Bilaterality in slipped upper femoral epiphysis.

The frequency of slipping and osteoarthritis of the contralateral hip was recorded in 260 patients with slipped upper femoral epiphysis between 1910 and 1960. Twenty-three of these patients (9%) had primary bilateral slipping, 32 (12%) had a contralateral slip diagnosed later during adolescence and a further 104 (40%) had signs of contralateral slipping at follow-up 16 to 66 years later, giving a total of 159 cases (61%) with bilateral slips. Of the 104 slippings diagnosed at follow-up, 25% showed osteoarthritis. It is concluded that, with a slipped epiphysis, prophylactic contralateral pinnin

Periarticular heterotopic ossification after total hip arthroplasty for primary coxarthrosis.

Periarticular heterotopic ossification (PHO) is a common roentgenographic finding, occurring in more than two-thirds of patients after total hip arthroplasty (THA) for coxarthrosis. In the present study, 56 patients treated with bilateral THA were analyzed to determine the correlation between heterotopic ossification on the two sides. A strong correlation was found between the grade of PHO on the two sides: patients who developed severe PHO after the first THA invariably developed considerable PHO after surgery on the other side. The incidence and grade of PHO were higher in men than in women.

Bilateral hip and knee arthroplasty.

Twenty-six severely deformed patients with prosthetic arthroplasty of both hips and both knees because of rheumatoid arthritis (16), juvenile rheumatoid arthritis (6), ankylosing spondylitis (2), osteoarthritis (1), and inflammatory arthritis, cause unknown (1), were followed for a mean of 6.8 years. Severe pain was the primary indication for surgery, and consistent and substantial pain relief was achieved. Most patients also had limited ambulatory and functional improvement. Computer analysis showed that this improvement was related only to preoperative walking and function and a combined hip

Radiographic evaluation of patients with Bechterew's syndrome (ankylosing spondylitis). Findings in peripheral joints, tendon insertions and the pubic symphysis and relations to non-radiographic findings.

One hundred and twenty-two consecutive hospitalized patients aged 35 years or more, and with definite radiographic bilateral sacro-iliitis were re-examined. All but 3 met established criteria for definite ankylosing spondylitis (AS). All peripheral joints from 114 of these patients were radiographed, and definite arthritis in one or several joints was demonstrated in 47 (41%) of them. The distribution pattern of arthritis was generally symmetric or slightly asymmetric. Articular new bone formation was a common finding in arthritic joints. The frequency of patients with hip arthritis was low (2

The influence of total hip replacement on hip pain and the use of analgesics.

The influence of total hip replacement (THR) on pain and use of analgesics was evaluated in 511 patients with McKee-Farrar and Brunswik prostheses. The mean age of the patients was 63.8 years and the mean follow-up time 4.2 years. The average grade of pain decreased from 1.8 to 4.9 as evaluated according to Charnley. The proportion of regular users of all analgesics decreased from 75% to 17% (P less than 0.001). Indomethacin was the most common preoperative drug and the proportion of its regular use decreased from 47 to 12% (P less than 0.001). The preoperative association which existed betwee

Prevalence of coxarthrosis.

In a survey of 4027 double-contrast colon roentgenograms, the hip joints were examined and classified with regard to the presence or absence of primary coxarthrosis. The hips were graded according to type and severity of coxarthrosis. The data collection was performed in precisely the same manner as in an earlier study undertaken on 3903 similar examinations that had been performed more than 20 years earlier. During this time period, the prevalence had not changed, nor was there any change in sex ratio or distribution between bilateral and unilateral cases or between types of coxarthrosis. Als

Anterior approach to hip arthroplasty.

Direct anterior approach to the hip joint utilizing a curved transverse skin incision and splitting the tensor fascia muscle longitudinally provides an effective exposure for carrying out primary total hip arthroplasty. Excellent visualization of the acetabulum is afforded by this direct approach. Modified femoral rasps of varying lengths are employed to rasp the femur with the leg in external rotation. Trochanteric osteotomy was carried out on only three occasions in a series of 104 procedures performed on 85 patients during a three-year period. The duration of operation averaged 65 minutes.

Ring Y chromosome without mosaicism.

A 53-year-old man with a short stocky build, mild mental retardation, gynecomastia and hypogonadism was found to have a small ring Y chromosome unassociated with mosaicism. The ring Y was represented by a minute portion of chromatin or sometimes paired dots which were no larger than the short arm of the normally expected Y. No brightly fluorescent segment of the ring Y was present nor was it observed elsewhere in the karyotype. A primary medical problem was severe osteoarthritis, necessitating bilateral hip arthroplasties. Plasma testosterone was markedly decreased and plasma gonadotropins wer

Primary osteoarthrosis of the knee or hip. Prevalence of Heberden nodes in relation to age and sex.

One hundred twenty probands with primary osteoarthrosis of the hip or knee were examined prospectively; there were 194 controls. In addition, 193 patients with hip or knee arthrosis were examined retrospectively. Restriction of rotation on physical examination accurately differentiated arthrotic hips from normal ones. Among women, 38% of the controls and 49% of the probands had Heberden nodes (P more than .1). Their prevalence increased with age. Individuals with multiple arthrosis joints were not older than those with single joint involvement, and they has a similar prevalence of nodes. Of th

Studies in osteoarthritis of the hip. I. Classification.

A definition and classification of primary OA hip is presented based on radiological criteria. Most cases of unilateral idiopathic OA hip remain unilateral over the years and the majority of these show a distinct radiological pattern with articular cartilage loss in the superolateral area of the joint. In contrast, OA hip with loss of cartilage in the medial area is usually bilateral. A supermedial form appears to be an intermediate type, but is less well defined. It is suggested that superolateral and medial forms of OA hip are fundamentally different in nature. Leg-length disparity, to be dis
