The Genetics of Generalized Osteoarthritis (GOGO) study: study design and evaluation of osteoarthritis phenotypes.

PURPOSE: The primary goal of the Genetics of Generalized Osteoarthritis (GOGO) study is to identify chromosomal regions associated with increased susceptibility to generalized osteoarthritis (OA). Here we describe the study design and phenotype of the 2728 participants from the 1145 families recruited for this study. METHODS: GOGO is an investigator-initiated collaboration involving seven clinical academic sites and sponsored by GlaxoSmithKline. Family ascertainment was carried out between 1999 and 2002. A qualifying family required self-reported Caucasian ethnicity and at least two affected

Bilateral varus osteotomies in hip deformities: are early interventions superior? A long-term follow-up.

Coxa valga (with or withour excessive femoral anteversion) combined with acetabular dysplasia is a well-known cause of early osteoarthritis. Many authors have stated that the best result of an osteotomy can be achieved at an early stage of these osteoarthritic changes. In this study, we present 26 patients with a symmetrical hip deformity for which we performed a therapeutic osteotomy on the symptomatic hip. The contralateral hip had the same anatomical predisposition to develop an OA, but there were only minor to no complaints. We advised and performed an early osteotomy on these hips. On rad

[Long-term results of uncemented hip arthroplasty for dysplastic coxarthrosis].

UNLABELLED: The purpose of the study was to evaluate the long-term results of uncemented total hip arthroplasty (THA) in young patients (below 50 years of age) with dysplastic coxarthrosis. In the last 20 years, 862 primary uncemented THA, were implanted in our Institution. This surgery was performed in 459 patients. From this group 220 hips in 177 patients (167 women and 10 men), who had dysplastic coxarthrosis, were included in our study. The left hip was operated in 66 and the right one in 68 patients. Bilateral surgery was performed in 43 cases. The means age at the time of hip surgery was

Staged bilateral total hip arthroplasty using rough and smooth surface femoral stems with similar design: 10-year survivorship of 48 cases.

BACKGROUND: Various studies have reported good long-term results using femoral stems with either smooth or rough surfaces. In this retrospective cross-sectional survivorship study, we reviewed the 10-year results of 51 bilateral staged cemented total hip arthroplasties using the Harvard or the Charnley femoral stems-which have almost similar geometry but a different surface finish. METHODS: 51 patients were reviewed at median interval of 10 (Harvard group) and 11 years (Charnley group) after the primary operation. We evaluated cement mantle thickness, alignment of the components, presence of

[Bilateral hip joint replacement as a one-stage or two-stage procedure for dysplastic coxarthritis: a comparative analysis of 30 patients].

AIM: Dysplastic coxarthritis usually occurs earlier than primary coxarthritis. As a rule dysplastic coxarthritis mostly affects young and active individuals. Bilateral coxarthritis may lead to severe impairment requiring surgical replacement of both hip joints, as the patients are frequently dissatisfied with the substitution of one joint alone. METHOD: In a retrospective study, 15 patients with bilateral hip dysplasia who had received total endoprostheses in both hip joints in a one-stage procedure (group A) were compared to 15 patients with bilateral dysplasia who had been operated on bilat

Dependency issues in survival analyses of 55,782 primary hip replacements from 47,355 patients.

Artificial hip joints are used in only one hip for about 85 per cent of the patients and in both hips (bilateral) for about 15 per cent of the patients. The occurrence of bilateral prostheses and the influence they have in survival analyses of joint arthroplasties are seldom considered. In this study we therefore focus on issues related to bilateral primary hip prostheses, time to revision surgery, and some commonly used statistical methods. We used information from 47,355 patients with 55,782 primary hip prostheses reported to the Norwegian Arthroplasty Register between 1987 and 2000. Due to

The Otto Aufranc Award. Highly cross-linked polyethylene in total hip arthroplasty: randomized evaluation of penetration rate in cemented and uncemented sockets using radiostereometric analysis.

The annual wear rate in polyethylene articulations should be 0.1 mm or less to avoid future osteolysis. Highly cross-linked polyethylene shows an 80 to 90% wear reduction in hip simulator testing, but the clinical documentation of this new polyethylene is still inadequate. We evaluated the highly cross-linked polyethylene in two prospective randomized clinical studies. Thirty-two patients (12 men, 20 women; 64 hips) with a median age of 48 years (range, 29-70 years) with bilateral primary or secondary arthrosis of the hip had hybrid total hip arthroplasty with liners made of highly cross-linke

Contemporary total hip arthroplasty with and without cement in patients with osteonecrosis of the femoral head.

BACKGROUND: The rate of failure of primary total hip arthroplasty in patients with osteonecrosis of the femoral head is higher than that in patients with osteoarthritis. The purpose of this prospective study was to document the clinical and radiographic results of arthroplasty with so-called third-generation cementing and the results of second-generation cementless total hip arthroplasty in ninety-eight consecutive patients with osteonecrosis of the femoral head. METHODS: Fifty patients who had had simultaneous bilateral total hip arthroplasty with a cemented stem in one hip and a cementless

X-linked spondyloepiphyseal dysplasia tarda: molecular cause of a heritable disorder associated with early degenerative joint disease.

Spondyloepiphyseal dysplasia tarda (SEDT) is a genetically heterogeneous disorder often associated with the early onset of osteoarthrosis. The X-linked recessive form (SEDL) affects men and is characterized by reduced height, arm span exceeding total height, and barrel chest deformity. The radiographic phenotype comprises a hump-shaped deformity of vertebral bodies and mild epiphyseal dysplasia of the femoral head associated with early signs of hip arthrosis. The disorder is caused by mutations in the SEDL (or sedlin) gene on Xp22.12-p22.31. In 4 male patients from a German family, we identifi

Survivorship of hip prosthesis in primary arthrosis: influence of bilaterality and interoperative time in 45,000 hip prostheses from the Finnish endoprosthesis register.

We studied the influence of bilaterality and interoperative time on survivorship of the hip prosthesis. The material consisted of 45,000 (3,153 bilateral) total hip arthroplasties and 38,000 patients operated on for primary osteoarthrosis, using data from the Finnish Endoprosthesis Register between 1980 and 1999. Cox regression analysis showed that male sex, young age, uncemented prosthesis and time of surgery (first 10-year period) were significant risk factors for aseptic loosening. We found no difference between the survivorship of the first bilateral and that of unilaterals, but second bil

Prevalence of fat embolism following bilateral simultaneous and unilateral total hip arthroplasty performed with or without cement : a prospective, randomized clinical study.

BACKGROUND: Controversy exists regarding the safety of bilateral simultaneous total hip arthroplasty, in part because of the potentially higher prevalence of pulmonary fat embolism. The purpose of the present study was to determine if unilateral and bilateral simultaneous total hip arthroplasty procedures resulted in different prevalences of fat embolization, different degrees of hemodynamic compromise, or different levels of hypoxemia or mental status changes. METHODS: One hundred and fifty-six consecutive patients undergoing primary total hip arthroplasty were prospectively enrolled in the

Relationship between radiographic measurements of reconstructed hip joint position and the Trendelenburg sign.

We evaluated 60 limbs, including 34 primary total hip arthroplasties in 30 patients (mean age, 56 years) at a minimum of 2 years postoperatively. Femoral offset ratio (%FO) was calculated by dividing the femoral offset by the distance between the centers of the bilateral femoral heads on radiographs. The tilt angle of the pelvis by the Trendelenburg test was measured using the magnetic sensor system. In the limbs having a negative Trendelenburg sign after reconstruction, the %FO averaged 20.1% (range, 14.7% to 24.7%), and the tilt angle of the pelvis averaged +0.8 degrees (range, -1.0 degrees

[Spontaneous bilateral fractures of the femoral neck. Etiology-frequency-diagnosis-therapy].

INTRODUCTION: Medial and pertrochanteric fractures of the femoral neck occur commonly in patients with bone weakness due to osteoporosis or osteomalacia. Combined bilateral hip injury is much rarer. PATIENTS AND METHODS: The case of a Turkish woman with bilateral fractures of the femoral neck due to secondary hyperparathyroidism caused by nutritional vitamin-D 3 deficiency is described and compared to 80 cases of bilateral hip fractures mentioned in English and German literature. RESULTS: Until the late 1950's the most common reason for simultaneous bilateral femoral neck fractures was convu

[Individual cement-free total hip endoprosthesis in a patient with a rare form of dwarfism (Fuhrmann syndrome)].

We present a single case study of a 47-year-old female patient with a rare form of primary dysostotic dwarfism. This syndrome was first described by Fuhrmann in 1972. Seven years ago, this patient underwent bilateral cementless total hip arthroplasties for severe osteoarthritis about her hips. Custom-made components were used. While achondroplastic patients present with wide femoral medullary canals, it is clinically relevant that Fuhrmann Syndrome features narrow femoral medullary canals. This complicating anatomical factor in Fuhrmann syndrome as well as the increased anteversion angles were

Range of joint motion and disability in patients with osteoarthritis of the knee or hip.

OBJECTIVE: To establish the relationships between the range of joint motion (ROM) and disability in patients with osteoarthritis (OA) of the knee or hip. Two related issues were addressed: (1) the inter-relationships between ROMs of joint actions, and (2) the relationship between ROM and disability. METHODS: Data on 198 patients with OA of the knee or hip were used. The ROM was assessed bilaterally for the hip and knee, using a goniometer. Disability was assessed using a self-reporting method (questionnaire) and an observational method. Correlation and factor analysis were used to establish t

[Imaging of chronic hip pain in adults].

Adult hip pathologies are mainly represented by the degenerative disease, so called "osteoarthrosis, or more precisely coxarthrosis". The means of imaging are exposed, according to their specific value: X Rays (measurement of the characteristic angles of the adult hip), Arthrography, CT Scanner, Arthro-CT Scanner, MRI, Bone Scintigraphy, Ultrasonography. Clinical findings differentiate a mechanical syndrome and an inflammatory syndrome. The coxarthrosis is the most frequent, under two forms: primary (idiopathic) coxarthrosis and secondary coxarthrosis. Primary (idiopathic) coxarthrosis has a l
