Age-associated differences in XBB.1.5 trivalent booster vaccine-induced adaptive responses revealed by single-cell RNA sequencing.

Older adults remain highly vulnerable to severe SARS-CoV-2 outcomes despite multiple vaccinations, yet age-associated differences in immune responses to updated COVID-19 booster vaccines remain incompletely characterized. Here, we administered an XBB.1.5 trivalent recombinant protein booster (WSK-V102C) to 22 individuals (<38 years) and 20 individuals (≥73 years), all of whom had previously received 2-3 doses of inactivated COVID-19 vaccines. Neutralizing antibody responses against multiple SARS-CoV-2 variants were quantified and compared between age groups. Meanwhile, single-cell RNA sequenci

Medial pivot designs result in improved patient reported outcome measures and range of motion when compared to cruciate retaining total knee replacements: A systematic review and meta-analysis.

BACKGROUND: Knee arthroplasty remains one of the most important treatment options in improving quality of life for patients with end-stage knee osteoarthritis. However, roughly 20% of patients remain dissatisfied with their outcome. Perceived implant instability and range of motion are factors that may contribute to dissatisfaction. The medial pivot (MP) total knee replacement (TKR) is postulated to provide increased stability due to greater implant conformity and replication of anatomical function compared to cruciate retaining (CR) implants. This systematic review and meta-analysis evaluates

Ixekizumab for the treatment of psoriatic arthritis: an Italian multicentric retrospective observational study.

Ixekizumab (IXE), an IL-17 A inhibitor, demonstrated efficacy in clinical trials in patients with psoriatic arthritis (PsA), and favorable data have emerged from real-world evidence studies on psoriasis as well. However, real-world data specific to PsA remain limited. This study aims to assess IXE effectiveness in reducing disease activity in patients with PsA and determine its drug retention rate (DRR) over 24 months. The secondary aim is to identify factors potentially affecting long-term persistence on therapy. A retrospective observational study was conducted. Consecutive adult patients me

A Division-Associated Envelope Protein, MAB_2363, Drives Intrinsic Resistance and Virulence in Mycobacterium abscessus.

Mycobacterium abscessus exhibits intrinsic resistance to conventional antibiotics, significantly limiting treatment options. Our previous studies established that MAB_2362 (SteA) is a key regulator of cell division that contributes to intrinsic resistance and virulence. Considering that SteA-like proteins often act alongside SteB counterparts, we hypothesized that the adjacent gene MAB_2363 encodes the corresponding SteB-like division regulator. In this study, we found that deletion of MAB_2363 significantly increased susceptibility to multiple antibiotics and disrupted cell wall permeability.

Effect of prehabilitation before total knee replacement on postoperative patient-reported joint awareness, enablement and knee function: protocol for the PROTEKT randomised controlled trial.

INTRODUCTION: Knee osteoarthritis (OA) causes pain, reduced function and disability and may require total knee replacement (TKR). Although TKR is effective, up to 20% of patients remain dissatisfied, partly due to poor preoperative function and unrealistic expectations. Long waiting times for surgery may worsen patients' function, yet preoperative physiotherapy is rarely offered. Prehabilitation-exercise and education before surgery-could improve postoperative recovery, but current evidence is limited. This trial investigates whether adding prehabilitation to standard care before TKR improves

Global patterns and predictors of initial treatment in early rheumatoid arthritis: insights from a multinational machine learning study.

BACKGROUND: Rheumatoid arthritis (RA) treatment guidelines recommend early initiation of disease-modifying antirheumatic drugs (DMARDs), but actual prescribing decisions are influenced by multiple clinical and contextual factors. Machine learning (ML) offers a promising tool to uncover patterns in treatment selection and support personalized decision-making. OBJECTIVES: To identify the most important predictors of initial treatment in patients with newly diagnosed RA using ML algorithms applied to an international registry. METHODS: We conducted a secondary analysis of 16,684 patients from t

Management of Lower-Extremity Deformity in Arthrogryposis Multiplex Congentia: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) is a highly heterogeneous constellation of disorders defined by non-progressive congenital multiple joint contractures, typically manifesting across multiple limbs. The pathology results in significant functional impairment, including restricted range of motion, chronic arthralgia, and secondary musculoskeletal deformities like scoliosis. Given that AMC is an umbrella designation encompassing over 300 distinct etiologies, the profound clinical variability poses substantial diagnostic and therapeutic challenges. The relative rarity of AMC and the absence

Anatid herpesvirus 1 UL51 protein is palmitoylated by DHHC9 and DHHC18 for viral replication and virulence.

The tegument protein UL51 is conserved among herpesviruses, but the mechanisms underlying its role in viral pathogenesis remain unclear. In this study, using Anatid herpesvirus 1 (AnHV-1) as a model, we show that deletion of UL51 markedly attenuates viral replication and virulence in ducks. Immunoprecipitation coupled with LC-MS/MS identified interactions between pUL51 and multiple viral proteins, among which the association with pUL10 depends on pUL51 palmitoylation. This modification promotes pUL10 stability and its localization to the Golgi apparatus. Further analysis revealed that the palm

Fall risk prediction in older adults: a generalized linear mixed-effect model analysis based on physical performance measures.

BACKGROUND: Falls are a major cause of morbidity, loss of independence, and mortality among older adults, particularly those with osteoarthritis (OA). Traditional fall-risk assessments often rely on categorical thresholds of physical performance tests, which may not capture the continuous and multifactorial nature of fall risk. OBJECTIVES: This study aimed to develop a probabilistic model for fall-risk prediction in older adults by examining the association between continuous physical performance measures such as the 5-Times Sit-to-Stand (5-STS) test, gait speed, lower-limb power, and key cat

A co-created randomised controlled feasibility trial of a peer-led complex oral health intervention in UK secondary schools: the RAISED In Yorkshire RiY study protocol.

BACKGROUND: Tooth decay in secondary school children is one of the most prevalent non-communicable diseases in the UK and globally. Whilst almost entirely preventable, oral health inequalities remain a significant public health problem to reach and manage those most vulnerable. School-based prevention strategies targeted to localities with oral health inequality, and incorporating peer-to-peer interventions warrant further evaluation. These peer-to-peer interventions have a bidirectional benefit and are proposed to positively influence both the 'peer leader' and 'peer recipient'; they rely on

Joint association of depression and physical activity with incident cardiometabolic multimorbidity (CMM) among middle-aged and older adults in China.

Depression and physical inactivity are both associated with cardiometabolic disorders; however, their joint effects on the development of CMM, defined as the coexistence of ≥ 2 cardiometabolic conditions, remain unclear. We aimed to investigate the combined association of depression and vigorous physical activity (VPA) with incident CMM and to examine whether this association differed across metabolic status. This prospective cohort study included 2,661 adults aged ≥ 45 years without CMM at baseline (2011) from the China Health and Retirement Longitudinal Study. Participants were followed thro

Impact of laparoscopic sleeve gastrectomy on obesity-related comorbidities in patients over 65 years: a prospective comparative study.

BACKGROUND: Obesity in adults over 65 years is increasingly prevalent and linked to comorbidities like type 2 diabetes (T2D), hypertension (HTN), osteoarthritis (OA), and obstructive sleep apnea (OSA), where conservative treatments often fail. Laparoscopic sleeve gastrectomy (LSG) effectively treats severe obesity, but prospective comparisons with non-surgical management in this age group are limited. METHODS: This prospective, non-randomized, patient-preference cohort study enrolled 60 patients aged ≥ 65 years with BMI ≥ 40 kg/m² and ≥ 1 comorbidity (T2D, HTN, OA, or OSA). Group A (n = 30) un

Do Hip Precautions Matter After Posterior Approach Total Hip Arthroplasty With Capsular Repair? A Randomized Control Trial.

BACKGROUND: The clinical efficacy of posterior hip precautions in preventing dislocation after primary posterior approach total hip arthroplasty (THA) remains uncertain. Such mobility restrictions may delay patients' return to daily activities and functional recovery. The aim of this study was to determine whether THA hip precautions impact hip dislocation rates. METHODS: We conducted a prospective, randomized study evaluating the effect of hip precautions on the risk of dislocation after primary THA. A total of 1,133 patients were enrolled and underwent posterior approach primary THA by mult
