Effect of photobiomodulation on fatigue-related errors in ankle joint position reproduction: the moderating role of perceived ankle instability in a randomized, placebo-controlled trial.

To investigate whether pre-exercise photobiomodulation (PBM) attenuates fatigue-induced decline in ankle joint position reproduction (JPR) and determine whether perceived ankle instability modifies this response. In this randomized, placebo-controlled trial with participant blinding and blinded outcome assessment, 30 healthy young adults (age: 24.3 ± 3.7 years; 15 male, 15 female) received either PBM or placebo treatment applied to the lower-leg musculature before a fatiguing protocol. PBM parameters included a wavelength of 830 nm, average power of 1 W, and total energy of 300 J per limb. Ank

RNA G-Quadruplexes in the Porcine Deltacoronavirus Genome: Structural Regulation and Therapeutic Targeting for Antiviral Strategies.

Porcine deltacoronavirus (PDCoV) is an emerging enteric coronavirus that poses a considerable risk to swine production worldwide. G-quadruplexes (G4s) are noncanonical nucleic acid secondary structures known to regulate critical steps of viral life cycles in multiple viruses; however, the existence and biological role of RNA G4s (RG4s) in PDCoV remain unclear. In this study, we identified putative G4-forming sequences (PQSs) in the nonstructural proteins 2 (Nsp2) and M genes of the PDCoV genome, which are highly conserved across PDCoV strains, based on bioinformatic analysis. Fluorescence turn

Biomarkers of treatment response in rheumatoid arthritis: from conventional markers to tissue immunophenotype.

Rheumatoid arthritis (RA) is a biologically heterogeneous immune-mediated disease characterized by substantial variability in therapeutic response. Despite the availability of multiple conventional synthetic, biologic, and targeted synthetic disease-modifying antirheumatic drugs (DMARDs), many patients fail to achieve adequate disease control or experience secondary loss of efficacy, underscoring the need for predictive biomarkers that can guide treatment selection. This narrative review was based on a structured literature search of PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar,

Bilateral Sternoclavicular Joint Dislocation with Inferior Sternal Migration: Surgical Management and One-Year Outcome.

Bilateral sternoclavicular joint (SCJ) dislocation is an exceptionally rare injury, particularly when associated with inferior migration of the sternal body and multiple rib fractures. We report the case of a 51-year-old male who sustained high-energy thoracic crush trauma resulting in bilateral SCJ dislocation, inferior sternal displacement, rib fractures, and a Grade I open humeral shaft fracture. Surgical management required a multidisciplinary approach including SCJ reduction, ligament reconstruction with a semitendinosus tendon graft in a figure-of-eight configuration, clavicular fixation

The diagnostic accuracy of the PHQ-9 in primary care in relation to the presence of single or multiple depression-related risk factors.

BACKGROUND: International guidelines recommend risk-adapted depression screening in primary care. However, empirical evidence on the diagnostic accuracy of depression screening questionnaires in patients at risk for depression remains limited. OBJECTIVE: To evaluate the diagnostic accuracy of the Patient Health Questionnaire-9 (PHQ-9) for detecting major depressive disorder (MDD) in primary care, stratified by the presence of single depression-related risk factors and the amount of risk factors. METHODS: This secondary analysis used data from 985 primary care patients participating in the GE

Growth Hormone-Insulin-like Growth Factor Axis and GDF-15 in Critical Illness: Implications for Survival Stratification.

Background: The growth hormone (GH)-insulin-like growth factor (IGF) axis is profoundly dysregulated in critical illness. GDF-15 and individual IGF-binding proteins (IGFBPs) have separately been proposed as prognostic biomarkers, but to our knowledge, no prior study has simultaneously characterized all major GH-IGF axis components and GDF-15 in the same critically ill cohort, precluding assessment of their joint intercorrelation structure. Aim: To provide the first simultaneous characterization of the intercorrelation structure among ten GH-IGF axis components and GDF-15 in a single ICU cohort

Editorial Commentary: Knee Hyperextension Injuries: What Are We Really Diagnosing and How to Solve the Problem?

Knee hyperextension injuries remain a poorly defined entity, often functioning more as a descriptive label than a true mechanistic diagnosis. Rather than representing isolated sagittal-plane events, these injuries reflect the final position of complex multiplanar loading patterns, resulting in highly variable structural damage that frequently extends beyond the cruciate ligaments. Emerging clinical observations suggest that patients with subtle hyperextension mechanisms may present with persistent pain and instability despite largely normal imaging and standard ligament testing. This highlight

The association of objective gait measures with patient-reported outcome measures and physical performance following total knee arthroplasty: A systematic review.

BACKGROUND: Gait analysis, patient-reported outcomes (PROMs), and performance measures are used to assess outcomes following total knee arthroplasty (TKA). Comprehensive understanding of the relationship between gait measures from three-dimensional (3D) motion analysis and clinical outcomes, including falls, remains unclear. This review aims to explore the association of objective gait measures with PROMs and physical performance. METHODS: Using standardized systematic review methodology, 4 databases were searched from inception to April 2025. Eligible studies reported gait measures alongside

Mediators of the effects of an HIV prevention advocacy training intervention on prevention advocacy among persons living with HIV in Uganda.

Pathways through which advocacy training enhances engagement in advocacy among persons living with HIV (PLWH) are not well understood. In a secondary analysis from a randomized controlled trial of a peer advocacy training intervention for PLWH, we examined mediators of intervention effects on HIV prevention advocacy (i.e., encouraging others to engage in HIV protective behaviors) across members of their social network. A sample of 210 PLWH (105 in each of the intervention and control arms) provided information about their advocacy to their social network members at baseline, and 6-, 12-, and 1

CoLchicine for Treatment of OsteoArthritis of the Knee (CLOAK): Clinical and biochemical outcomes from a three-month double-blind, placebo-controlled study.

OBJECTIVE: Knee osteoarthritis (KOA) causes pain and progressive disability, but pharmacologic treatments are limited. Colchicine inhibits inflammation that might modulate KOA, but efficacy trials have yielded mixed results. We tested whether colchicine, without concurrent NSAIDs, improved KOA pain, function, synovial effusion size, and OA-associated inflammatory serum biomarkers. METHODS: Participants with symptomatic KOA and radiographic Kellgren-Lawrence grades 2/3 were randomized to receive three months of daily colchicine or placebo in a double-blind manner, with no concurrent NSAID use.

Clinical observations, environmental parameters, and gut microbiota alterations associated with gas bubble disease in the endangered Anji salamander (Hynobius amjiensis).

Gas bubble disease (GBD) is a non-infectious disease caused by the supersaturation of total dissolved gas (TDG) in aquatic environments, which can damage multiple tissues and organs in aquatic animals and be potentially fatal in severe cases. This study reports, for the first time, clinical symptoms resembling GBD in wild larvae of the endangered Anji salamander (Hynobius amjiensis). By combining clinical observations, habitat water quality testing, and 16S rRNA sequencing, we investigated the ecological drivers of this disease and its impact on the host's fecal microbial community. Affected i

Feasibility, acceptability and preliminary impact on diet quality of the Healthy Weight for Life™ + medical nutrition therapy; a randomised controlled pilot study.

BACKGROUND: Poor diet quality has been associated with greater risk of people developing overweight and obesity, type 2 diabetes, cardiovascular disease, and osteoarthritis. For each of these conditions, weight management is recommended in respective guidelines. Current weight management interventions, such as the Healthy Weight for Life™ (HWFL) program that uses meal replacements, have not been assessed for impact on diet quality. Further, the role of a dietitian providing medical nutrition therapy (MNT) in conjunction with the HWFL program has not been evaluated. MNT is an evidence-based app

Type 2 diabetes is not associated with excess risk of periprosthetic joint infection in obese patients undergoing total hip arthroplasty.

BACKGROUND: Obesity and type 2 diabetes (T2DM) are considered risk factors for complications following total hip arthroplasty (THA). How these risk factors separately or in combination influence the risk of reoperation due to periprosthetic joint infection (PJI) or all-cause reoperation is not fully known. We sought to investigate the 2-year risk of reoperation following THA due to osteoarthritis (OA) among people with and without the metabolic risk factors, T2DM and obesity. METHODS: We linked all patients with THA registered during 2008–2019 in the Swedish Hip Arthroplasty Register (SHAR) to

Probiotics and palmitoylethanolamide (PEA) for osteoarthritic pain: individual effects in a multiple baseline design study.

BACKGROUND/OBJECTIVES: Osteoarthritis (OA) is a leading cause of chronic pain. As healthcare services struggle to meet demand to treat chronic pain, many individuals self-manage their symptoms. Probiotics and palmitoylethanolamide (PEA) may be promising therapeutic options because of their anti-inflammatory properties. PEA is an endogenously produced N-acylethanolamine, considered "endocannabinoid-like" due to its structural similarity to endocannabinoids. While it has non-direct influence on the endocannabinoid system, it primarily acts via non-cannabinoid pathways, most notably through activ

A case of AL amyloidosis-associated arthritis initially misdiagnosed as rheumatoid arthritis.

Excessive production of immunoglobulin light chains can result in amyloid light-chain (AL) amyloidosis, which manifests as arthritis in ~3.7% of cases. Myeloma-associated amyloid arthropathy, a musculoskeletal manifestation of AL amyloidosis, often mimics rheumatoid arthritis, making an accurate diagnosis challenging. Musculoskeletal ultrasonography has emerged as a valuable tool in the differential diagnosis of arthralgia; however, the specific sonographic features of AL amyloidosis-related arthritis remain poorly characterised. We present a case of AL amyloidosis-associated arthritis initial

Significance of a 4-week home-based prehabilitation program in accelerating 3-month recovery post total knee arthroplasty: a retrospective cohort study.

BACKGROUND: Preoperative rehabilitation is one of the strategies for enhanced recovery after surgery (ERAS) following total knee arthroplasty (TKA), but the optimal duration remains inconclusive. This study aims to evaluate the impact of a 4-week home-based prehabilitation program on accelerating postoperative recovery in patients undergoing TKA. METHODS: In this retrospective cohort analysis, 176 patients undergoing primary unilateral TKA were categorized into two groups: those who completed a 4-week home-based prehabilitation program (training group, n = 72) and those who did not (control g
