), an IL-17 A inhibitor, demonstrated efficacy in clinical trials in patients with psoriatic arthritis (PsA), and favorable data have emerged from real-world evidence studies on psoriasis as well. However, real-world data specific to PsA remain limited. This study aims to assess IXE effectiveness in reducing disease activity in patients with PsA and determine its drug retention rate (DRR) over 24 months. The secondary aim is to identify factors potentially affecting long-term persistence on therapy. A retrospective observational study was conducted. Consecutive adult patients me
