Temporary in Situ Pinning With Subsequent Modified Dunn is a Safe Alternative to Primary Modified Dunn in Moderate and Severe Slipped Capital Femoral Epiphysis.

BACKGROUND: Surgical hip dislocation with modified Dunn procedure (MD) is a viable treatment option for moderate to severe slipped capital femoral epiphysis (SCFE). In the acute setting, an experienced surgeon may not be immediately available for the MD. However, in situ pinning (ISP) is a well-accepted initial treatment and may be more familiar to the on-call orthopaedic surgeon. Our aim was to compare outcomes at short and midterm follow-up in patients with moderate/severe SCFE undergoing ISP with subsequent MD versus primary MD. METHODS: Between June 2017 and July 2024, we treated 29 patie

Spine Surgery First Is Associated with Greater 1-year HOOS Improvement in Patients with Hip-spine Syndrome.

STUDY DESIGN: Retrospective Cohort Study. OBJECTIVE: This study aimed to: (1) compare demographic differences between spine-first and hip-first patients; (2) identify differences in perioperative outcomes; (3) determine whether surgical sequencing between hip and spine surgery influences patient reported outcomes (PROMS). SUMMARY OF BACKGROUND DATA: Hip-spine syndrome refers to coexisting lumbar degenerative stenosis and hip osteoarthritis. Surgical counseling remains challenging due to the symptomatic overlap between hip and spine disease, and there is no clear consensus on which disease sh

Differences in Baseline Demographic Characteristic and Complexity of Patients Based on Distance Traveled to Academic Center for Surgery.

BACKGROUND: Many patients undergoing total hip (THA) or knee arthroplasty (TKA) require specialty surgical care. Therefore, referral to tertiary academic medical centers may increase, but may predispose for worse outcomes. This work sought to assess baseline characteristics as well as 90-day and 1-year complication rates for patients based on the distance traveled for surgery. METHODS: A retrospective review of patients who underwent primary THA or TKA at a tertiary care center between 2017 and 2020 was completed. Patients who underwent arthroplasty for fracture, revision arthroplasty, or lac

A Shorter Surgical Interval May be Associated with Improved Patient-reported Outcomes in Hip-spine Syndrome.

STUDY DESIGN: Retrospective Cohort Study. OBJECTIVE: This study aimed to: (1) identify demographic differences based on the interval between surgeries; (2) compare differences in perioperative outcomes; (3) determine whether the interval between hip/spine surgery influences patient-reported outcomes (PROMs). SUMMARY OF BACKGROUND DATA: The presence of concurrent hip and spine disease in hip-spine syndrome creates a unique challenge in surgical decision-making given overlapping disease, with ideal timing between index spine and hip procedures remaining unclear. METHODS: A retrospective revie

Natural history of sleep before and after total joint arthroplasty is poor and unimproved by melatonin: A randomized controlled study.

INTRODUCTION: Poor sleep has been shown to affect surgical recovery with negative impacts on function and pain. Osteoarthritis can negatively impact sleep, as can surgery, making arthroplasty patients especially at risk for these ill effects both before and after surgery. Our study goals were to monitor sleep recovery and determine if perioperative melatonin could improve sleep quality compared to a placebo. METHODS: From 2020 to 2023, a cohort of 48 primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) patients were randomized to melatonin (n = 23) or placebo (n = 25) for 7

Defining Clinically Meaningful Improvement Thresholds for Inpatient Vs Outpatient Medicare Total Hip Arthroplasty Patients: A Reassessment of Centers for Medicare and Medicaid Services Patient-Reported Outcome Performance Measure Policy Benchmarks.

BACKGROUND: The Centers for Medicare and Medicaid Services Patient-Reported Outcomes Performance Measure defines substantial clinical benefit (SCB) on the Hip Disability and Osteoarthritis Outcome Score for Joint Replacement (HOOS-JR) as a 22-point improvement for all Medicare patients undergoing total hip arthroplasty (THA). Because inpatient and outpatient THA patients differ at baseline, a single threshold may not fully reflect substantial improvement across care settings. PURPOSE: We sought to determine separate HOOS-JR SCB thresholds for inpatient and outpatient Medicare THA patients. M

Higher Area Deprivation Index Is Associated With Greater Practice-initiated Perioperative Communication Workload in Patients With Primary Total Joint Arthroplasty.

BACKGROUND: Although deficits in social determinants of health (SDOH) have been previously associated with adverse clinical outcomes after primary THA and TKA, their role in the perioperative communication workload remains poorly characterized. Even though it remains essential to appropriately identify and address modifiable SDOH before a procedure, orthopaedic practices must also have the resources to handle the coordination of care effectively. Understanding how deficiencies in SDOH can impact communication workload would help support effective resource planning and equitable patient engagem

Long-term outcomes of the modified Dunn procedure in moderate and severe slipped capital femoral epiphysis: a prospective case series with 7-year follow-up.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescents and may lead to femoroacetabular impingement, early osteoarthritis, and long-term functional disability if inadequately treated. While in situ pinning remains the standard treatment for mild slips, it fails to correct the deformity in moderate and severe cases, potentially predisposing to degenerative changes. The modified Dunn procedure (MDP) was developed to restore proximal femoral anatomy through surgical hip dislocation while preserving vascular supply. The aim of the study is to evaluate

Feasibility, acceptability and preliminary impact on diet quality of the Healthy Weight for Life™ + medical nutrition therapy; a randomised controlled pilot study.

BACKGROUND: Poor diet quality has been associated with greater risk of people developing overweight and obesity, type 2 diabetes, cardiovascular disease, and osteoarthritis. For each of these conditions, weight management is recommended in respective guidelines. Current weight management interventions, such as the Healthy Weight for Life™ (HWFL) program that uses meal replacements, have not been assessed for impact on diet quality. Further, the role of a dietitian providing medical nutrition therapy (MNT) in conjunction with the HWFL program has not been evaluated. MNT is an evidence-based app

Radiological stability of a quadrangular uncemented femoral component compared with its shortened version in primary total hip arthroplasty : a prospective randomized controlled trial.

AIMS: Cementless femoral fixation is widely used in total hip arthroplasty (THA), with increasing interest in shorter femoral component designs. These femoral components aim to preserve proximal bone, promote physiological load transfer, and facilitate minimally invasive approaches. Among these, 'shortened' femoral components, derived from conventional stems and retaining a diaphyseal extension, may offer advantages in terms of implant positioning. However, clinical evidence regarding their mechanical reliability remains limited. This study aimed to compare the two-year radiological stability

Periacetabular osteotomy of the hip: an 8-year follow-up of 96 consecutive cases.

INTRODUCTION: A periacetabular osteotomy (PAO) is a joint-preserving surgical option for treatment of acetabular dysplasia. The procedure aims to prevent, or at least delay, the development of osteoarthritis, and subsequent need for total hip arthroplasty (THA). The conversion rate to THA differs widely in the literature, but most of the studies have few patients, and the follow-up time is often short for THA as an endpoint. The aim of this study was to evaluate the long-term outcome after PAO surgery with the rate of conversion to THA as the primary outcome. MATERIALS AND METHODS: Patients ≥1

Robotic-Assisted Versus Conventional Total Knee Arthroplasty: A Systematic Review and Meta-Analysis of Randomized and Quasi-randomized Controlled Trials Evaluating Perioperative and Radiographic Outcomes.

Robotic-assisted total knee arthroplasty (RATKA) has garnered attention in the field of orthopedic surgery. It has been developed to improve surgical precision and prosthesis alignment in comparison to conventional total knee arthroplasty (CTKA). It utilizes advanced robotic workflow systems as opposed to manual jig-based techniques. This review evaluated perioperative and radiographic outcomes to assess the overall safety and effectiveness of RATKA with CTKA. A comprehensive search for randomized and quasi-randomized control trials was conducted across 3 databases, PubMed/MEDLINE, Cochrane Li
