More than 90% of patients with so-called primary or idiopathic osteoarthritis of the hips in whom sufficient data were available to make an assessment of the normality of the hip joint at the cessation of growth clearly showed demonstrable abnormalities in the hip joint. The most common are mild acetabular dysplasia and/or pistol grip deformity. This latter deformity is associated with mild slipped capital femoral epiphysis (recognized or unrecognized at the time), Legg-Perthes' disease (recognized or unrecognized at the time), multiple epiphyseal dysplasia, spondyloepiphyseal dysplasia, and/o

Twenty-six severely deformed patients with prosthetic arthroplasty of both hips and both knees because of rheumatoid arthritis (16), juvenile rheumatoid arthritis (6), ankylosing spondylitis (2), osteoarthritis (1), and inflammatory arthritis, cause unknown (1), were followed for a mean of 6.8 years. Severe pain was the primary indication for surgery, and consistent and substantial pain relief was achieved. Most patients also had limited ambulatory and functional improvement. Computer analysis showed that this improvement was related only to preoperative walking and function and a combined hip

In 279 patients who had undergone total hip replacement because of primary coxarthrosis, the occurrence of fractures, other diseases, and roentgen examinations was compared with that in the same number of age- and sexmatched controls from the same population. Coxarthrosis patients had had significantly more fractures than the controls before their hip operation but did not have significantly more after the operation except for fracture distal to the femur implant, which was rare but occurred only in the patients who had undergone surgery. Fragility fractures, including hip fractures, were equa

Different body build has been observed in hypermobile women with coxarthrosis as compared with other hypermobile individuals. The study summarizes results of somatometric measurements in 38 hypermobile women with coxarthrosis based on congenital dislocation of the hip (CDH) and the same number of hypermobile controls. The following differences were found: Patients with coxarthrosis were heavier, shorter, more robust and had a larger quantity of fat tissue. In the control group ten out of 38 (26%) showed arachnodactyly, whereas in the coxarthrosis group no arachnodactyly was found. In Heath-Car

One hundred and eighty-five hips from 152 patients with primary or secondary osteoarthritis were studied in an attempt to assess the degrees of hip pain in contrast to radiological and other clinical findings. In 30.8% and 26.4% of the primary and secondary osteoarthritic hips respectively, hip pain showed some gradual decrease as time elapsed. Pain relief probably in association with osteophyte formation at the craniolateral acetabular rim occurred in 62.5% and 33.3% of the primary and secondary osteoarthritic hips respectively. Significant parameters observed in the primary osteoarthritic ca

Pelvic radiographs of 200 consecutive Japanese Oriental patients who were admitted for hip surgery at the Hospital of Kobe University in Japan were compared with those of 199 consecutive American white patients who were admitted for the same purpose to a New England hospital over a similar four and a half-year period between 1972 and 1976. One hundred and fifty-three Japanese Oriental and 157 American white patients had either primary or secondary osteoarthritis. The remainder had other types of hip pathology, such as avascular necrosis, rheumatoid arthritis, and ankylosing spondylitis. The ra

Surface replacement is an alternative procedure especially in younger patients who absolutely need hip replacement. Accurate diagnosis is imperative and the range of indications is naturally small. A follow up study of 44 patients (range, 10 months to 4 years) showed 2 loosenings (5%); other severe complications were not seen. In 39 (89%) hips was no more pain after operation and there was a distinct improvement of motility in 39 (89%) cases. Avascular necrosis should not be treated by surface replacement. Primary inflammatory diseases (rheumatoid arthritis) seem to have a higher incidence of

Fifty-six patients, whose average age was 64 years and who had primarily undergone resection of the femoral head with or without subtrochanteric angulation, were examined clinically and by radiographs at an average of 11.5 years after the operation. The functional activity of the patients, the mobility and stability of the operated hip joint as well as the resulting shortening of the leg and the radiographic results were assessed. The result was very good in 9 patients and in 36 patients was good or satisfactory. Eleven patients had poor results. On the other hand 52 of the patients were subje

A 10-year follow-up study was made on a consecutive series of 93 patients who underwent hip replacement with a metal-to-metal prosthesis - Müller's so-called self-lubricating total hip prosthesis - during the years 1967-1970. Primarily 106 hips were operated on. One-third of the patients died within the 10-year period. Of the other two-thirds (63 patients/75 primarily operated hip joints), 38 (57 per cent) still had their primary prostheses and 29 had undergone reoperation. In 18 of the reoperations a new prosthesis was inserted and in 11 the extraction of the primary prosthesis was not follow

The rate of post-operative dislocation after primary total hip arthroplasty in 1739 cases was 3.3 per cent; 0.9 per cent being classified as recurrent dislocations. Malposition of the socket, limb shortening or any other single technical factors could not be demonstrated to be responsible for post-operative dislocation of the hip. A previous osteotomy predisposed for dislocation, but in no case was this dislocation classified as recurrent. Cases with a previous cervical-femoral fracture were significantly more commonly dislocated compared with coxarthrosis cases. This fact probably could be ex

Osteoarthrosis of the hip (also known as coxarthrosis) as a result of a high-region dislocation of the hip, presents a difficult task for the orthopaedic surgeon, the solution of which is still under frequent and intense discussion. Total hip replacemnt (hip endoprosthesis) involves technical difficulties which result on the one hand, from the underdeveloped anatomic conditions (underdeveloped primary acetabulum with soft spongiosa on valgus position of the highly sclerosed secondary acetabulum associated with underdeveloped shaft of the femur); on the other hand, a repositioning is difficult

Although degenerative joint disease is an old and exceedingly common problem, clinical investigators have not reached a consensus regarding the etiology of this disease. Comparative osteological analysis of the knee, hip, shoulder, and elbow joints of 789 individuals from four human skeletal populations (Black and White Americans, Pueblo Indians, and Alaskan Eskimos) indicates that age of onset, frequency, and location of degenerative changes are directly related to the nature and degree of environmentally associated stress, as reflected by the variable life styles of the populations sampled.

Dysplasia of the hip is almost unknown in the Bantu peoples of Africa, except in cases of arthrogryposis and in those with partly European ancestry. Evidence is produced to show that the importance of intrinsic genetic factors in hip dysplasia far outweighs that of the extrinsic factors. These are of great importance only if allowed to act on a hip in which genetic factors have already determined dysplasia. Evidence is also produced to contradict the theory that the "piggyback" carrying method used by Bantu mothers is the reason for the immunity from dysplasia of the hip. The absence of "prima

A radiological review of two groups of intertrochanteric osteotomies of the femur for primary osteoarthritis of the hip has been made. Each group oroginally consisted of forty-one hips. In one group a Wainwright straight V-spline without compression had been used for fixation, and in the other group an AO angled plate with compression. The time for bony union was equal in the two groups but the incidence of non-union was lower in the AO group. Regression of cysts and of bone sclerosis was more frequent in the Wainwright group, possible as a consequence of the greater medial displacement and va

A retrospective study was carried out to determine what were the features of either success or failure of intertrochanteric osteotomy of the hip for degenerative hip disease. Seventy-three patients (78 hips) were reviewed clinically and roentgenographically with a follow-up of two to 12 years. The hips were classified into the superior, medial and intermediate types of the primary degenerative hip disease and into the dysplastic and coxa magna variety for the secondary degenerative hip disease and then further classified into Stage I and Stage II. Stage I was defined by the roentgenographic cr

Although congenital subluxation of the hip is an important primary abnormality, upon which OA hip may later supervene, other factors may also be important in individual patients. When osteoarthritis is seen with subluxation, it probably represents congenital subluxation and not displacement of the femoral head secondary to remodelling. Osteoarthritis in these cases is usually superolateral or supermedial in type, with the major impact of the disease in the superior articular cartilage. A similar form of osteoarthritis is seen in cases where leg-length disparity exists, with or without the pres
